Incident Summary A basketball thrown by a student at full strength hit my left arm extremely hard while I was supervising playground duty. Also hit on the right back of hip by a second ball. The ball was thrown instead of being put away gently. My arm is still quite sore and now I have pins and needles most of the day.

Incident Summary INJURED HIS LOWER BACK WHILE CHANGING A TYRE ON THE EDUCATION QLD TOYOTA RAV 4 ON HIS WAY TO SERVICE ISISFORD STATE SCHOOL IN HIS ROLE AS THE INCIDENT OCCURED AT APPROXIMATELY 8 AM, ROUGHLY 40KM FROM lSI FORD ON THE ISIFORD ROAD. Incident Summary When coming back into class with student after being at a specialised activity, the student went over to play with the wooden blocks when I told him to 'stop' and that we were not playing with blocks now. I asked him to put them away, as I picked one up to put on the shelf he hit my my left hand (below the thumb) with a wooden block. My thumb and hand below it instantly went numb and after having an ice pack on it for 8 to 10 minutes it had pins and needles for the rest of the day with bruising developing.

Incident Summary On the second day of the assesment and rating i felt very tight in neck and shoulders. The following day I spoke to my son about my jaw and teeth being sore. Over the next 4 days I was at home on leave relaxing. As it was raining I was able to lay in bed. The pain in my jaw was increasing. Sunday the 13th I took pain releif. On Monday the 14th I noticed the pain had increased and took pain releif when I was moving the pain seemed to settle. On Tuesday 15th I returned to work. I dropped my chidlren to my sister and commented that I wasn't feeling well and that my jaw was sore. I began work for the day. I noted that when I sat at my desk I had pain radiating into my face. I took pain releif. By approx 10.30am I was in extreme pain and I asked my supervisor if I could walk across to the massage palce across the road. agreed and I walked to the massage place. The massage therapist was not available and so I walked to the physiotherapist and asked if sometone could see me. They said they had an appointment at 12pm. I walked back to work and let know. I went to the appointment. The physiotherapist asked about my work station and I explained that we use ipads in the fielsd and she explained that when you drop your jaw it can cause problems in the neck. She said that I had restricted C3 and C4 movement which is where the trigiminal nerve meets the brain stem. She carried out mobilisation, accupuncture and tapped my back into a shoulder back posture to stop my jaw dropping forward. I made an appointment for 7.30 am the following day (wed 16th December) On the way home I stopped at the chemist and purchased panadol and coedine to try and control the pain. On Tuesday and wednesday prior to taking the pain eleif I was unable to sit down, only stand or lay down The following day I went to the appointment and was still in extreme pain. After mobilisation and accupunture and strapping , I saw the GP .I went to my GP who asked me to get my teeth checked andf prescribed L YRICA pain releif. I began taking the L YRICA on WED 16th. On Wednesday and Thursday I was unable to drive due to the pain relief. Thursday I was bale to sit down again.My mother drove me to see the physio on Thursday the 17th. I was mobilised, strapped and given accupuncture. I made an appointment with the dentist on Friday 18th. The dentist took xrays and did not identify any major issues with my teeth. He did shave one of my back teeth down a bit where a point may have been contributing to the pain in the jaw area. I purchaed a lumber roll recommended by the physio and a mouth guard to wear at night. I purchased a chair support for my work desk. I went to physio on the 21st. The physio put needles in my face and upper back strapped my back. I returned to work and my collegues assisted me in rearranging my work space. Lifting the screens, moving the screens and putting the bolster on my chair.

Incident Summary entered room where a medical procedure was taking place for a student requiring an insulin reading. After the procedure was performed on the diabetic student the needle was picked up by who after playing with the needle stated he injected himself. The needle was the lancing device that is used to obtain a blood sample from the student. Initial conversations with staff around this and the unsurity of whether this occured or not led to the Principal making a decision with the HOC to call Mother and notify her of the incident. Further investigation by the school nurse and myself led to the decision that there was no possible way the needle could be re-fired. mum was shown the device and reassured this one would not inflict a needle prick. Concern is that the needle used after this one for the injection of insulin is not a protected needle which could inflict a cross contamination if other students are present during the procedure.

Incident Summary As per incident report attached -While covering for on bus duty, student did a runner. I had to prevent him from leaving F Block area. I was in a back brace and had to run after student,on ce I stopped him, I started to walk him back, when he suddenly dropped to the ground with no warning. I was caught by surprise but the jolt has upset my pre-existing injury to no end. Extra strain on spine and lower back, also arms, shoudler and legs. Today (01/03/12) pins and needles in both legs and feet, right shoulder and lower back (Siatica) extremely agilated.

