Soror Angela Smith called the meeting to order at 12:11 p.m. with the ritualistic opening. There was a
quorum present. Handouts were verified. Soror Mekasha Brown moved to adopt the agenda
w/additions (33 for, 0 opposed, motion carried). Minutes for the previous meeting were read, Soror P.
Clark moved to accept the minutes as written (43 for, 0 opposed, motion carried).
Visiting Sorors: Soror V. Baston-Sutton recognized Shamon Forbes, Azure Dismuke, Janae’ Branch, and

• Program Planning Development (V. Baston-Sutton) The revised Risk Management Manual is
available and posted on the National website in the Program Planning and Development

(White Elephant Gift Exchange), and that a list will be provided for sorors to sign up to provide a

• NPHC (Yolanda James) - Safe Haven is in need of boys and girls toys (no guns or knives) and women's pajamas for its annual Christmas Boutique. Items are due by the November NPHC
meeting. If you are interested in running for any open positions, please come to the November
NPHC meeting. Our membership funds must be paid by January 31, 2017
• U.N.O. (Brittany Bailey) - In collaboration with the Epsilon Alpha Chapter of Omega Psi Phi Fraternity, Inc. Sorors fed over 600 members of the homeless community on Saturday, October 22, 2016, at Unity Park in Fort Worth
• Technology (Kimberly Hinton) - Discussed updates to FWA Website, email reminders, updates
to Facebook, upcoming activities, and deadline for all committee reports will be at 1:00 p.m., the
Saturday before E-Board Meeting

Meeting Date: 11/01/2016 Meeting Location: No Committee Meeting Participants: 0
Summary of Committee Activities: During GEMS December session, Dec. 3rd, there were 10 young
ladies, 1 parent, and 5 Sorors present. The young ladies completed job applications and participated in
mock interviews. Applications were completed for a variety of businesses (Forever 21, Pac Sun, Dillards,
etc.) and careers (anesthesiologist, veterinarian assistant, and gynecologist). Each young lady was given
positive feedback following their interview. The session concluded with our holiday celebration. The
young ladies participated in Christmas exchange game. The gifts ranged from sunglasses, facial masks,
holiday throw blankets, stylish chokers, bubble baths, and over the knee socks. The session was very
productive. Surveys will be sent and feedback presented upon receipt.
Upcoming Activities: Delta GEMS will be attending Delta Day at Jubilee Saturday, December 17th.
Delta GEMS will be ending 2016 being servants to the community (serving lunch at Union Gospel
Mission December 31st 11am - 1pm)

Happy Hour crowd. Discussed activities for FWA to celebrate our own milestone honorees: 25, 50 and
70 year milestone memberships in Delta. Discussed consideration of recommending to National
honoring 70, 75 + milestone memberships.

Summary of Committee Activities: Summary Audit Report
The Internal Audit Committee completed a review of the financial records of the Fort Worth Alumnae
Chapter of Delta Sigma Theta Sorority, Inc. for the first quarter period of July 1, 2016 to September 30,
2016. The review included a random sampling of disbursements, receipts, deposits and 100% of
membership dues collected during the period. The initial review was completed on October 22, 2016.
Additional hours, on approximately 5 days, caused additional information to be examined. Questions
were answered by the financial team. An additional 2-3 hours were dedicated to the data entry in the
Red Zone. Total hours for the first quarter audit activities were approximately 15 hours. The
NIAC held a webinar on November 16, 2016 in which 25% validation was instructed, however, our initial
examination had already been completed, so for this quarter we exceeded the number of records to
examine.
A summary of the major findings and recommendations are listed below:
Finding #1: The testing of disbursements showed that 2 checks out of 22 tested (9% of the sample
tested), had no backup documentation attached to the vouchers

Recommendation #1: The chapter policies and procedures dictate that all disbursements must have
accompanying documentation. In the instance where no invoice or statement exists to support the
documentation, utilization of the chapter minutes with the approval highlighted will be sufficient. Emails
can also be used as approval documentation for the disbursement.
Finding #2: The testing of disbursements showed that 2 checks out of 22 tested, had discrepancies
between the documentation totals and the check totals.
Recommendation #2: The Treasurer ensures the amounts on the documentation match the amount of
the check to not cause uncertainty. For example: If the requestor puts an amount on the voucher and
that amount is adjusted, then the Treasurer should cross out the original amount and correct the
amount and initial.
Finding #3: The review of the check inventory was not made available for the audit committee to
inspect; therefore, no representation of pre-signatures or missing checks can be validated.
Recommendation #3: The Treasurer ensures that the checkbook is available for the audit committee to
inspect. Note: Treasurer was not aware of this requirement.
Finding #4: The testing of receipts and deposits revealed that 1 of 29 receipts tested (3%) the deposit
number on the report did not match the number of the receipt log. Recommendation #4: The Treasurer
should ensure that the deposit # match the receipt log #.
Finding #5: The testing of dues for the period showed that 3 of 21 (14% of the sample) were not
submitted within 30 days of receipt.
Recommendation #5: Grand Chapter funds must be deposited within deposited within 30 days of
receipt as per page 8 of the National Fiscal Officers manual.
Finding #6: The testing of disbursements showed that 1 check out of 22 tested (4% of the sample
tested), had no signatures on the voucher.
Recommendation #6: The Treasurer should ensure that all vouchers have 2 approval signatures on the
voucher. The Financial team could review before the books are turned over to the audit team for
review.
After review by the Chapter Internal Audit Committee or their designee, it has been determined that the
chapter has satisfactory financial internal controls and the review supports the existence and utilization
of these controls.

