Specialist Palliative Care Team is: Available 7 days a week for North Tyneside community 9am-10pm. This is both

for planned visits (Mon-Fri 8am-4.30pm) and rapid palliative care input. Available Monday – Friday, 9am-5pm in the hospitals (Wansbeck, NSECH and

North Tyneside) Palliative care unit (PCU) providing specialist palliative care in a dedicated

inpatient setting which operates 24/7 Palliative medicine consultants in all services The community and hospital teams (other than PCU) have palliative care nurses

(band 7, 6 and 5, plus band 3 in the community team) which includes Marie Curie @ Northumbria staff

The teams have palliative care Allied Health Professionals in community physiotherapy, occupational therapy, social worker, technical instructor

Specialist nurse in palliative care (Learning Disabilities) Community Nursing support Team (CNST) including overnight Marie Curie

support service (to be renamed as North Tyneside Macmillan Care Support Service)

North Tyneside Day Hospice Dedicated palliative care nursing home team

Specialist Palliative Care Services provided in area: North Tyneside Palliative Care Unit – dedicated inpatient specialist palliative care

beds (19 beds – all single rooms) Community palliative care team with clinical nurse specialists, consultants,

physiotherapists, occupational therapists, social workers and technical instructor. This includes the Rapid Response Palliative Care Team who are able to respond within one hour to patients 7 days a week from 9am – 10pm

The following organisations are included within the locality: Newcastle upon Tyne Hospitals NHS FT Marie Curie Hospice and Community Nursing Service St. Oswald’s Hospice (adult and children’s)

Specialist Palliative Care Team The teams are located in: Freeman Hospital – covering all wards and departments NCCC – covering wards 33, 34, 35, 36, (includes haemato-oncology), chemo day unit and haematology supportive services oncology outpatients, radiotherapy treatment area and the clinical trials unit. RVI – covering all wards and departments within the RVI and the NGH acute services. Arthurs Hill Clinic - Community SPC team covering Newcastle residents registered with a Newcastle GP and those residents who may be on the borders of Newcastle but who are registered with a Newcastle GP and those patients in Newcastle within an NTW facility eg St Nicholas Hospital whose normal place of residence is Newcastle. All the Specialist Palliative Care Teams currently consist of clinical nurse specialists and consultants in palliative medicine. There is some social worker resource attached to the Freeman team and input from a Consultant Psychologist working into all the NUTH SPC teams. Each team has secretarial support all week.

SPC team working out of the community are commissioned to provide a 1 hour rapid response to patients in their own home or in Assessment suite and Emergency department at RVI 7 days a week 9am-5pm with the aim of reducing inappropriate hospital admission and facilitate rapid discharge. There is currently a pilot of seven day working in the three acute hospital teams, this is a nursing resource only and is available to give telephone advice, see new referrals and carry out urgent reviews of patients known to the service. In addition Newcastle upon Tyne Hospitals NHS Foundation Trust has a dedicated End of Life team consisting of a Lead Nurse for End of Life and End of Life Nurse Specialist. It currently has temporary funding for a Clinical Educator in End of Life and a Macmillan End of Life Staff Nurse and Health Care Assistants. This team provides support, advice, education to staff and direct care to patients in the last days and hours of life. The NUTH Palliative Care team is split across the Trust’s three main sites and community and aims to provide: Advice and support on symptoms The opportunity to discuss physical, emotional and social needs The opportunity to express thoughts and feelings Advice and support to families and carers Liaison with the hospital team that is involved in your patient’s care Liaison with the local Hospice services for inpatient, outpatient or day care Liaison with GPs, District Nurses and other Community Staff An educational resource to NUTH staff students, external providers and care

homes Support and education to Care Homes in Newcastle 1 hour rapid response 9-5 seven days a week in community and Assessment

suite and emergency department

Provides telephone support, ward consultations and outpatient consultations within the Friarage Hospital as an ‘in reach service’ from the community. The team also services the Hambleton and Richmondshire community which includes supporting patients at home, within community hospitals and care homes. In addition the team offer a range of education/training programmes. Teesside Hospice Specialist Palliative Care Service Teesside Hospice Care Foundation (THCF) provides specialist palliative care to all residents of South Tees CCG areas (patients from out of the area can be referred and funding is negotiated on a case-by-case basis). We are committed to providing specialist palliative care free of charge to patients and carers.

The specialist palliative care team which includes doctors, nurses, physiotherapists, occupational therapists, dieticians, social workers and bereavement counsellors offer a wide range of services:

1. In patient admissions for pain, symptom management, and end of life care.

(THCF has 10 single patient bedrooms that have en-suite facilities). 2. Day Care Services The Edward and Glenis Guy Day Care facility at THCF

provides 16 places per day Monday –Thursday and 12 places on Fridays at East Cleveland Hospital, Brotton and provides a range of other services including: • Psychological support e.g. For those having difficulty in dealing with the

knowledge of diagnosis/prognosis, those with housing, personal, family issues • Defined period of family/carer respite • On-going support for those discharged home from the in patient unit • Active rehabilitation • Regular assessment and monitoring of pain and symptom management • Supportive interventions:

o Transfusions etc. o Complementary therapies o Physiotherapy