Incident Summary was playing in the Development Cup Squad on the Emerald State High School oval against Springsure. As she was tackled my the opposing No.10 player she landed heavily underneath his weight. When the player got up he stood on her wrist. At that point she felt some pain, but continued to play the game. This tackle happened just before half time. During half time mentioned it was a little sore, but that she wanted to continue playing. Later that night when she arrived back home (at approximately 8:00pm) she told her mum that her wrist was hurting quite a bit more and that she had pins and needles in her fingers. She was taken to the hospital for xrays and found it to be broken.

Incident Summary took a tea towel out of the cutlery draw in the staff room and shook it open. When she did this, a syringe fell to the ground. She picked it up thinking that it was to give medicine to a child (like neurofen). It had a red top on it and she pulled it off and saw that there was a long needle pointing out the top. She spoke to

(Teacher Aide) about it before informing me. I called immediately (ESO) and she told me to call told me to call I called a few times and left a voice message. There was no reply so I put the needle in a plastic bag and placed it into the safe for overnight. I called

again in the morning and she told me to dispose of it at the police station and to advise the staff of the matter. That afternoon I called in all staff for an emergency staff meeting and explained what was found and how it is being dealt with. I asked them to be very vigilant and careful around the school.

Incident Summary On entering my classroom this morning, the floor appeared shiny and covered with a film. As I walked into the kitchen area, I slipped and landed on my left hip. The top half of my body twisted and as a reflex, I put out my left hand to stop my fall. Soon after, my left arm had the feeling of 'pins and needles' and then went numb. The wrist is sore and pain travels into my forearm when the wrist is moved.The muscles in my back and chest are also aching and sore.

Incident Summary received the flu vaccination needle and then immediately suffered a reaction- she fainted. She came to and was sat down but then fainted again. was tended to by the nurse administrating the vaccination and by another staff member from office). was laid on the floor with her feet up but continued to 'pass out' (periods of time where she was unresponsive, limp and breathing very shallow). I came into the room with lying on the floor and the staff waiting for their vaccination still in the room -they appeared unaware of what was experiencing. complained (limited speech) of her mouth feeling a little swollen and her tongue a little thick (she kept swallowing). continued to have the blank episodes (appeared similar to 'petit mal' seizures) every minute or so. The nurse then administered an adrenaline shot and the ambulance called. The staff were then sent from the room by myself and I continued to monitor (on floor holding her hand) with and the nurse. The ambulance arrived and took her measurements and then removed her to hospital.

Incident Summary was stitching a baby quilt when accidently got her finger in the needle. She called me over, I saw what had happened and called to inform the office. The needle went through her finger and got bend and stuck in the pressure foot. She could not remove or move her finger for that reason. I released the needle to ease the pain. The office was informed and the amulnace was called instantly. The paramedic arrived after 30 minutes and gave her the pain reliever.

the nurse came in and helped the paramedic remove her finger. She was taken to the hospital with the needle still stuck in her finger.

Incident Summary I, Cleaner while earring out my duties in Student Residence No. 5 my right arm rubbed along the edge of the rail resulting in a splinter peneterating the skin.The splinter appeared to be about 20mm-25mm in length. I attempted to remove the splinter myself using a splinter needle from a first aid kit. This proved to be unsuccessfulll then contacted my line manager who sent me to a GP.

Incident Summary was making a tackle on a Springsure player and got hit in the head. He was left laying on the ground after the tackle and were the first people on the seen. Before he was turned he was asked where his pain was and he said his only pain was in his head. He was left as he was until his breathing had slowed down. He had feeling in his arms and legs, his only pain was in his head none in his neck. He was carfully turned over by onto his back. He had what looked like a bump on the right hand side of his head but he said the pain was on top of his head in the middle. He was asked what his name was, he answered correctly. But was unsure of where he was or what day it was. He wouldn't open his as he said opening his eyes hurt. He breathing was fast. At this point in time (Sports Trainer) was called over to assist. She assessed him for movement with fingers and toes which was present, pins and needles which weren't. Again verified that no pain apart from in his head which he said had started to ease. After 10 mins after contact he was slowly sat up then helped up to his feet and was assisted from the field to his mother. Advised by to seek medical attention from the local hospital.

