>Summer should be a time of carefree vacations,outdoor activities and backyard parties. Sadly, my jobas a health care professional is to ruin all of that.Summer is also the season for bug bites, allergiesand (hopefully not) anaphylaxis. The truth is that al-lergic reactions can happen at any time. Sometimesthe cause is known, but other times it is a mystery.This is nothing to worry about,really. With a little knowledge,seasonal and food allergies canbe treated at home. This month Ishall tell you about allergic reac-tions, starting from the minorand working up to the more se-vere.

There are many different kindsof allergic reactions, some benign and some serious.Your body responds to every foreign invader with thesame reaction. It notices something unwelcome inthe body and releases histamines which causesswelling and inflammation. This is a very simplifiedexplanation of what happens, but it is essentially theprocess. If you brush against some poison ivy yourbody responds to the sap by causing a local inflam-mation of the skin. Depending how your body re-acts, it could be a small rash or a massive blister. Whywould your body respond this way? It is the most ef-fective way to stop the spread of this foreign andpossibly dangerous substance. Your body respondsthe same way no matter what the allergen: pollen,poison oak, viruses, spider bite venom or shellfish.What differs is the severity and distribution in thebody.

Lets begin with how to treat minor, localized aller-

gic reactions. The classic example is, everyone'sfavourite, the mosquito bite. Your flying, buzzingfriend has a snack on your blood and leaves a bit ofvenom behind. The body notices this, releases hista-mines to cause inflammation and stop the venomfrom spreading. This leaves you with an itchy, redbump on your skin. There are many options for treat-

ing this itching. The first option isto ignore it as it will eventuallygo away on its own. If the symp-toms are minor, try applying atopical steroid cream to calm theallergic reaction. For a more nat-ural approach, try using Aveeno(oatmeal) cream or a homeo-pathic anti-allergy cream. Often

they work very well. Don't forget about the oldstandard, pink calamine lotion. Ask your PeoplesPharmacist about which anti-allergy cream would bebest for you.

Perhaps we should jump out of order of severityand talk about the most dreaded allergic reaction,anaphylaxis. This occurs when the allergic reactionbecomes severe and affects multiple areas of thebody. People can have an anaphylactic reaction toanything, but the most common culprits arepeanuts, medications, insect stings, latex andseafood. The symptoms are generally wide spreadand can include: hives, stomach upset, swelling ofthe tongue or throat, difficulty breathing or fainting.A reaction of this magnitude is gravely serious andyou should seek medical attention immediately.

The majority of people who could have an ana-phylactic reaction are aware of this condition and

Blood clots are a collection of sticky blood cells that form whena blood vessel is damaged. The body creates blood clots as a nor-mal response to blood vessel damage. The main job of a blood clotis to seal the leak in a damaged blood vessel. This prevents theblood from leaking out and protects the person from bleeding.

Clots (or thrombi) that block the arteries and prevent flow ofblood and oxygen to an organ can lead to areas of tissue damage(infarcts). When blood clots break (called an embolism) away fromthe area they're meant to protect, they can endanger other organs.

Clots that block blood flow are the main culprits in most heartattacks and strokes. They can also damage other organs:- When a blood clot (thrombus) forms in one or more arteriesthat supply blood to the heart, it blocks the blood flow to a partof the heart muscle, reducing or completely cutting off the oxy-gen supply to the cells in that area. As a result, the part of the heartmuscle that is deprived of oxygen dies, and a heart attack occurs. - Clots that block the flow of oxygen to the brain are the primarycause of strokes. - Clots that form in the eye may cause sudden blindness. Presence of an obstructing blood clot (thrombus) is referred to asthrombosis. Thrombosis in a vein is almost always associated withphlebitis (inflammation of a vein). Thrombophlebitis is an inflam-mation of a vein in the area where a blood clot has formed.

Thrombophlebitis is classified as either superficial or deep. Inother words, thrombosis can affect either superficial (surface) ordeep (below the surface) veins, causing thrombophlebitis.Superficial thrombophlebitis occurs when a blood clot affects veinsnear the skin surface, or superficial veins.

