Local Action Plan 2017-2020
Yorkshire & Humber
Background
As of 1st January 2017 there were XXX people known to the Association with MND, 6 MND Association branches and groups, XXX regularly active Multi-Disciplinary Teams (MDTs) and 2 MND Care Centres/Network (Sheffield and Leeds). In addition we are about to fund an additional MND coordinator post in Bradford (2 days per week).The total population is c.5.2m. Expected incidence or the number of people expected to develop MND each year is XXX people and the expected prevalence or the number of people living with MND at any one time is XXX people. This three-year Local Action Plan (LAP) has been developed by the Local Delivery Team (LDT) in consultation with key stakeholders. It defines priorities and activities to support the Regional Delivery Plan (RDP) developed from the strategic goals and objectives of the Association as set out in our 2017-2021 Strategy. The RDP includes Regional Performance Indicators (RPIs) which will be used to monitor progress alongside quarterly narrative reports to the Directors Team. This plan will be formally refreshed mid-way through the Strategy in 2019

Use influencing and negotiation to improve health and social care services not funded by the Association where there are identified shortfalls
Utilise LDT meetings and wider consultation to help identify shortfalls & inform local clinical and social care priorities, and work with regional staff and HSCPs to influence better provision. Target commissioners where there appear to be fundamental shortfalls in provision. In particular:

Recently highlighted in HSJ article as having on of highest proportions of adults waiting more than 19 weeks for a wheelchair (Q3 2016 NHSE data/not MND specific data). Own anecdotal data (St Michael’s hospice) suggests that issues are evident for pwMND, e..g not taking on any additional work:

Recruitment, retention and succession planning of all regionally based volunteers