Incident Summary I was taking a weapon off a student who refused to let go of the weapon. He was attempting to hit another student with the weapon so I gripped on tighter. I had the weapon in my right hand. A few minutes later another student was leaving the Principal office, extremely aggressive and agigitated, trying to fight the first student who was now out side the building, and student number 2 pushed passed the Principal and the Acting BSM towards me. He barged towards me and I put my hands up and onto his arms, up near his shoulders to stop his from advancing. I was able to turn the student around so he was facing forwards as I have prevoiusly been injured by this student when facing front on. I asked him to breath deeply and calm down, repeatling the phrase while the student continued screaming. He took his hat off his head and whacked it backwards on to my legs. The Principal called out the door for community assistance. The student continued screaming and wriggling and I continued to losen my grip. A community member assisted and I assisted him to sit in the chair. As I did I felt my wrist and arm get pins and needles and begin to ache.

She directed me to the area outside the classroom. She suddenly spotted the lid and went to pick it up. I directed her to stop immediately and had her wash her hands. I picked up the lid and placed it with the other items in the container.

was also unwell and went to first aid room but didn't vomit. had fainted during the tour (lnc#52045) and fell and broke her teeth. She was taken by ambulance. All activities were cut short and whole group returned to accommodation. Medical attention was sort for and

accompanied these students to medical centre. and began vomiting at the accommodation. & returned from appointment and had been given a needle to stop the vomiting. improved but required further treatment. An appointment was arranged for 9.30pm. & reported they were unwell after dinner. Medical appt made for and for 9.30pm.

escorted the students to their appointments. had appointment at 9.30pm. While at Laser Games felt unwell and travelled (with parents permission) back on the bus to acommodation. made phone calls to all ill student's parents. and began to feel ill and were taken back to the accommodation. All three were vomitting once back at accommodation. and also felt ill at the laser games and began vomiting there just before they left. Teachers contacted parents and made them aware. Principal approved to use her school corporate card to pay for the medical expenses for 9 students. 9:30pm . 10:30pm - all received either a table or injection to stop the vomitting. The hospital declared this an epidemic (more than 3 people with illness) and provided sick bags, antiseptic wipes, hydralytes and tablets to stop the vomitting. became unwell and Sheryl stayed up with her. She started to vomit so afterwards gave her a tablet and she didn't vomit again.

also joined and and was vomitting. Attempted to rehydrate but she kept vomitting. also joined us who had vomitting and diarrhorea. He was given a tablet and eventually returned to bed. was also not well but didn't vomit until an hour before breakfast the next morning.

Students attended the dinner and questacon activities that night. and were not feeling well on their return to accommodation. vomited and was given a table and went to bed. No further medical treatment was required by students throughout the night. The next morning 20/2 every student attended breakfast. Teacher, was feeling unwell and vomitting. She took a tablet, did not have breakfast. The bus trip back to Gladstone was sick and lethargic & vomitting on occasions. was also unwell and reported vomiting, Teacher, was unwell and reported vomitting and Teacher, was also experi8encing diarrhorea and general unwellness.

to look at my neck. commented that my earring had left a mark on the side of my neck and that the area was red. Initially my ear was ringing and my neck felt tender to touch. I placed an ice pack on the right side of my neck. I started my class but after approximate 20 minutes my neck and shoulders started to seize (tighten) and I had pins and needles in my fingers and my upper arm started to feel numb. I went into the classroom next door and reported to that I was not feeling well and she combined the classes together. I went down to the staffroom and spoke to who contacted the office on my behalf to inform them that I had been injured and was going to the hospital.! went up the office and spoke to who called numerous doctors surgeries to get an appointment but was unsuccessful. My brother then collected me and took me to the Rockhampton Base Hospital.

