Health

A Patient-centered medical home (PCMH) is not simply a place. It is a comprehensive primary care delivery model that meets the majority of each patient's mental and physical needs. Comprehensive care requires a team of healthcare professionals including physicians, advanced practice nurses, physician assistants, nurses, pharmacists, nutritionists, social workers, educators, and care coordinators.

That's why Head Start helps families find preventive care with the Early & Periodic Screening, Diagnostic and Treatment (EPSDT) schedule. Preventive care keeps children healthy and reduces costs by identifying and treating conditions early. EPSDT is a comprehensive, preventive care guide developed by the Oklahoma Health Care Authority.
