MEDICAL PLAN Premera PPO vs. Kaiser (formerly Group Health) Determine whether you would like to remain on your current medical plan, or switch to another plan. If remaining on current plan, there is NO ACTION NEEDED. If you are switching plans, fill out the proper enrollment form. DENTAL PLAN Delta Dental vs. Willamette Determine whether you would like to remain on your current dental plan, or switch to another plan. If remaining on current plan, there is NO ACTION NEEDED. If you are switching plans, fill out the proper enrollment form. VISION PLAN Ensure that you enroll (or drop) dependents from the VSP plan, as necessary. LIFE INSURANCE Determine whether you would like to buy additional life insurance coverage through The Standard. If so, fill out the proper form online. For Benefits Effective January 2018

TOPIC THINGS TO CONSIDER

� LONG-TERM HEALTHCARE

Review the Unum Long-Term Healthcare documents to determine whether you would like to “buy up” coverage. Please note that Unum is increasing their rates in 2018 and current individual and group policyholders will be sent information packets near the end of November to discuss that change in more detail. If you are adding coverage, please contact HR so that we can discuss the 2018 increases with you.

� FLEXIBLE SPENDING

ACCOUNT (FSA)

Determine whether you would like to participate in one of the FSA plans next year, as well as how much you would like to contribute to the account. Forms and detailed information are posted online for these two accounts: Health Care FSA – $2,650 max contribution annually Day Care FSA – $5,000 max contribution annually

� DEFERRED COMPENSATION

WA State DCP vs. ICMA-RC The City offers two deferred compensation plans. Detailed information about these retirement plans are posted online. Please note that ICMA offers a Roth 457 plan as well. If you are not enrolled in a deferred compensation plan, review both

plans and decide if you want to participate. Fill out the appropriate enrollment form.

If currently enrolled in a deferred compensation plan, evaluate whether you would like to change the contribution amount.

� TUITION REIMBURSEMENT If you are planning on requesting tuition reimbursement in 2018, set a time to meet with your supervisor to discuss. Application forms (which can be found online) are due to HR no later than December 31, 2017.

� AFLAC SUPPLEMENTAL

INSURANCE

Determine if you would like to enroll in one of the three group plans offered: Hospital Indemnity, Accident Advantage Plus, or Critical Illness Advantage. Contributions are pre-tax.

� UPDATE BASIC

INFORMATION
