Ø Frontline Provider: a Medical Service Provider whom a Resolution Insurance Member can access service by providing their Resolution Insurance Membership Card. Page 1 to 12

Ø Specialist Providers: a Medical Service Provider who is a Specialist to whom a Resolution Insurance Member can access service by providing their Resolution Insurance Membership Card. Page 13 to 22
