Mark Patients for Merge Tip Sheet Process – Any Staff can Mark a Patient for Merge. This information is sent to a workqueue in HIM. The HIM staff will research the patient records and determine if they should be merged.

Mark Patients for Merge is used when an end user believes that there are two accounts in Epic for a single patient. When two accounts are marked for merge by an end user, the accounts will fall into an EMPI workqueue for further investigation. “Mark for Merge” does not mean the two accounts will be merged. Users will see a Patient Advisory when they enter a patient’s chart that has been marked for merge to alert them to the fact that the accounts are under investigation for a potential merge.

How to Mark Patients for Merge: Try It Out 1. Epic button > Tools > Identity Tools > Mark Patients for Merge

(this Activity may be directly under the Tools button)

    2. In the First Patient Lookup window – enter the patients name with the most information

    3. In the Second Patient Lookup window – enter the patients name that you identified as similar

    4. The Mark Patients for Merge window opens:

    5. This step is not required but the end user has the option to select what patient data they believe is

correct. The Merge Result column is a combination of what is selected from the First Patient and Second Patient columns. The Merge Result column should contain the data the end user thinks is correct.

    6. Select a Reason for Marking these patients for Merge. 7. Click Mark for Merge.

Note – this will not merge the patients. It will only identify them as Potential Merge candidates. After researching both records, HIM will determine if these records will be merged.

    Patient Advisory

A Patient Advisory will now appear on each patient’s chart that was marked for merge so users are aware that the account is under investigation for a potential merge. Users will also see a hyperlink within the Patient Marked for Merge advisory. By clicking on this hyperlink, the associated record will automatically launch so that the potential duplicate patient’s chart can be reviewed.
