1.0. EMERGENCY RESPONSE PLAN (PY-1 DEV. PROJECT OPERATIONS)

1.1. Introduction: It is a primary and continuing policy of Hindustan Oil Exploration Company Limited (HOEC) that in the conduct of its activities, it will act positively to protect the health and safety of its personnel and others who may be affected by these activities and take necessary precautions to prevent adverse effects on the physical environment in which its activities are carried out. The Health, Safety & Environment (HSE) policy of HOEC is provided below for reference. This document details the Emergency Response Strategy & Management Plan for all PY-1 Dev. Project operations like production & drilling activities at the PY-1 Offshore Block and Onshore Gas Processing Terminal. The intent of this ERP is to ensure adequate and appropriate response strategy for controlling / minimising the impact of production operations and drilling operations on PY-1 Facilities. The primary authority and responsibility for combating an emergency at the operational site lies with the Company during normal production operations and with the Drilling Contractor when drilling unit is operational at the platform. Nevertheless, the Company shall extend all possible assistance and also has the right to take over the response operations incase the Drilling Contractor is unable to handle the emergency. Notwithstanding the above, HOEC shall have its own emergency response plan, with response chain and data directory of the concerned drilling team and Management members. The Drilling contractor is expected to comply with the requirements of this HOEC Drilling Emergency Response Plan. On declaration of emergency, all work permits issued at the operational areas shall be suspended. Personnel shall safely close down all their operations and move towards the muster station as per the procedure. Fresh permits should be issued after thorough assessment of the workplace. Accident / Incident report has to be filled in and submitted to HOEC office without fail, on identifying an emergency scenario. The priorities during the course of the response action shall always be in the following order:

• Safety of personnel • Safety of property • Safeguarding the environment

The purpose of this ERP is to act as a guide and is not intended to serve as an infallible rule book to be used in lieu of sound judgment based on the knowledge and particular

circumstances at hand in any given area or situation.

1.2. HSE POLICY

HOEC CORPORATE HSE POLICY

While pursuing this objective, it is HOEC's conviction that the Company must be committed to avoid injury and to preserve the health & safety of its employees and any other persons who at any point of time may be affected by its activities and to conduct its operations in a manner that provides optimum protection to the environment in which these operations are conducted.

• The Company's HSE Policy forms an integral part of Line Management Objectives and is an essential part of the individual objectives of each employee. Line Managers will be held equally responsible for HSE objectives as for other objectives such as financial and production.

• The Company's HSE Policy must be communicated to all employees, its suppliers and to all

others that the Company may be associated with. • The Company's policy is that working safely and protecting the environment is a condition of

employment. No employee will start, or knowingly be a part of, any unsafe act or violation of the Company's HSE Policy.

• Each employee, full time or part time, has the responsibility to ensure compliance with the

Company's HSE Policy and Procedures therein. Each individual will conduct himself in a manner so as to protect himself, others who may be affected by his actions and the environment.

• The client, contractors, sub-contractors, suppliers and any third party personnel working with

HOEC are required to conduct themselves in a manner which is in compliance with HOEC HSE Policy, as well as the policies of their respective companies and the laws & regulations of the countries where the tasks are being conducted.

• The Company will support employees to care for their own health however the ultimate

responsibility for personal health lies with each employee. Managing Director carries the ultimate responsibility for Health, Safety & Environment (HSE) and expects each and every employee to be as equally committed to achieving this objective.

1.3 PY-1 Block Location Details:

The PY-1 offshore block is located in the northern part of the Cauvery basin. The block encompasses an area of 75 square kilometres in Bay of Bengal where the water depth ranges from 40 m to 250 m. The aerial distance from Porto Novo town to nearest boundary of PY-1 block is about 18 km east, while from Cuddalore town it is about 25 km towards southeast. The offshore PY-1 block is off the coast of Tamil Nadu and the details block co-ordinates are given Table-1.

There is no land/coast within 10 km radius from any of the PY-1 block boundary. The block location is shown in Figure-1. 1.4 Onshore Terminal: The Onshore Gas Processing Terminal will control the day-to-day process operations of the offshore platform. The Onshore Terminal is located at Pillaiperumal Nallur (PPN) village, 3 Kms from Thirukadaiyur town, Tarangampadi Taluk, Nagapattinam District, Tamil Nadu. Installation Manager & all onshore & offshore operations crew will be located at the onshore terminal; with offshore operators visiting the platform as & when needed. Normal access to the platform will be through fast crew boat. During operational exigencies, helicopter services will be made available for normal transportation between offshore and onshore terminal. Both the offshore platform and onshore terminal have helipad facility and also for onshore terminal medevac, if needed. All the offshore operations will be controlled from the Onshore Terminal Control Room through micro-wave link. The OT has a radio room with facilities for communications like aero VHF, Marine VHF and walkie-talky. 1.5 Shore-Base Details and Logistical Support: The Shore Base for all operational and logistical support will be located at Chennai. It also is intended to establish a supply base at Karaikal Port. The drilling warehouse is located immediately adjacent to the Onshore Plant. Personnel transfer will be by Helicopter from Pondicherry Airport or by fast crew boat from Cuddalore. The flying time to the drilling location by Helicopter will be approximately 20 minutes. travel time of Offshore Supply Vessel (OSV) from Chennai to the platform location in the block will be around 9-12 hours depending on the weather. The Onshore Terminal has a helipad provision though the helicopter will be operated from Pondicherry Airport. The offshore operations will be supported by an offshore supply vessel and the fast crew boat. The offshore supply vessel will be standby at the platform during helicopter movement to and from the platform and will standby the platform whenever personnel visit the platform for maintenance purposes. The field will have a patrolling vessel for security purposes. 1.6 Platform Location: The SUN Platform is designed for unmanned operations. The platform consists of four decks; Helideck, Main deck, Cellar deck, and Sub-cellar deck. The SUN Platform coordinates are given in the table below. One well had been drilled from the platform location prior to the installation of the platform through a sub-sea template. The subsea template has 9 slots to accommodate future wells. However during this current campaign HOEC plans to drill three wells including one tie-back well.

