• Silente, nicht temporäre Störungen des Glukosestoffwechsels
• Primär im kritischen Status auftretende Phänomene
- Steigerung der hepatischen Glukoneogenese
- Verminderte insulin-stimulierte Glukoseaufnahme (Skelett- und Herzmuskel)
- Gegenregulatorische Signale: Anstieg von Glucagon, Cortisol, Wachstumshormon
- Katecholaminfreisetzung- Cytokinfreisetzung: IL-1, Il-6, TNF
Hyperglyämie: Wandel in der intensivmedizinischen Betrachtung
• Historisch:Moderate Hyperglykämie unterstützt Versorgung von Gehirn u. Blutzellen = „Überlebensstrategie“ auf akute Verletzung bei Stop der Nahrungszufuhr
• Aktuell:Intensivmedizin mit Verbesserung der Überlebenschancen (chronisch kritisch Kranke)Totale parenterale Ernährung (TPN) mit permanenter Tendenz zur Hyperglykämie. Hyperglykämie als „Überlebensstrategie“ im Sinne der früheren Betrachtung nicht mehr notwendig

Einschlusskriterien: - Anamnestisch bekannter Diabetes mellitus bzw. antidiabetische Therapie o. Blutglukose > 200 mg/dl bei Aufnahme
- 10 % Glukoselösung 30 ml/h über 24 h i.v.
- Regelmäßige Kontrollen des Serum-Kaliums, ggf. Substitution, Zielbereich: hochnormale Serum-Spiegel
- Normal-Insulin 50 IE in 50 ml NaCl 0,9 % i.v.. über 24 h, Dosierung nach kap. Blutglukose (2h):