Incident Summary The incident occurred on an school excursion to Leslie Patrick Park. The class was at the creek looking at its features. The children were given large plastic tongs to pick up rubbish and put it into a garbage bag to clean up the creek. Before beginning the activity the students were specifically told not to pick up anything with their hands. Before I could stop her, (yr1 student) picked up a small syringe with her hands right in front of me. arm covering her mouth and was still choking like a couching fit when I arrived! cleared the students away from the scene and instructed to get a teacher to call for assistance. came to assist and removed the students from the area. kept in the recovery position checked around her and I removed her hand straight from her mouth and asked if asked her if she could hear m did not reply but opened her eyes and looked at me, I asked to let me see in her mouth if she was choking on anything, at this point stopped choking and coughing and the saliva stopped coming from her mouth and she opened her mouth wide and started inhaling deep breaths. told to remain in this position and keep the breathing calm. The redness in her face dispersed and I asked her again of she was starting to feel bette then said she wanted to sit up. I sat her with her back against the wall. did not look coherant at this stage and was not answering my questions as to how she was feeling. I took hand and asked her to squeeze my fingers which she did. I then told her I was going to ask her some questions and explained I needed her to answer them if that was possible. asked her the day of the week and she responded with thursday, I asked her what her name was and she responded with and I held up three fingers and asked her how many fingers I was holding and she answered 3. The colour then came back to her face and came and offered assistance and told that an ambulance was on the way and that her mother had been calle became upset and panicked by the thought of the paramedics arriving and started insisting to only call her mother. ensured that her mother was on the way and that everything would be fine and then returned to her class. remained with and asked if she could remember what happened prior to her choking. said she couldnt remember anything. I asked her if there was a feeling in her stomach, chest or head first and she said she could not remember. I then asked her if she knew how she ended up on the ground, did she sit or fall and still said she had no idea. She then told me that she felt really sad before it happened. then explained that this was common lately as of the end of year seven and that she takes medication every night for panic attacks. At this moment the ambulance arrived and became hysterical, shaky and very concerned that they were going to give her needles or thumb pricks. Incident Summary was in a group following procedures of a prac on compression. He was compressing some air in a syringe by pushing it against a rubber stopper. He was using his body weight perhaps as leverage?? For some reason he slipped while pushing down, the syringe hit his finger and he started to bleed at the same time he lost balance and fell over. In his fall the back of his head hit the work bench. fell to the ground. At this point students informedimmediately on what happened. was standing back up at the time and so directed him to a tap to wash his bleeding finger. was speaking to him and getting him to explain what happened, he could not recall. requested him to sit down and grabbed him a chair and helped him sit down while she got some paper towelling for his finger. requested him to apply pressure. At this point told him not to move until he felt better and if he could to up to the office to be treated for first aid. His friend said he would help bring him up to the office. In this time the bell had rung and

was informing students of their homework, checking their diaries and allowing them to go. and had left. presumed they went to the office. rang the office five minutes later to check tha had arrived. nformed her no-one of that name had turned up.

Incident Summary I was walking from the carpark at Avoca street towards A Block when I saw and 2 of her friends standing next to the path. began to slump in her stance and one of her friends caught her. I raced over and gently lowered her to the ground. Once on the ground she began convulsing which lasted a period of about 5 minutes until she began to lose conciousness. I was able to get her to respond and open her eyes quickly but her responses were very vague. She complained about pins and needles in both of her arms. She was placed in the recovery position and had her head/neck supported throughout this time. I continued to ask her questions and found that she had a fall from a horse on Saturday and was airlifted to the Mater Childrens and released on Monday. She said she had damaged ligaments in her shoulder/collarbone and could not remember if she was knocked out when she fell off the horse. A number of times whilst supporting her in the recovery position she complained about pain at the back of her head at the base of her skull. Myself and Amanda Lane continued to monitor and comfort her until paramedics arrived.

Doctors phoned back and advised to call the Sunshine Coast Health Public Unit (54096600), All parents were rung and asked to come to the school. The father of the diabetic was in Brisbane and couldn't not attend the school. The Sunshine Coast Health Unit returned our call mentioned that they have contacted the Infectious Disease Unit and left a message and was awaiting them to call her back with where we go to from here, from the unit has asked us to email her the children's details and parents contact details.

Incident Summary In class, bent to tie his shoe laces and his back became sore. He told his teacher who monitored him until lunch 15 minutes. At lunch he said his legs were numb, teacher explained he may have pins and needles and uncrossed his lower legs after asking permission and let him remain in class with friends to eat. Teacher notified Admin 15 minutes later when he still did not want to move. Teacher was aware of some previous back discomfort and contacted Mum. Mum spoke about another incident previously outside school where it was very minor and thought his back might just be sore. She said she would come to school and check on him. Other students re-entered after lunch and students began to work. still did not move but did not appear to be in much discomfort, he just did not want to move his legs. An ambulance was called. Ambulance and Mum arrived. The paramedics reported there did not seem to be any back spasms, they got

to stand and walk with assistance. He was in minor discomfort. went home with Mum, after Mum had spoken to paramedics. They did not recommend he travel to hospital.