Deep venous thrombosis (DVT) occurs when a blood clot af-fects deeper, larger veins, such as those in the lower legs andthighs. DVT is more worrisome than superficial thrombophlebitis.These clots can break away (called emboli) from a blood vesseland cause a pulmonary embolism if they travel to the lung. (Formore information on pulmonary embolism, see the section "Symp-toms and Complications.") DVT is more common for people over40 years of age.Causes

Ischemic strokes (strokes caused by blood clots blocking an ar-tery that supplies the blood to the brain) can be due to blood clots

that have formed in the heart as a result of rhythm disordersknown as atrial fibrillation and atrial flutter.

Atrial fibrillation is a type of an irregular heartbeat (arrhythmia)that triggers a rapid, quivering beat in the upper chambers of theheart (the atria). This irregular pumping may cause some blood toremain in the heart chamber and form clots, which can then travelto the brain. Emboli can also form at the site of artificial heartvalves, after a heart attack, or as a result of heart valve disorder orheart failure.

Cardiogenic embolism refers to clots that form inside the heartand travel to the brain. A stroke may occur in someone who hassuffered a heart attack. A heart that has been damaged by a heartattack doesn't pump blood properly, which can cause formationof a blood clot that travels to the brain. The presence of artificialreplacement heart valves can also lead to blood clot formation. Bytaking blood-thinning medications, people with artificial heartvalves help to prevent clots from forming.

Some blood clots may form in a narrowed artery as a result ofatherosclerosis, commonly known as hardening of the arteries. Inthis case, the arterial walls slowly thicken, harden, and narrow overtime until blood flow is reduced. As a result, these arteries becomevulnerable to injury. If they tear, a blood clot may form, completelyblocking the already narrowed artery and shutting off oxygen topart of the heart or brain.

Blood clotting problems and rare blood disorders also causeblood clots to form, but the cause of blood clots is not alwaysknown.

For some women, the use of oral contraceptives (birth controlpills) may increase the risk blood clot formation. The risk is higherfor women over 35 years of age who smoke or have a history ofprevious blood clots.

A variety of things can cause inflammation of a superficial vein.One common cause is due to trauma or injury, for example fromsolutions or medications given intravenously (into a vein) in hos-pitals. Piercing the vein to give the solution or medication cancause irritation. Any trauma to a vein (such as an injury from a caraccident) will trigger inflammation in the area, leading to pain,discomfort, redness, and swelling. During this process, there is in-creased blood flow to the injured area, and a blood clot often

forms in the inflamed or injured area of the vein. Superficial throm-bophlebitis is an uncomfortable condition but rarely causes seriousproblems.

Sometimes, thrombophlebitis is caused by a bacterial infectionin the vein. The usual culprit is a bacteria called Staphylococcus,commonly found on the skin. In certain cases, thrombophlebitisdevelops without an obvious reason. It may develop in the legveins of pregnant women, in people with varicose veins, and insome people with cancer in the abdomen (particularly the pan-creas). Women over the age of 35 years who smoke and take oralcontraceptives (birth control pills) are at a higher risk of develop-ing blood clots.

DVT occurs when blood clots form in the deep veins of the legsor pelvis, and is often caused by:- prolonged sitting or bed rest- surgery or trauma (especially hip surgery, gynecological sur-gery, heart surgery) - medications such as estrogen, and birth control pills with higherlevels of estrogen - injury to the leg or immobilization (such as casting after a bro-ken bone)

Certain inherited conditions can make DVT more likely to occur.Blood flow in the veins depends on contraction of surroundingmuscles, and with inactivity, such as extended bed rest, the bloodstarts to collect and blood clots can easily form.Symptoms and Complications

Blood clots that result in a heart attack may cause chest pain(angina) that usually starts in the centre of the chest and moves tothe jaw, the back, the left arm, or occasionally the right arm. Lesscommonly, the pain may be felt in the abdomen.Heart attack pain is usually severe but not always. Some peoplehave "silent heart attacks" without any symptoms. Some peoplehave reported feeling a sense of impending doom as a heart attackcomes on. There's tightness and often a pounding in the chest.The heart may speed up and beat irregularly. Shortness of breathor difficulty breathing, nausea or vomiting, fainting, or collapsemay also occur.

Women may experience slightly different heart attack symp-toms than men. Women are more likely than men to have nausea,and less likely to have sweating as a symptom of a heart attack.Women may not feel the chest pain as distinctly as men. The mostcommon places for women's chest pain to spread to are the neck,jaw, or back.