A copy of the Detailed Audit Report has been provided for the chapter’s records.
Upcoming Activities: 2nd Quarter Audit in January
Recommendation: 1Q Internal Audit Additional Recommendations: The Financial Secretary compile a
separate listing of 100% of the membership dues received in the period reviewed and the Treasurer
keep a listing that documents the date received and date transmitted to nationals for review; Recording
Secretary email the Executive Board minutes and chapter minutes to the Internal Audit chairperson
upon completion to [email protected]. Treasurer forward email correspondence from
PayPal transactions to internal auditor via chapter email and/or print copies for Treasurer Report. Both
Financial officers email the [email protected] their reports in Excel version upon
completion not pdf.
The Treasurer emails the [email protected] the bank statements upon receipt from the
bank. This way copies can be made ahead of time for the committee to use for the audit.
The Financial officers review all vouchers and documentation monthly before the audit convenes to
ensure all paperwork is attached and proper signatures are affixed.

Summary of Committee Activities: Jabberwock participants and parents met for the 2nd time,
Saturday, November 19th at the Bethlehem Center. Soror Bevelia Curley facilitated the workshop -
"Strategic Planning to Secure College Funds: Scholarships and Financial Aid".
Upcoming Activities: The next workshop is Saturday, December 17th at Shamblee Library, (note different location for this workshop only) at 12noon. This will also be the first collection meeting for participants. The committee is asking chapter members for their support with the following:
Securing gently used ball gowns for our participants, which will be returned after the pageant;
Helping to make this year's scholarship pageant a success by purchasing an ad in the souvenir book; and
Locating and inviting previous Jabberwock participants and winners to purchase tickets and attend the pageant an April 2nd in celebration of Jabberwock's 40th Anniversary.

New Member Orientation was held on November 12, 2016 before chapter meeting. A total of 10
sorors were present: 3 new members (Machell Denson, Elan Wylie and Jasmine Lowe), 7
committee members.
Two additional new Sorors (Clintonia Bryson and Donna Allen) were given the New Member
Orientation documentation.
DID Training for the chapter was held on December 3, 2016 from 8:30am – 2:30pm at the
Women’s Center, 1723 Hemphill Street, Fort Worth, TX. The facilitators, Sorors Monica Brewer
of Austin, TX and Pier Crenshaw of Grand Prairie, TX, presented Parliamentary Procedures and
Sisterhood, respectively.
1) Attendance/Registration: As of 12/01/2016 only 32 chapter members had paid to
attend DID to try to make quorum, Sorors were allowed to pay at the door on the day of
the training. Nine additional sorors paid at the door, for a total of 41 attendees.
2) Expenses
Breakfast costs: $118.10 (included food, drink and paper goods)
Lunch boxes were ordered from Jason’s Deli at a total cost of $423.80.
Trainers’ mileage (at the published government rate of $0.565 per
mile) and gifts ($50 each) totaled $385.21.

Summary of Committee Activities: Know Your Rights! Get the 411 from Our 911
This workshop for youth, parents and the community was sponsored by The Fort Worth
Alumnae Chapter of Delta Sigma Theta Sorority, Inc. (GEMS, EMBODI, Academy, Jabberwock and Social
Action committees), Kappa Alpha Psi Fraternity- Fort Worth Alumni Chapter, the Fort Worth Black Law
Enforcement Officers' Association, and the National Association of Black Police.
The purpose of this workshop was to educate our youth about how to properly interact with police
during a traffic stop or investigation. There is a great need for our youth to be educated on how to gain
respect from authority and properly channeling one's behavior if ever confronted by the police.
Officers affiliated with Fort Worth Black Law Enforcement Officers Association and the National
Association of Black Police conducted mock traffic stops with the youth participants. Deputy Chief
Vance Keyes of the Fort Worth Police Department also assisted in the program. These interactive
sessions allowed the youth and police officers to recreate various scenarios involving positive and
negative contact with law enforcement. The topics covered were “How to Keep a Ticket from Becoming
a Felony Arrest,” “Comply, Comply, Comply” and “Who Can I Turn To? “, “Reporting Bad Cops”.

There was also a question and answer session, with Judge Louis Sturns, Attorney Leon Reed and Tarrant
County Prosecutor Valerie Baston. They answered questions regarding searching vehicles, recording
traffic stops/criminal investigations, asset forfeiture, citizen rights and other legal issues.
Students were also encouraged to seek out careers in the legal and law enforcement fields, so that our
communities can have more minority representation in these areas. The students were challenged to be
active participants in the political and law-making process by becoming registered voters, voting in each
election cycle, and contacting local, state and national elected officials about laws that impact the
minority community. Sgt. Jakisha “JJ” Jones, of the Arlington Police Department, also raffled a
Thanksgiving dinner basket, which was donated by the African American Peace Officer Association of
Arlington and Kappa Alpha Psi Fraternity, Inc.
This was the third year in which this workshop was conducted. The goal of the workshop has been to
reduce the number of negative encounters between police officers and minority youth, educate youth
on searches and seizures, federal and Texas laws. As a result of this annual workshop, this program is
being replicated in local schools.