Incident Summary A sharp knife was left in the rubbish and plastic bags from lunch. While packing up the rubbish I pushed my hand down onto the point of the knife causing a deep wound which bled profusely. The wound appeared to be at least 1 em deep. I had just told the class that they needed to place the pressure foot down to hold the item in place and that they were not to sew without it down. Students had been advised that they needed to keep their hands/fingers well away from the needle area. All safety requirements were addressed the previous day and the students were told again today at the beginning of the lesson to remember to keep hands clear of the needle. had not put the presser foot down and then sewed over her finger.

Incident Summary The basketball kept getting stuck in the ring nets. I climbed up the net to fix the net. I was hanging on for about 15 seconds. Upon letting go and landing on the ground, my fingers on my left hand were red and swollen. Within five minutes all had returned to normal, except my ring finger, which had pins and needles feeling. Three days later it still has the same feeling with a pressure build up.

Incident Summary Student, walked into the school grounds via the front gate with what appeared to be a used syringe in his hand. (needle was pointing out from his body). Deputy Prinicipal, , opened Office door to walk out to a class and saw him with the syringe. said, "Look what I found . Deputy noticed that had touched the end of the needle with his fingers and directed him to not touch the needle. Deputy steered him into the office area away from others and into sick bay area instructing him to place the needle in the sink on top of paper towel. Deputy then informed the other Deputy Principal,

and Principal, about the situation. The needle was then removed and placed in a syringe disposal container and was given soap to wash his hands. Admin officers were instructed to contact parents about the situation however there was no answer on any contact numbers. was asked where he found the needle and he told them outside the school grounds, near the gate on the ground and that he had picked it up. Deputy asked if the needle sharp had pierced his skin anywhere on his hands or body area to make sure that he wasn't hurt. said "no" however Deputy checked his hands for marks. Deputy has for clarification about a red mark/scratch but said that was old. Deputy went to Principal to discuss plan of action due to parents being uncontactable. When Deptuty returned to he had one of his tracksuit pant legs pulled up and he was looking carefully at it. Deputy asked if he had a mark on his leg from the needle. shook his head and pulled his pant leg down however not long after had his pant leg up again and was looking at it. Deputy told

that he wasn't in trouble, but that we needed to make sure he was safe and asked again if the needle had gone into his legs. said that he had accidentally stabbed himself with the needle and when the Deputies and Principal looked there was a small dot red mark on his leg. Deputy and Principal explained that he might need to go to the Doctor and that we couldn't get hold of his parents so we would drive to his house and inform them of the situation and if needs be, drive to the house. took the Principal and Deputy Principal to the exact place where had found the syringe and they then checked along the front of the school to ensure there wasn't another syringe.

Incident Summary had her immunisation injection then sat outside after her needle for her 15 minute resting time. Students came to get me at 2:40pm saying wasn't feeling well and had the shakes. I asked to check on her as I was busy with other students and sending students in for their needles. He gave her some cold water and checked on her as we were instructed to do by the nurses. I then checked on her and she said she was ok and she'd let us know if she got worse. Students came in a few minutes later and said that wasn't well, shaking and she thinks she's having the symptoms that she has before an epileptic fit.

went out to her and she explained that to him. came and got me. He explained that felt like she was having the warning signs of an epileptic fit. I went out and she had her arms up in the air and was shaking. I asked her how she was. She explained that she was having the signs that lead to a seizure. I asked her when the last time she had a seizure was and she said 2 years ago. I asked are you still prone to getting them, she said yes my paediatrician (stumbled to say the word) said I can get them at any time and I'm having the symptoms before I have a fit. I reassured her that she'll be ok. I spoke to the nurses who were dealing with a student who was feeling faint and they said to call the first aid officer and move her away from here, contact dad to give him a heads up. They weren't concerned as much as she was and they said moving her away from students would be beneficial for her. The nurse stated that if she does have a seizure the office/first aid person will call an ambulance. I rang to get the wheelchair and administer first aid and mentioned that had had a possible reaction to the injection and she thinks she thinks she's having symptoms before a seizure and I had spoken to the nurses and at the same time rang her dad (as instructed to by the nurse) to inform him of what was going on and he said he was on his way. came and collected with the wheel chair and took her to the office. was collected by 2:45pm by

Incident Summary I walked to the Prep C classroom to take an Arts lesson. Students had already entered the classroom and were being supervised by the PE teacher. On entering the classroom, I observed a student seated at a table using a puffer. I then focussed my attention on gathering the students into a group and settling them for the lesson. I marked the roll and told the student to join the group when he had finished. After the activity had commenced a teacher aide came in and stated that she thought that he had used his epi pen.