Blood clots that result in a stroke usually cause symptoms on theopposite side of the body from where they are causing the block-age in the brain. This may result in loss of feeling on one side of the

face, arm or leg, or blindness. If the left side of the brain is affected,speech problems can occur. Affected people may be unable tospeak or to understand spoken words. Other symptoms of strokeinclude confusion, a severe headache, or sudden loss of coordina-tion or balance.

Signs of stroke should not be ignored, however brief or seem-ingly insignificant. Even if symptoms only last a few minutes andthen vanish completely, it is important to seek medical attentionright away.

Inflammation in superficial or surface veins (such as those usedto insert intravenous (IV) lines in the arm or to draw blood) pro-duces pain and discomfort but it usually isn't considered serious.Blood clots that form in superficial veins rarely break loose andtravel in the blood to cause blockage (thromboembolism) andcomplications in organs such as the lungs.In the case of DVT, a blood clot in the leg may cause pain, swelling,redness, and increased warmth. The leg may ache when stand-ing. This usually occurs in one leg only. While many people withDVT have no signs or symptoms, the classic symptoms are:- firm swelling- pain or tenderness over a vein - sharp pain when the foot is flexed upwards - redness - warm sensation over the affected area - dull, aching tightness in the calf, especially with walking - dilation (widening) of the surface veins of the leg

DVT can lead to serious complications. A blood clot that formedin deeper and larger veins, such as those of the legs, abdomen, andpelvis, can break away and become a travelling blood clot, or em-bolus. The embolus can travel and lodge in the lung, a conditioncalled pulmonary embolism.

Because a clot in a deep vein may not cause symptoms early on,the first sign may occur when the clot has broken loose and trav-elled to the lung. Symptoms of a pulmonary (lung) embolism arebreathlessness, chest pain, and bloody sputum. If you have any ofthese symptoms, get emergency medical care right away.Making the DiagnosisTests to check for presence of blood clots may include:- computed tomography (CT or CAT scan; a special techniquethat uses a computer to combine many 3-ray images into a de-tailed image of an area body that is 100 times more clear than aregular 3-ray)- magnetic resonance imaging (MRI)- ultrasound studies of leg veins or the arteries of the head & neck- angiograms or venograms- an ultrasound of the heart (echocardiogram)

I recommend that everyonewho has a friend or family member with an epineph-rine pen should know how to use it. I have a few tipsfor using adrenaline pens. The first is to hold the de-vice firmly in your hand and do not put your thumbon the end of the device. It is a natural thought to tryand apply more pressure upon injection. However, ifyou put your thumb on the wrong end, you risk in-jecting your thumb. There is often a concern aboutwhether the reaction is severe enough to use an in-jector. The current consensus is that if you are un-sure, it is best to give the injection anyways. Theallergic reaction could still be worsening. It is truethat these pens are expensive, but what is the cost ofpotentially saving a life? Another great idea is to reg-ularly check the expiry dates on your epinephrine in-jector pen, they generally expire every 16 months. Ifyou do have an expired pen, bring it into your Peo-ples Pharmacist for a demonstration of how to use it.I always make my patients practice injection tech-nique into a phone book. Please don't practice in-jecting an orange. These pens are powerful andoranges are small and soft. You might hurt yourselfand waste a nutritious orange. It is important thatyou know how to use an epinephrine inject pen,please visit your Peoples Pharmacist for a demonstra-tion.

Hopefully I have not scared you too much. Weshould mention those reactions that fall in the mid-dle, those that are troublesome but not life threaten-ing. The classic examples of these are seasonalallergies. Most of us experience symptoms of nasalcongestion, sneezing or watering eyes. As with all al-lergies, the exact cause might not be known. How-ever with seasonal allergies, the time of year whenyou are affected can be predicted. Congestion mightaffect you in early spring, or perhaps you suffer morein the fall. Whatever time of year it is, take notes onyour calendar as to when your allergies flare up. Thiswill tell you a few important things; when your al-lergies hit, what has worked and what has notworked. It you wish to use natural supplements,many of these supplements work best if they aretaken in advance. With this knowledge you can en-sure you have the items on hand needed for symp-tom relief.

Superficial thrombophlebitis is usually diagnosed according toyour symptoms. A doctor will take your medical history by askingabout your symptoms and conducting a physical examination.In the case of thrombophlebitis, an ultrasound of the suspectedveins may be done to confirm the diagnosis. Since the leg pain as-sociated with DVT is very similar to muscle pain, your doctor mightlook for signs of swelling and enlargement of the calf due toswollen leg veins.