2.0. STRATEGY PLAN 2.1. Purpose and Objective: The purpose of developing the action plan for use during an offshore emergency is to achieve the following objectives:

• Prompt action initiation to minimise any adverse effects to personnel, environment or property;

• Ensure adequate resources are available;

• Familiarise all persons, involved in handling emergency & with the Emergency Response Plan (ERP);

• In the event of a blowout resulting in oil spill, the primary response would be to secure the safety

of personnel, control the well and the secondary response will be to combat the oil spill. If required external assistance would be mobilized;

• Ensure notification is given to relevant company, parties and Government agencies.

2.2. Scope: The plan details the actions to be taken from the notification of an emergency till its termination. Frequency of exercise, review of the plan, revisions of the plan, enquiry and resumptions of operations are also detailed. The ERP covers all the emergency scenarios anticipated in the PY-1 Dev. Project and adequately covers:

a) Offshore Platform; b) Pipeline; and c) Onshore Terminal.

2.3. Emergency Response Training & Exercise: The personnel at the site shall be adequately trained on personal safety, rig safety and fire fighting. Emergency (mock) drills shall be carried out regularly atleast once in a week during drilling operations and once in a month for platform & onshore plant operations. The risks involved in the offshore operations have been identified through Quantitative Risk Assessment (QRA) process and the following are the identified risk scenarios:

There will be atleast one major emergency exercise prior to any major activity like drilling and major modification to the offshore/ onshore facility, to determine the efficiency of the ERP. The exercise will be organised by the Production/Drilling Manager & HSE Manager. Reviews will be ordered after each emergency to discuss actual events and to identify & implement corrective measures. 2.4. Revision of ERP Production Manager shall be the custodian of this ERP, who is the authorised person for Management of Change. Revisions, if any, shall be discussed with the HSE Manager and other team members and then circulated. The ERP shall be revised after:

• The critique of the mock exercise • A major emergency, if any • If any major changes take place in the facility. • Only the pages in the manual, which undergo changes may be revised and dated.

2.5. Major Emergencies

In case of major emergency, an enquiry will be ordered to investigate the cause of the emergency and to identify the remedial measures. The Asset Manager will order the investigation process. The investigation committee members will be identified on the basis of their knowledge and exposure to emergency. The committee will invariably include the HSE Manager.

The ERP will be reviewed keeping in mind the findings of the investigation and an action plan firmed for completion of the identified remedial actions. 2.6. Response Objective The safety system of the platform / rig will be followed in combating an emergency; however HOEC shall provide all possible assistance. The following are the response objectives:

• In the event of any emergency (ex. blowout resulting in an oil spill), the primary objective would be to safeguard the life of personnel, and the secondary concern will be control and minimise any environmental concerns;

• In the event of any major offshore emergency such as fire or gas release, the first person noticing

it will take appropriate actions at the earliest. He will inform the Offshore Installation Manager (OIM) / HOEC Drilling Supervisor, who in turn shall decide further course of action, as needed;

• Incase of any major cyclone warning or Earthquake alerts, all equipment shall be secured and all

the personnel will be ready for evacuation;

• To adjudge the effectiveness of the response plan, and to effect any changes needed for even better response strategy.

2.7 Notice of Events: 2.7.1 Agencies to be informed:

• Joint Managing Director / his nominee will formally communicate to all the authorities in writing; • Production Manager / his nominee will communicate with the authorities orally, in order to

convey the information/ seek assistance, during the initial stage.

