Read the instructions and click on “Logon” to go to the “House Bill 2504 Faculty Input Form”. If you don’t know your username/password or have any questions, please contact the IT Service Center at 915-831-6440.

When accessing the HB2504 site from a remote location (e.g. home, wireless), this dialog box will appear. You will be prompted to logon with your username and password used for PC/Outlook. For username field, please use the following format: epcc\username

Page 4: Once you have completed the form,  click  “ Submit ”

Please fill out the following fields:

- Title – Select from the dropdown menu (e.g. Mr., Mrs., Ms., Dr.)- Publication – Type your publications related to your discipline- Post Secondary Education – A. Degree – Choose one from the dropdown menu B. Major – Type in your major C. College/University – Choose from the dropdown menu Note: If your degree or college/university is not listed, please report it to your faculty/program coordinator. D. Month – Choose from the dropdown menu. E. Year - Enter year (e.g. 1998).

When complete, click “Add”. Click “Delete” to remove a previous entry.

Page 5: Once you have completed the form,  click  “ Submit ”

For the Teaching Experience section, please fill out the following fields:

- College/University – Choose one from the dropdown menu- Years Taught – Enter number of years in numeric format (e.g. 4)- Discipline – Enter name of discipline When complete, click “Add”. Click “Delete” to remove a previous entry.

For the Courses Teaching section, please upload your Syllabus Part 1 for each CRN or section listed by clicking “Browse”, select file, open, and click “Upload”. To replace an uploaded syllabus, repeat previous steps. Note: The field will only allow one file per CRN or section.

Page 6: Once you have completed the form,  click  “ Submit ”

This dialog box will appear. Select your Syllabus in Word, RTF or PDF format, select file, click “Open”, then click “Upload” in the Options field.

House Bill 2504 requires the budgets for every discipline to be displayed. No input is required for the Discipline Budget section. To view your discipline budget(s), click on “Click here”.

Page 8: Once you have completed the form,  click  “ Submit ”

The Budget posted is “Budget Form 1”, which is the same one posted on the website by Budget & Financial Services. To return to the input form, close the Budget by clicking the “X” on the upper right corner.

Page 9: Once you have completed the form,  click  “ Submit ”

Once you have completed the form, click “Submit”.

You will see a message letting you know that you have successfully completed the House Bill 2504 Faculty Input Form. Please remember that you can return to the House Bill 2504 Faculty Input page at any time to update your profile or upload your syllabi.
