The denture implant is an alternative for edentulous ridge with bone that does not

provide retention and stability to a conventional full denture. A cemented prosthesis

implant retrieves and provides an improvement in chewing, swallowing, phonetics and

therefore the life quality of patients. The conventional treatment of a protocol is

performed in two surgical stages, followed by a prosthetic one. This article describes a

technique of the rehabilitation of a patient using implants in a single surgical step with a

total fixed dentogingival prosthese in the upper maxilla.

Keywords: Dental implantation. Dental prosthesis. Complete denture.

Patient MDN, female, 51 years old, has none of all elements since 13

(thirteen) years of age, using conventional dentures and partial removable

bottom. In surgical planning a prosthetic implant placement was indicated to

the upper maxilla and lower jaw in oral rehabilitation using fixed

dentogingival prosthese.

In a completely edentulous atrophic maxilla, were installed 08 (eight)

Ankylos Frialit – Dentsply implants associated with freeze-dried bone graft

and biological barrier (non- resorbable gore-tex membrane) in a single step

surgical procedure. Billateral sinus lift and concomitant implant installation

was also performed. Distribution of the implants was made in accordance

with reverse planning and after a 6 month period, healing abutments were

installed to allow rehabilitation.

The prostetic rehabilitation started transfering the position of implants to

help the choice of abutments on the fixed dentogingival prosthese and

temporary one. With abutments prepared (Fig. I) a transfer molding with

addition silicone was performed to make the fixed dentogingival prosthese

structure, at this time the new temporary denture was installed (Fig. II) .

CASE REPORT

An occlusion mark was taken for analisys on dental cast in the articulator. During

the clinical exam a facial analisys evaluated the soft-tissue profile in upper and lower

lips in relation to Steiner's S line. These structure evaluations were made to replace the

orolabial facial muscle, providing the patient an adequate functional occlusion, normal

overjet and overbite, and a pleasant facial esthetics.

In search of clinical success, it is essential that the planning guarantee the needing

of each patient. Fixed prostheses restores the sense of confidence and produces great

satisfaction to its users, but within their applications - especially in edentulous jaw

implants cases - it has been related difficulties in restoring the lip support and reported

phonetics interferences. To recover these structures of facial and lips support it is

necessary to use materials with high quality such as porcelain, capable of providing a

good esthetic interface with the gingival tissues and a harmonious relation to the

adjacent teeth and antagonists.
