When Lake Orion resident Judith Truscotttold her former dentist that local anesthesiadidn't numb her mouth like it should, he saidhe understood. He said he'd dealt with pa-tients with similar problems, and everythingwould be fine.

Judith wasn't so sure. In fact, she has amajor fear of the dentist, and her next ap-pointment would involve a root canal. Sheput her fears aside, though, and went to herappointment.

After the procedure, her dentist pushed uphis surgical mask, looked at her, and said ithad been a long time since he'd had to admin-ister that much anesthetic. Judith wasn't sur-prised.

"I told you so," she said as the dentistflagged her chart for future reference.

Judith wasn't satisfied. Afterward, sheavoided dental work for years. She had herteeth cleaned and attended regular checkups,but would not undergo the dental work sheneeded. Judith knew her oral health was de-teriorating, yet did not know how to over-come her fears.

"I had a major fear of the dentist," she said.But as a registered nurse, Judith also knew

that avoiding dental work could put her over-all health at risk. She understood the link be-tween poor oral health and heart attack,stroke, and other serious systemic problems.

"To be so afraid that it keeps you fromtaking care of your health is horrible," sherecalls. "If you do not have a fear, you can'tunderstand it. I was so apprehensive beforeI found Dr. Greenfield that I would cancelan appointment three times and just neverget the work done."

When Judith found Dr. Greenfield at LakeOrion Family Dentistry, her fears were putto rest. "He listened to me and explained mytreatment options, what he wanted to do,and why," Judith said, noting she felt in con-trol of her treatment plan with no pressure."Then I met with Karla, the nurse anesthe-tist at Dr. Greenfield's office, and she wasexcellent. Wonderful!"

In addition to an inability to numb withlocal anesthesia and an intense fear of dentalprocedures, Judith also suffers from acuteasthma. She asked questions most peopledon't know to ask, and Karla provided theanswers and explanation Judith needed.

After much discussion, Judith, Karla, andDr. Greenfield determined that two sessionsof dental procedures would be required. Judithrequested that the milder phase of treatmentbe completed first, and Dr. Greenfield waspleased to comply. Karla would administerIV sedation to relieve Judith's anxiety duringtreatment.

"I felt nothing, and I remember nothing,"said Judith. "There was no pain, no fear."

The procedure took just a short period oftime. Afterward, Judith had no anxiety aboutthe second phase of her treatment plan, whichwas scheduled just a few weeks later.

During the second appointment, Karla ad-

ministered IV sedation, and this time, Dr.Greenfield placed three crowns and performedlaser periodontal treatment.

"There was no bruising, no pain, not evenat the injection sites," Judith said. "In fact, Ileft the next day for a six-week Florida vaca-tion."

Judith ranks her experience a 12 on a scaleof 1 to 10.

"I was a basket case when I came in," shesaid. "They really made me feel good; theyare very attentive. I never feel like a numberat Dr. Greenfield's office, and my hygienist,Ronda, is wonderful."

Judith has referred her son and daughter-in-law to Dr. Greenfield, and she hopes hecan help others who are in her situation, filledwith fear and in desperate need of dental workto restore and protect their health.

Orthognathic surgery is needed when the jaws dont meet correctly. Teeth are

straightened with orthodontics and then corrective jaw surgery repositions

misaligned jaws. This not only improves facial appearance, but also ensures

that teeth meet correctly and function properly.

Who Needs Orthognathic Surgery?

People who can benefit from orthognathic surgery include those with an

improper bite or people with jaws that are positioned incorrectly. Jaw growthis a gradual process and in some instances the upper and lower jaws may

grow at different rates. The result can be a host of problems that can affect

chewing function, speech, long-term oral health and appearance. Injury to the

jaw and birth defects can also affect jaw alignment. Orthodontics alone can

correct bite problems when only the teeth are involved. However,

orthognathic surgery is required when there is a skeletal imbalance of the

upper and lower jaws.

Difficulty in the following areas should be evaluated:

difficulty chewing, biting or swallowing

speech problems

chronic jaw or TMJ pain

open bite

protruding jaw

breathing problems

Any of these can exist at birth or they may be acquired after birth as a result

of hereditary or environmental influences or the result of trauma to the face.

Before any treatment begins, a consultation will be held to perform a complete

examination with x-rays. During the pretreatment consultation process, feel

are fully informed about the aspects of your care, you and your dental team
