News Autumn/Winter 19 Welcome to Vermilion’s autumn 2019 update. The Vermilion Team is delighted to inform you about all our exciting updates, news and developments moving forward. Vermilion is delighted to open a new facility at no. 50 St John’s Road, just along the street from our original referral clinic at no. 24. This houses our “Smile Studio”, a welcoming and calm environment for our dental hygiene service. No. 50 has also been set up as a dedicated admin hub where all treatment letters and patient correspondence are generated, as well as handling all incoming calls for Vermilion’s two Edinburgh clinics. The ethos behind No. 50 is all about constantly improving our service to our referring dentists and their patients. We now have increased capacity to accommodate a team of first-class administrators who are dedicated to producing our treatment plans in a timeous and professional manner. By removing the telephones from the busy reception desk at no. 24, this ensures that our reception team can spend quality time helping our patients with any queries. In addition to our two wonderful dental hygienists, Colette Ballantyne and Michaela Zilinska, the team in situ at no. 50 includes our welcome team for hygiene patients, our dedicated referral co-ordinator, typists, events, finance and support staff. No. 50 is located on the busy intersection where Clermiston Hill meets Corstorphine High Street and there is an abundance of parking on neighbouring streets. Our original clinic at no. 24 continues to welcome patients for all other services including restorative dentistry, oral surgery, endodontics, periodontics, orthodontics and dental implant maintenance and after care. We are all set for the upcoming Refer & Restore Programme which launches on 10 October. This is a three-part series that takes place this autumn, with a second option to attend next spring. This course is ideal for dentists who would like to learn how to restore straightforward

implant cases without having to undertake any daunting implant surgery. Vermilion has already taught over 25 dentists to restore their own implants which has resulted in a new revenue stream for their own clinic – and patients love the convenience of this. The key to Refer & Restore is a strong

collaboration between Vermilion and the referring dentist and we are constantly on hand to provide mentorship and ongoing support for the benefit of our patients. Our past Refer & Restore colleagues will even be attending the second part of the new course for an update on crown

construction, digitisation, material choices, abutment selection, and laboratory liaison, demonstrating the need for ongoing learning and support. We are delighted to partner with Southern Implants and Passion Dental Design, and the course is spearheaded by

Vermilion Clinicians David Offord, Steve Siovas, Nadir Khan, Martin Morley, Colette Ballantyne and Michaela Zilinska. They will cover a breadth of topics from live implant impressions and surgery, crown fitting, periodontal disease and the business case of restoring your own implants.

Scotland’s dental referral scene is at an interesting juncture. At Vermilion, we are acutely aware that we must never rest on our laurels. There are now several dentists offering a referral service and our many loyal

referring dentists have a choice to whom they can refer their patients for treatment. At Vermilion, we never claim to be the best or the most specialist. What we do is always put our patients first and strive to give them an exceptional dental service which in turn makes them feel happy and satisfied with the trust they put into our team.

We also never ever take our referring dentists for granted. We remain extremely grateful for each and every referral that we receive and do our utmost to ensure that our expertise, our care and our passion

translate into the best outcome for the patient. We hope that by reading this newsletter, you will understand that the investment that we have made in our product, our people and our service is not to be perceived as a grand statement, but as a necessity. The dental scene is ever evolving, and through

increasing our space, recruiting new talent, constantly researching and analysing our clinical work, surveying our dentists, asking for feedback, encouraging our own people to reach their full potential and

treating our referring dentists as co-partners all play a vital part in raising our game. Humility, understanding and commitment are just some of our values at Vermilion and, above all, we aim to always give it our best and do the right thing by our referring dentists and their patients.

Martin has been with Vermilion for over a year now and is ever positive and welcoming for all our patients. Martin has three key roles at Vermilion:

• Initial implant consultations where he obtains the basic data set required to work up the case including charting, BPE, radiography and intra-oral scan.

• Simple implant restorations under the SAC classification (Simple, Advanced, Complex) where a patient is referred for simple implant rehabilitation.

• Implant maintenance: Martin heads up the maintenance team who carry out essential annual reviews of implants placed at Vermilion, to ensure the long-term health of the implanted teeth. He assesses three areas: condition of the crown/bridge/denture, oral hygiene status and radiographic assessment of marginal bone levels. Martin provides all referring dentists with an annual report of their patient’s implant journey.

Vermilion Borders opened in the gorgeous town of Kelso in May 2018. We are very grateful for the many dentists who have referred their patients to our purpose-built referral facility in the Borders. We are hosting a welcome morning for all Borders and Northumberland based referrers on Saturday, 9th November where they will get a

chance to meet the team. This will followed by a Braai hosted by our friends at Southern Implants, followed by an afternoon at the gee-gees!

Abstract

Aim: This retrospective observational study evaluates the safety and efficacy of intravenous sedation using midazolam only or combinations of two or three drugs including propofol and alfentanil for complex dental surgery such as implant placement or sinus augmentations. Methods: The study analyses 60 patients aged between 42–79 who required advanced sedation techniques for complex oral surgery procedures. Twenty patients had midazolam only(M), 10 patients a 2-drug combination of midazolam and propofol (MP) and a third group of 30

patients had the combination of midazolam, propofol and alfentanil (MAP). The last two regimens were carried out under the supervision of a dedicated consultant anaesthetist.

Results: Higher mean minimum heart rate (beats/min) was observed in the midazolam group compared to the group sedated with the three-drug regimen P<0.001. The time between the last drug administration (LDA) and end of surgery (EOS) was significantly longer(p<0.005) in group M(≤45mins) compared to the other two sedation groups MP and MAP (≤15 minutes). The final titration of midazolam of 11.2 ± 4mg(5–20mg) was a

significantly greater in group M than in either of group MP or MAP(p<0.005).

Conclusion: In patients requiring conscious sedation to undergo complex and lengthy oral surgery, a combination of agents (midazolam, propofol and alfentanil) can provide a predictable steady state of sedation up to the end of the surgery resulting in better operating conditions for the surgeon and improved patient experience. The shorter duration of action drugs propofol and alfentanil improved haemodynamic stability, sedation quality and reduce the final doses of each sedative agent.

In June, we welcomed 80 referring colleagues to the Vermilion Symposium which was held in Merchants’ Hall in central Edinburgh. This was an educational and networking event with a theme in primary dental care. We were delighted to welcome Professor Trevor Burke, Professor of Primary Dental Care at the University of Birmingham and Editorial

Director of Dental Update, as our keynote speaker. He explored many factors which may affect restoration survival, such as the choice of material, quality of material used, plus a variety of tooth, patient and clinician factors. In addition, Vermilion Clinicians Madeleine Murray, Will McLean and Louise O’Dowd talked the audience through the

new classification of periodontal diseases and explored “How long do restored and periodontally teeth last?” The Symposium was a highlight of Vermilion’s ongoing free, verifiable annual programme of CPD which benefits many of our referring dentists and colleagues who participate in our many educational events.

Dr Nicola Kingsford is Vermilion’s Research & Audit Co-ordinator. Her task is to carry out research and internal audits, both of which are seen as essential components of the clinic’s patient safety and quality improvement processes. Nicola has recently submitted a paper to Oral Surgery on the safety and efficacy of intravenous sedation using midazolam only or combinations of two or three drugs. This follows a recent audit of patients who required advanced sedation techniques for complex oral surgery procedures.
