Many of the these forms will be available at the Fast Track Enrollment & Information Event in both printed and web-based format, but please note you will need to bring some important forms and documents with you to the event (or you may fax them in after the event), as noted below. Required Enrollment Documents Online forms – can be completed online at the Fast Track Event Student Information Form Enrollment Acceptance Form Academic Placement Form Student Residency Questionnaire Family Information Form Family Educational Rights Form (FERPA) Family Income Form Student Directory and Photo Release Form Hard-copy forms – printed materials we will have for you to complete at the Fast Track Event (or you may bring them) Request for Records Form Student Data Form Hard-copy forms – printed materials you should bring to the Fast Track Event COPY of birth certificate (do not bring original) – or fax COPY of immunization records – or fax Proof of residency (any current home utility bill or your lease agreement, but please, no cell phone bills) – or fax If you need to submit any of these documents by fax, please send them via 866.788.0368. Or you can scan and e-mail them to us at [email protected]. Documents should only be faxed or e-mailed after an online application has been submitted via the OHVA website or at a Fast Track Enrollment & Information Event. Please note: Documents pertaining to name change, custody, etc., may be required to complete the enrollment process. Please contact an Enrollment Consultant at 866.339.9071 or your Personal Admissions Liaison (PAL) if you think this may apply to you. Additional steps, such as attendance at the Admissions Conference, must be completed at a later date in order for your student’s enrollment to be approved.
