Corporate Health Management will be providing on-site seasonal flu vaccines here in our Confer-ence Center. Cash and checks will be accepted, and corporate billing may be arranged by calling Corporate Health Management, 214.361.0995. On the day vaccines are administered, please re-member to wear sleeves that can be easily raised. The vaccines will be administered on a first come first served basis:

There have been many questions concerning the H1N1 influenza vaccine. At this time, Corporate Health Management, the provider of our vac-cines, has not received official news regarding the dosage of the H1N1 vaccine or when for certain the vaccine will be released.

REGARDING DELIVERIES AND CONTRACTORS
• Every Vendor/Contractor who does any work or makes any deliveries in the building must have an approved Certificate of Insurance on file in the property management office.
• The Insurance Certificate must be on an Accord Form, meet all required limits and have the ex-act verbiage for the “Additional Insured” and the “Certificate Holder” that is on the sample form.
• A Security Clearance/Special Access Form must be submitted by your authorized tenant contact and approved by the property management office at least 24 to 48 hours in advance of the service being provided.
The necessary sample forms are located on www.mylincolncentre.com. If you have any questions, please do not hesitate to contact the management office at 972-980-9700.