A diagnosis of DVT is usually confirmed with a compression ul-trasound. Compression ultrasound detects differences in echoesor sounds made by flowing blood, and can easily detect the pres-ence of blood clots in deep veins.Treatment and Prevention

Medications are usually used to stop progression of DVT andprevent the blood clot from worsening, breaking away, and mov-ing to the lungs. If you think you may have DVT, seek medical at-tention right away.

Blood-thinning medications such as warfarin* or heparin areusually recommended. These medications may be continued forseveral months after a blood clot has been diagnosed. Most peo-ple do not require admission to a hospital to treat DVT, and thosewith DVT can usually return to normal activities within 2 to 3 weeks.

For some people, long-term treatment with warfarin (an anti-coagulant) may be necessary to prevent new blood clots fromforming. Your doctor may also recommend that you wear a com-pression support stocking on your leg to prevent DVT. Painkillersmay be used to reduce the pain.

To relieve mild inflammation and discomfort, the affected areashould be elevated and warm, moist packs applied for 15 to 20minutes at a time throughout the day. For people with superficialthrombophlebitis, activities such as walking are recommended. Ifthe inflammation and symptoms last longer than a day or two, orif symptoms become worse, see a doctor as soon as possible.In cases where the thrombophlebitis is due to an infection, treat-ment with antibiotics often takes care of the problem. In rare cases,when the antibiotics aren't enough to control the infection, surgi-cal removal of the inflamed portion of the vein may be required.

To help prevent DVT, avoid long periods of immobility such asthose during long car trips or airplane flights. Try to walk aroundand stretch for a few minutes every hour or so. Elevate your legsabove your heart level if possible, and if you have a history of bloodclots, wear support stockings or socks.

Prevention of blood clots is the best way to deal with the prob-lems of heart disease and stroke. It is important to reduce or quitsmoking and to control high blood pressure. High cholesterol lev-els also present a risk for blood clots and may be checked by yourdoctor. A healthy diet and regular exercise also help to reduce therisk of heart attack and stroke.

Focus On SunscreenHelps Protect Against Skin Cancer

Sunscreens help prevent the sun's ultraviolet (UV)radiation from reaching the skin. Two types of ultra-violet radiation, UVA and UVB, damage the skin andincrease your risk of skin cancer.

SPF – or Sun Protection Factor – is a measure of asunscreen's ability to prevent UVB from damagingthe skin. Choose a SPF of 30 or higher and a sun-screen that is water resistant.

The most common mistake with sunscreen usageis that most people do not apply enough sunscreenonto their skin. Most adults need at least one ounceof sunscreen, about the amount you can hold inyour palm, to fully cover all exposed areas of yourbody. It’s also recommended to reapply sunscreenat least every two hours to remain protected.

Whenever I’m asked about allergies, I recommendseveral lifestyle changes before using a supplement.I feel the best basic help for any condition is to drinklots of water. Extra water will make any nasal/sinussecretions thinner and easier to clear. Saline nasalrinses are also found to be helpful in removing pollenand nasal secretions. There is an easy to use sinusrinse product called ‘Sinus Rinse’ by Neilmed. Askyour Peoples Pharmacist for more information.Washing one's hair before bed is also a good idea.This will remove any pollens that are trapped in thehair and prevent them from falling onto the pillowduring the night. This way, you won't be sleepingwith a lot of pollen close to your face and nose. I rec-ommend changing the pillowcase every few days.Avoidance of certain allergenic foods will often help.The most common culprits are milk and other dairy,eggs, wheat, nuts and artificial dyes.

Another idea is to watch the newspapers orweather channel for your local pollen count. Thismight help you plan your week in order to avoiddays when your allergies might be at their worst.This may be especially helpful for people who exer-cise outdoors. Another tip, my wife mentioned tome, is the miracle of the Vicks inhaler and Tiger Balm.At night, when symptoms might be bad, place a LIT-TLE bit of Tiger Balm under the nose. The aromaticvapors will help to keep the nasal passages open soyou can rest better. Also, keep a glass of water byyour bedside, in case you wake up with a dry mouthin the middle of the night. This happens often withpeople who have allergies.