Incident Summary went to the Doctor yesterday with ear pain and has an ear infection. He believes dust got into his ear trimming back bushes. When was working casually at the beginning of the year trimming back the bushes up high it creates dust and accumulated in your ear. Incident Summary Study tour student ate a pistachio nut offered to him by is his study tour buddy. started to get an itchy tongue and feeling a little difficult with his breathing. He came to International Department and wanted to speak to the Japanese Study Tour Coordinator. They came directly over to Administration Sick Bay. WE asked the Coordinator and if the student in question was anaphylactic. We advised them that we would be calling 000 if this was the case. They said that he was. We asked if he had his epipen and he did not. We asked him does he have an epipen. He said he does. He did not have it with him in Australia. (Australian Agent Coordinator JAS) requested ambulance. 000 called. On advising the situation and condition and that PBC had the school Epipen, 000 support advised to administer it now administered the EPIPEN direct into his left thigh, held it for the 10 seconds and then massaged the needle site. Present were the Sick Bay Coordinator and the Japanese Study Tour staff. stayed on the phone to 000 whilst this was happening to ensure that any information they were giving was conveyed. WE sat with the student and remained on the phone until the AMBULANCE officers arrived.

Incident Summary states the following: She came in the workplace (Port Curtis Road State School) during the school holidays to gernie the concrete areas under A & B Block. This cleaning involved using the gernie for extended periods of time to clean underneath these buildings. It wasn't until the end of the week she started noticing her right hand/wrist aching and starting to have pins and needles. This aching feeling and discomfort was starting to keep her up at night. Incident Summary Whilst I was on playground duty at F Block (Area 6), approached me to let me know that his friend (Year 10) had been hurt. I went with immediately and assessed the situation. appeared to be in a lot of pain and told me that he was finding it hard to breath. He was hunched over, pale and shaking, and groaning. He told me that a friend had landed on him when they were play fighting and that his friends head and shoulder had hit him hard in the ribs. I told not to move, radioed for the Hot Spot on PGD, and sent student runners to the Performing Arts staffroom (nearest building) to ask a teacher to contact First Aid. I crouched next to nd asked him to try to take slow breaths if possible and told him that people from First Aid would be with us shortly. It was at this point that I heard a popping noise followed by a gurgling noise. groaned and exclaimed loudly "It's popping again!". He was quite panicky and I calmly asked him to stay still. who I had asked to remain with us told me that he had heard the popping noise when had first attempted to get up after being hit. At this point, I asked a number of other students to please go to the PA again and ask a teacher to contact First Aid and ask them to contact an ambulance. Soon after, and approached us to provide support. I thanked the students who had delivered the messages and asked them to return to their classes. and I stayed with We reminded him to try to stay calm, to take slow breaths and to stay still. As was crouched, and I offered our hands as support if he needed to adjust his position to become more comfortable. We also asked if he wanted to lean on his bag. arrived and contacted mother, she let him know that an ambulance was on the way and that she would ride with n the ambulance and meet with her at the hospital. As the ambulance had not yet arrived, I called to see if there was a delay and to relaysymptoms. I left with and when I did this as I did not want to further distress him. The ambulance personnel assured me that an ambulance was on its way and told me to keep still. Whilst we were waiting for the ambulance explained that his group of friends often play fight or "muck around". He said that the group would watch out for teachers and stop play fighting when a teacher was seen nearby. When the teacher had walked away, they would resume play fighting. He said that today, some of the boys in the group had taken it too far and had stomped on people whilst they were on the ground. I told that this behaviour needs to stop and that I would be reporting on it further. The ambulance arrived soon after and was guided into the school by two senior students (as arranged by whose class was close by). The ambulance officers assessed and offered him pain medication, which he refused. had small cuts/abrasions to his hands as a result of the play fighting and complained that they were too sore to have a needle put into. They assisted him to stand and move to a stretcher. They asked if they could remove shirt but he would not cooperate. They informed him that it would be cut off by hospital staff. They moved to the ambulance and left with them to go to the hospital. was asked to report to class.

