To approve order granting the request submitted by Kevin Morris that DallasCounty reduce its claim for its workers compensation lien from $17,502.88 to$12,500.00, and authorizing the County Judge or her designee to execute anyreleases on behalf of the County in favor of Kevin Morris upon the receipt ofpayment in the amount of $12,500.00 from Kevin Morris.

To approve order determining that no objections shall be filed to the Award ofSpecial Commissioners in the case styled County of Dallas v. To approve order approving acceptance of a donation in the amount of $20,000from TXU Electric and Gas to assist those in need with their electric or gasutilities.

To approve order approving the FY 2005 Comprehensive Energy AssistanceProgram (CEAP) Contract No. 585102 in the amount of $1,986,044 with theTexas Department of Housing and Community Affairs (TDHCA) for the periodfrom January 1, 2005 through December 31, 2005, and authorizing the CountyJudge to electronically sign the related contractual documents on behalf of DallasCounty.

35. COURT ORDER:

To approve order supporting the resignation of Bank of America as Trustee of theWorking Mothers fund and, should Dallas County be eligible as the trustee of thefund, approving the transfer of the trust fund to Dallas County as well as namingthe Dallas County Commissioners Court "Trustee" of said fund on behalf of DallasCounty Health and Human Services.


36. COURT ORDER:

To approve order approving the reallocation of FY 2004-2005 Ryan WhiteC.A.R.E. Act Title I grant funds in the amount of $200,068 to specific serviceproviders, and authorizing the County Judge to sign the contract amendmentswith the specific service providers on behalf of Dallas County.

37. COURT ORDER:

To approve order extending current contract (Bid No. 2003-006-1234 - AnnualContract for the Purchase of Gonorrhea and Chlamydia Test Kits) with BectonDickinson Diagnostics for an additional twelve (12) month period, effectiveJanuary 21, 2005 through January 20, 2006, at an estimated annual cost of$100,000.00.

38. COURT ORDER:

To approve order authorizing the modification, price increase, and extension ofcurrent contracts (Bid No. 2004~002-1418 - Annual Contract for Landscaping)with Alcorn Landscaping and J & S Lawn Services for a combined total bid priceof $69,137.95 for the period from November 21, 2004 through November 20,2005.

39. COURT ORDER:

To approve order authorizing the workers compensation insurance of Mail Mart,Inc. to be waived for the entire life of the contract, as required in Bid No. 200411 8-1 544 (Annual Contract for Fulfillment of County Clerk Real Estate Records),and authorizing payment of all invoices being held by the Auditor's Office to bereleased as well as future payments due to Mail Mart, Inc. under this contract.

40. COURT ORDER:

To approve order authorizing the award of RFP No. 2005-003-1579 (AnnualContract for Psychological Screening and Counseling Services for LawEnforcement Personnel) to Dr. Lehman, Ph.D. and Associates for the period fromFebruary 1, 2005 through January 31, 2006 at an estimated annual cost of$40,000.00.

To approve order authorizing the award of RFP No. 2005-014-1590 (AuctioneerServices) to Lone Star Auctioneers, Inc., based on listed commission percentagerates, for the period from January 18, 2005 through January 17, 2006.

To approve order authorizing the award of Bid No. 2005-031-1607 (One TimePurchase of a Forklift) to Equipment Depot (lowest compliant bidder) for a totalcost of $18,422.

To approve order authorizing the County Judge to sign the attached and anyfuture Houston/Galveston Co-Op (H-GAC) orders for all acquisitions procured withHomeland Security Grant Funds.

To approve order authorizing the Dallas County Purchasing Agent to advertise forthe solicitation of competitive bids for the modernization of four (4) jail elevatorsat the Lew Sterrett Justice Center - West Tower.

46. COURT ORDER:

To approve order approving Change Order No.7, in the amount of $63,839.00,to the contract with Cadence McShane Corporation for the Henry Wade JuvenileJustice Center Expansion (Bid No. 2002-135-1175) and the addition ofthirty-two(32) calendar days to the contract, thereby changing the completion date fromNovember 22, 2004 to December 24, 2004, and authorizing the County Judgeto execute said Change Order on behalf of Dallas County.


47. COURT ORDER:

To approve order accepting the best and final offer, in the amount of$779,706.49, from Bauhaus for Knoll furniture for the George Allen Civil Courtsproject, authorizing this purchase to be made using the U. S. Communitiesfurniture contract, and authorizing a contingency in the amount of $40,000 tocover any unforeseen furniture needs for this project.

48. COURT ORDER:

To approve order approving the fee proposal from James Pratt Architecture/Urban Design, Inc., in an amount not to exceed $160,216, and authorizing theCounty Judge to execute Amendment No. 1 Phase II Architectural ServicesSupplemental Agreement No.2 to the contract for additional professional servicesfor the Old Red Phase III Clock Tower Reconstruction.

49. COURT ORDER:

To approve order authorizing the County Judge to execute Work Order No.8, inan amount not to exceed $38,366.11, with Arredondo, Zepeda & Brunz, Inc.(AZ&B) for certain engineering services needed for the CDBG FY 2003 CockrellHill Water Main Improvements, Phase IIIB project.

To approve order authorizing the County Judge to execute Work Order No.9, inan amount not to exceed $11,128.31, with Arredondo, Zepeda & Brunz, Inc.(AZ&B) for certain engineering services needed for the CDBG FY 2003 CockrellHill Water Main Improvements, Phase IIIC project.

To approve order authorizing the County Judge to execute two (2) Agreementsto Contribute Bond Funds (Modified) with TxDOT and authorizing the CountyTreasurer to remit payment in the amount of $105,000 to TxDOT to assist thecities of Dallas and Irving in funding the local 10% share of the total estimatedcost of right of way and utility adjustments for the Loop 12/Spur 408 (IH20 toIH35-E) Project 91-549.
