Community-acquired pneumonia due to Chlamydia pneumoniae, Haemophilus influenzae, Mycoplasmapneumoniaeor Streptococcusoneumoniaie in patients appropriate for oral therapy. (For specific dosage recom-mendation, see DOSAGI N ADMINISTRATION.)

NOTE: Akzihhmmycin should not be used in pediatric patients with pneumonia who are judged to beinppropritehrUoralheay becaus of moderate to severe illness or risk factors such asanyfoftfiehowing: patients with cystic fibross, padients with nosmoonul acquired infec-

NO1TE: Penicillin bvthe intramuscular route is the usual drug of choice in the treatment of Streptococcus py-genes inifecto In the prophylaxis of rheumatic fever. ZITHROMAX(@ is often effective in the eradication of'sus'cep.-tible strains of Streptococcuspyoge, nesfrorn the nasopharynx. Because some strains are resistant to ZITHROMAX,susceptibility tests should be pe rmoredwhen patients are treated vvth ZITHROMAX'. Data establishing efficacy ofazithromycin in subsequent prevention of rheumatic fever are not available.

Appropriate culture and susceptibility tests should be performed before treatment to determine the causativeorganism and its susceptibility to azithromycin. Therapy vvith ZITHROMAX" may be initiated before results of thesetests are knowvn; once the results become available, antimicrobial therapy should be adjusted accordingly.

CONTPAINDICATIONSZITHROMAX" is contraindicated in patients wvith knovvn hypersensitivity to azithromycin, erythromycin or anymacrolide antibiotic.

WARNINGSSerious allergic reactions, including angioedema, anaphylaxis, and dermatologic reactions including StevensJono ydrome and toxic epidermal necrolysis have been repJorted rarelv In patients on iazithromycin therapy.Althughrar, ftaliieshav ben reported. These patients required

prolonged periods of observton and symptomatic treatment. The relationshipo these episodes to the long tissuehalf-life of azithromycin and subsequent prolonged exposure to antigen is unkov at present

If an allergic reaction occurs, the drug should be discontinued ana appropriate therapy should be instituted.Physicians should be awvare that reappearance of the allergic syrmptoms may occur wvhen symptomatic therapy isdiscontinued.

In the treaftnent of pnemonia, aithronricin has only been showfn to be.safe and effective in thetreatinent of community,-acquired pneumonia due to Chhwnyia Ituxw HsenmWq,o s in&ua e,ytcophasmapneuMmonx,or Sgtococcuspemn nstnsaprriefo oral therapy.

Pseudomellmbanoscoii has been reported with nearly all antibactensl agents and may range insevenYfomd t lie theatnirTlfore, it is importantto considerthis diagnosis in patients

who~ ~~~~~Speetihdahasbeuntothe administration of antibacterial agents.Treamenvfth atibcteia agents alters the nommal flora of the colon and may permit overgrowvth of clostridia.

Studies indicate that a toxin produced by Clostridium difficile is a primary cause of 'antibiotic-associated colitis."After the diagnosis of pseudomembranous colitis has been established, therapeutic measures should be initi-

ated. Mild cases of pseuaomembranous colitis usually respond to discontinuation of the drug alone. In moderate to severe cases, consideration should be given to management vvth fluids and electrolytes, Protein sup-plementation, and treatment vvth an antibacterial drug clinmicaillyeffective against Clostridium difficile colitis.

PR lAU OSGenmra: Because azithromycin is principally eliminated via the liver, caution should be exercised when azithromycinis administered to patients with impaired hepatic function.

There are no data regarding azithromycin usage in patients vvth renal impairment; thus, caution should be exer-cised when prescribing azithromycin in these patients.

The following adverse events have been reported with macrolide products: ventricular arrhythmias, includingventriculartachycardia and torsades de,oointes, in individuals with prolonged QT intervals.

There has been a spontaneous report from the post-marketing experience of a patient vvth previous history ofarrythiaswhoexpriecedtorsades depointesand subsequent myocardial infarction fbllowing a course of

Infrmaionfo Paiens: itromaxe oral suspension can be taken with or without food.Patients should also be cautioned not to take aluminum- and magnesium-containing antacids and azithromycin

simultaneously. The patient should be directed to discontinue azithromycin immediately and contact a physician if any signs of an

allergic reaction occur.Drugbrahcd Aluminum- and magnesium-contining antacids reduce fth pezak serum concenraaions Irate} but notthe ALIC (extenX of aii mycin absorpio.Administration of cimetidine (800 mngf) two hours pfior to azithromycin had no effect on azithromycin absorption.

Azithromycin did not affect the plasma concentrations or pharmacokinetics of theophyline administerea as a sin-gle intravenous dose. The effect of azithromycin on the plasma concentrations or pharmacokinetics of theophyllineadministered in multiple doses resulting in terapeutic steady-state concentrations of theophylline is not known. However, concurrent use of macrolides and theophylline has Deen associated with increases In the serum concen-trations of theophylline. Therefore, until further data are available, prudent medical practice dictates careful monitor-ing of plasma theophylline concentrations ~in patients receiving azithromycin and theophylline concomitantly.

Azithromycin dia not affect the prothrombin time response to a single dose of warrarin. However, prudent med-ical practice dictates careful monitoring of prothrombin time in all patients treated with azthromycin and warfarinconcomitantly. Concurrent use of macrolides and warfafin in clinical practice has been associated with increasedanticoagulant effects.

The following drug interactions have not been reported in clinical trials with azithromycin, however, no specificdrug interaction studies have been perfommed to evaluate potential drug-drug interaction. Nonetheless, they havebeen observed vvth macrolide products. Until further data are developed regarding drug interactions whenaAihoyi n hs rg r used concomitantly, careful monitoring of patients is advised:

In the animalstudies, no evidence of harm to the fetus due to azithromycin was found. There are, however, no aaequate and well-controlled studies in pregnant wvomen. Because animal reproduction studies are not always predictive of humanresponse, azithromycin should be used during pregnancy only if clearly neededNursu' Mothems It is not known whether azithromycin is excreted In human milk. Safety and effectiveness for pneumonia due to ChlamydiapneumoniaeandMRycoplasmapneumoniae were documented in pediatric clinical trials. Safety and effectiveness for pneumoniadue to Haemophilus infiuenzaeand Streptococcuspneumoniaewere not documented bacteriologically in thepediatric clinical trial due to difficuIt in obtaining specimens. Use of azithromycin for these two microorganismsIs supported, however, by evidence from adequate andwell-controlled studies in adults.