Incident Summary I had just signed off in Cleaner's Store Room and gone upstairs to the Admin office to let know I was leaving for the day to go to an appointment. I checked with her to see if I am allowed to work ADO hours during school holidays [I wasn't allowed at my last School]. I exited the building through the main door on Eddington Street, and was busy rearranging the mat that is usually placed outside. It had fallen when I opened the door. I was not paying sufficient attention, and slammed the door on my finger in the process. As I was running for an appointment, I kept going. It initially hurt really badly, and when I got to my appointment they gave me a glass of water with ice in it to place my finger in. I was wearing Jam berry wraps [vinyl decals that adhere to the nail surface] on my nails, so couldn't see the extent of the damage. As the pain had subsided a bit, I chose to muddle through, but got little sleep that nigfllhe following morning, 19/01/2016, at about 5.50am, I was working in E block and lifted a student desk. I accidentally jabbed the end of my finger against it, reacted and dropped the desk on my foot- which WAS in an enclosed shoe, the same approved shoes I wear every day to work. I managed to badly bruise underneath the toenail of my right big toe, and limped from After work, I went to my Doctor's surgery. My usual Doctor is away, and I saw another Doctor attached to the surgery. She used a hypodermic needle to make holes in both nails - left index finger and right big toe. The blood under the finger had dried and did not flow, but the blood under the toenail was still fluid, and exited the cavity under the nail under some considerable pressure. The toe has felt fine ever sincehe Doctor sent me for an X-ray of the finger, and the radiologist told me that it wasn't brokel()n the 20/01/2016 at 9.15am, I saw the Doctor again and she confirmed that the finger is not broken. She has restricted some of my work processes, but is allowing me to resume work on 21/01/2016.

(staff member) attended to and ambulance was calle< reported - Students alerted me that was in significatn pain. when I approached her she said she had shooting pains in her calf, behind her knee and her hamstring. She also said she had pins and needles in her left toes. Further examinations and prior knowledge of a lover back injury led me to investigate the lowe back in relation to the sciatic nerve. did a further examination and mentioned she had less feeling and sensation in her left leg compared to her right. Due to her history of back injuries I was led to believe she had further damage to her lower back and treated as a spinal injury, therefore an ambulance was called.

Incident Summary was talking to a teacher . She had just finished giving a student a needle and was putting it in the sharps bin. She pushed it in to the bin the wrong way round and got pricked on the finger.

Incident Summary went to step off the back of the kubota ride on mower. instead of stepping straight off, he stepped off to the side. His right foot got caught between the brace and the platform on the back of the mower.He then tried to turn to prepare for impact, he was unable to correct his foot, body twisted and landed right shoulder and right hip onto the ground. at 1 pm was asked to drive the bus, on his way home from Cardwell he had lower back pain, pins and needles in legs, and numbness in both feet (more so the right foot).

Incident Summary a year 4 student found a syringe before school (location unclear) he then carried it around in his pocket for the day. reported at 1:35pm to the office that had a syringe in his hand and had jabbed in his arm. The boys involved were brought to the office with the syringe in a sealed bag.

and were interviewed at the office by the Deputy Principal and acting Deputy Principal about the incident. reported that he found the syringe in the playground before school, he said that he put it in his pocket and went to class. pulled the syringe out at the end of second break play at 1:30pm at the end of the ramp near D Block (year 4 classroom). said to and "hey you guys want to try my needle" . said "yes" and pushed the needle into arm. It was then reported by that began chasing around and trying to jab them. witnessed the boys running around and asked them what was going on. then reported this incident immediatley to the office. The only student who was jabbed was

Incident Summary Class room windows are extremely difficult to open and close. I closed the window to secure classroom ( J007) in last lesson 2.40pm. I felt a sharp pain in my right shoulder, neck and arm ( back of elbow). By Friday evening, it was painful to lift my arm up past my breast height. The sharp pain was going down to my wrist and up to my shoulder. It felt heavy, numb patches and my pinkie and second finger was getting pins and needles sensation. I had a headache and pain in my temple above my right eyebrow.

Incident Summary Students were playing game of Dodge ball in Sports centre as instructed by Mainstream teacher I was supervising SEP students and partially participating when the ball came near me. I picked up the ball and threw it, as I did so felt a pain in my shoulder and mild pins and needles. I stopped participating in the game.

Incident Summary During eating duty several students reported to me that Student had hit them very hard in the head with their lunchbox. I approached Student and he hit towards me with his hat. I stated that he was hitting other students and he needed to go to the garden. He yelled and growled at me and refused to move. I continued to tell him he needed to move, he refused and hit me with his lunchbox in the left arm twice. I held him by the arm and guided him to the garden, he kicked me in the right ankle while moving. At the garden he refused to sit down, yelling at me that I couldnt make him. I sent a student upstairs to find a teacher on dutry, when I turned to speak to the student Student again hit me with his lunchbox into my back. I told him that if he hit me again I would call the police. He struck me a again in the left arm with his lunchbox and kicked me in the right leg. I let the other teacher on duty know I was taking Student directly to the office. He refused to go and swung his hat at me. came down and directed to the office. He refused at began yelling. ook him to the office but Student returned to the eating area screaming abuse at students at the teachers. My left arm continues to have pins and needles in it from the assault.