Summer and food allergies can be troublesomeand potentially dangerous for those affected. Tolearn how to treat summer and food allergies, visityour Peoples Pharmacist. They can help you choosethe right anti-histamine or supplement to treat yoursymptoms. Don't forget to write down what prod-ucts worked for your allergies in the past. If I had anickel for every time someone has said that theyhave forgotten what they took last year, that workedreally well, I wouldn’t have to write these articles any-more. I would still choose too, but I wouldn’t haveto. Also don't forget to check the expiry dates onyour adrenaline pen, and all your medications.

We live in a society where appearance is the platform bywhich self worth is measured. Media further objectifies thehuman body by using images of thin, athletic people to sellproduct; the overarching message is that a lean body is thegateway to health, happiness, love, wealth and popularity.Anyone who does not fit into the mainstream body imagebelongs in the “other” box. Never mind that few people be-long to the “in” group without plenty of hard work or sacri-fice and many of the images promoted by media are photoshopped beyond natural human form. This can set up an in-dividual to center primarily on perceived flaws and live theirlife through internalized deficiency dialogues based on notmeasuring up. The story that goes along with that thinkingis the “I will be happy when” story. True self-esteem is notbased on a future outcome, it comes from the honouring andacceptance of our own unique individual attributes and work-ing from that baseline.When it comes to body image and self-esteem we add fur-

ther insult to the equation with social shaming. Weightism,is a culturally induced disdain for the plus sizes and a by-prod-uct of the social emphasis on thin. Though we cannot denythe health risks attached to obesity, not everyone was born tobe a size zero. The stigma attached to being overweight con-tributes to a low self esteem loop that can actually exacer-bate the problem it was hoping to resolve. Shame oftendrives people deeper into the shadows where denial and fearget in the way of forging a healthier relationship with theirbody and body image.

Another sad truth is that our children are being pro-grammed with this message as soon as they are introducedto popular media. Movies and cartoons are not kind to over-weight individuals as they are often portrayed as villains,goofs, lazy and easy targets for public displays of humiliation.When did it become okay to discriminate against a group ofpeople based solely on body shape? Is this really the societywe want to create? I would much rather teach my childabout the diversity of the human form and all of the wonder-ful ways to support and celebrate their own unique body.

If you find yourself standing in front of the mirror and see-ing nothing but flaws, perhaps the following tips will helpreset your body image barometer and boost self acceptanceand self-esteem:

1. Beware of comparisons� There is always going to be ashort to a tall, a curly to a straight and a blue to a brown. Weoften only see the things that we would like to change aboutourselves and forget to celebrate and accentuate our ownbest attributes. Acceptance is the key. Be the best you thatyou can be�

2. Focus on that which you can change and let the otherstuff go. You cannot magically wake up tomorrow and be adifferent person but you could join a gym and tone up your5’ 3” frame or ask your hairdresser to work their own brandof magic on your hair. Dung out your closet and get rid of the“someday” clothes; buy clothes that not only feel good butlook great�

3. Treat your body as a temple and commit to your health.Poor health comes in all shapes and sizes. Starving yourselfto be a few pounds lighter or binge eating your way to com-fort are equally distressing. Choosing optimal health meansthat you start making food choices that nourish your body,mind and spirit instead of just feeding your appetite and in-securities.

4. Put an end to negative self talk. When the inner critic isspeaking, ask yourself if what the critic is saying is actuallytrue. We so often get bogged down with critical self talk thatwe actually start believing it. When you start to question it,you can actually begin to step away from self depreciation toself appreciation.

Your body is your temple. It makes no sense to be at warwith it. Be kind to your body and listen to what it is trying totell you. Marilyn Monroe had it right when she said, “want-ing to be someone else is a waste of the person you are.” Thetrue key to peace and happiness lies not in appearance but ac-ceptance. Lets work with what Mother Nature gave us ratherthan fight a battle that can never be won.

Microwave on high(100%) for about 3 minutes until softened (see Tip, page 133). Place stem-side upin greased 9 x 13 inch (23 x 33 cm) pan.Heat cooking oil in large frying pan on medium. Add green pepper and onion. Cook for about 8 minutes, stirring often, until pepper is softened.Whisk next 4 ingredients in medium bowl. Pour over onion mixture. Reduce heat tomedium-low. Stir slowly with spatula while egg is starting to set, scraping sides andbottom of pan until almost cooked but still wet. Spoon egg mixture into mushrooms.Arrange cheese slices over egg mixture. Cover with greased foil. Bake in 350ºF (175ºC)oven for about 20 minutes until mushrooms are tender and cheese is melted.Serve mushrooms, topped with lettuce, in rolls.