Incident Summary 2 incidents within 10 minutes1 - playing handball with the year 7 students, a student threw the ball to another student and it hit me in the left side of the head. No pain within two minutes. Accidental2 - playing handball with the year 8-10 students, a student threw the ball to another student, over me {while my back was turned) and it hit me in the right shoulder blade. I could not move my arm for 5 minutes and there was a dull ache/pins and needles for the next period at school. It was ok by 3pm.

Incident Summary Students were putting their recorders back in the bucket I, teacher was collecting their recorder sheets. As I have turned to put the sheets on my desk I have been stung by something above my elbow. I turned and asked the students what hit me and they told me a piece of recorder. I asked who threw it and found out that a student had taken the bottom piece of his recorder off and was trying to hit it with the rest of his recorder like a softball bat. I asked him who he was aiming for and he said no-on e. He was just trying to hit the piece of recorder. When the student hit my arm it made my hand go pins and needles.

Incident Summary Account as per t\t the end of the theory lesson the students were being instructed on how to generate a doughnut bandage. The box which contained the bandages also contained a training Epi pen. The students were going through the physical motions of the training epi pen. I ended up talking about the uses of an epi pen and how to use the device. The students were then moving around the classroom completing their doughnut bandages and getting another bandage to secure the doughnut bandage. I believe at that time one of the students picked up the extra epi pen (live), I was not aware that it was in the box, and attempted to inject it in to another student as part of the training epi pen procedure. The epi pen was live and slightly scratched the student drawing blood. The student injecting the epi pen attempted the injection in the forearm and not into a large muscle group and not held in as per procedure, therefore, there is little reason to suggest that a proper injection took place school nurse, walked by the class room and assessed the student. There was no immediate signs or symptoms of the injection and was therefore given the all clear from the nurse.

Incident Summary After giving his Insulin injection, I went to recap the Insulin Pen, as I was instructed to do by the school Nurse. As I recapped the needle, the needle has protuded through the side of the cap, through my glove and into the tip of my left index finger.

Incident Summary Student sewed over her (I) finger with needle on machine. All students had previously been instructed in safety reuse of machines. Teacher was assisting another student on the other side of the room when one of the students asked if she could help student who had injured finger. Teacher went over immediately, stopped her trying to pull her finger free, slowly loosened the needle clamp and gently lifted her finger out with the needle still embedded in the finger. Incident Summary Approx 12.20pm received a call from a member of the public who backs onto the school saying he has just witnessed a middle aged couple injecting themselves on school property. He had rang the police who said they were on the way. I ( ) then went down to the Music block (E block) to observe the pair. was watching them as they sat on the border line of the school still on school property. arrived and said he would watch while we took a class of students to the Main hall for a lesson. then returned to his room, I remained with who was instructing students. I then noticed that the pair had started to walk towards the Senior playground near the hall. I decided after observing them staggering slightly to close and lock the doors to the school hall which I did. I then noticed and observing them from a distance of their movements. The couple then began walking down the pathway heading towards the Prep area so I rang the Prep classes and told them to lock all doors till further notice. At this point I lost sight of the pair and contacted Admin via the office who had rang the police at approx 12.30 informing them of the situation. I then saw a Police officer run across the oval and speak to and . I then say that another officer was talking to the two people we were observing. The Police officer looked in the drain area where the pair had beeen injecting and found 2 used needles down the drain. then searched the grounds near the drain for any other needles.

Incident Summary The student realised after the bell went that the sharps container and needles were missing. She reported it to Mr. Willis and myself. We looked for the items during the lunch break and having not found them we spoke to the class involved during the next period. A female student then told me that she had seen the sharps container in a sanitary bin in the G-Biock girls toilets. The sharps container was there and the bin was placed in the staffroom away from student access. The unused needles have not been found.

Incident Summary I met outside the staffroom door. She asked me if her face looked like it was ©twitching©. felt unsteady. I took her into G101 and settled her in a chair. began to cry. She was unable to move her fingers or properly feel her upper body. could not feel much of her upper body at all on the right hand side. I