2.7.2 List of Reportable Incidents: Intimation to Oil Industry Safety Directorate (OISD) shall be made on occurrence of the following type of incidents, as per P&NG- [Safety in Offshore Operations[ Rules, 2008, by phone, fax within 24 hours in form -2:

(a) an accident causing loss of life or serious bodily injury in connection with offshore operations;

(b) an explosion; (c) blow out; (d) major fire; (e) bursting of any pipeline or equipment piping containing hydrocarbon, steam, compressed air

or other substances whereby safety of persons is likely to be endangered;

(f) breakage or fracture or failure of any structure, equipment or system, whereby safety of persons is likely to be endangered;

(g) any accident due to explosives or radio active substances; (h) collision of helicopter or vessel with the installation; (i) helicopter crash; (j) man overboard; (k) release of hydrocarbon or other noxious substances whereby safety of persons, installation or

marine environment is likely to be endangered; (l) uncontrolled movement of vessel or object in sea whereby safety of person onboard the

offshore installation or the installation itself could be endangered; and (m) any person present onboard an offshore installation or attending vessel or helicopter is found

to be missing

2.8 Control of Operations The responsibility for controlling the emergency shall primarily rest with the OIM of the rig/ Installation to the emergency situations as listed below: [list is not all inclusive - refer List of Emergency Procedures above for complete list]

The role of the HOEC at the Installation / rig and Chennai base will be to assist in all possible manner, arrange logistics and material support, keep the Top Management and regulatory authorities informed and updated, arrange for press release and press briefing, inform next–of-kin before releasing the information to the press and other agencies. 2.8.1 Planning long term Operation In case the emergency is likely to last for a long period, arrangements will have to be made to ensure that necessary personnel and facilities are in place. This involves assigning persons in shifts to man the Emergency Control Room (ECR) maintaining communication links, arranging for food and amenities, Hotel and transportation arrangements, Media management etc.

2.8.2 Updating information It is not uncommon that during the emergency the base office tends to hamper the pace of operations at the installation/ drilling rig by demanding excessive information to the extent that the command and control operations are affected. It is therefore desirable that a suitable interval is decided in advance for updating of information. Similar arrangements will have to be made for updating information to the press and regulatory authorities. Care should be taken that only authorised spokesperson will disseminate information. Care should also be taken to ensure that no other employee or contractors divulge any information regarding the emergency to any one. 2.9 Communication / Action log All efforts should be made to ensure that a log of events is maintained and preserved at the offshore installation /rig as well as at the base and concerned Radio Rooms. These event logs become valuable later for any enquiry and also facilitate in taking corrective action. 2.9.1 Press briefing and Media Management The Managing Director/ Jt. Managing Director will address all communications with the media and he will be the Spokesperson. All persons, at Chennai Office, Shore Base, Onshore Terminal and Helibase, when approached by any agency or media shall direct them to the spokesperson. 2.9.2 Information to Next- Of- Kin In the event of a fatality/ major injury, the information to the next-of-kin shall only be released by HR for HOEC employees. Management of Service companies and Drilling contractor are expected to notify the next-of-kin for their respective employees. It should be ensured that No Information is released to the Press or any outside agency before the information is provided to the next-of- kin. Also in the event of a fatality, appropriate authorities e.g. DGH, DGMS, Police and District Magistrate should be informed at the earliest. 2.10 Termination of Emergency Operations When the Emergency is brought under control, the HOEC Installation Manager (OIM) / Drilling Supervisor will inform Production Manager /Drilling Technical Manager at the base. Production Manager will then call to stop the emergency control room operations and inform all concerned.

2.10.1 Resumption of Normal Operations: Upon termination of emergency operations, the OIM and HOEC Drilling Supervisor, along with key members of his team shall conduct an evaluation of the platform / rig to find out what corrective measures are required and determine if it is safe to resume normal operations. Resumption of operations shall be announced over the Public Announcement system at the platform / rig and all personnel shall be briefed on further course of action.

2.10.2 Follow Up and Enquiry A team of the appropriate persons shall also be deputed from Chennai office to evaluate the situation on the spot. Their findings must be given in the form of a written report. In case of a major accident and injury, Managing Director will appoint an enquiry committee to determine the likely causes, circumstances leading to accident, corrective measures to be initiated etc. The HSE Manager will always be part of the team. The report must be submitted in writing to the Managing Director.

HSE Dept. shall follow-up till all on action items till completion. Items remaining must be flagged and taken up in every weekly meeting till closeout.

The emergency response set-up is categorised into 1. Emergency Response Group (ERG) members 2. Emergency Management Team (EMT) members

The Emergency Response Group (ERG) is the field-based team, which activates the emergency response immediately on realising the emergency.

The ERG shall further be organised as:

1. Emergency Response Group (ERG) leader 2. Forward Controller (FC) 3. Incident Controller (IC) 4. Emergency Response Team (ERT) members; 5. Radio Officer (Event log)

The Installation Manager /Drilling Supervisor shall be the Emergency Response Group (ERG) leader.

The Production Supervisor / Tool Pusher shall presume the role of ‘Forward Controller’ (FC). The Forward Controller shall be located at the Terminal/ rig control room. He shall direct and advise the Incident Controller (IC) on the course of action to be taken and status periodically. The Forward Controller (FC) shall constantly update the Installation Manager who in turn updates the Emergency Management Team (EMT) at Head Quarters. The Production Operator / Driller shall take the role of the Incident Controller (IC) and shall be at the scene of the incident along with his team members, to control the emergency.

The Emergency Management Team (EMT) shall comprise of the following members:

1. Production Manager (EMT Leader) 2. Asset Manager 3. Principal Facilities Manager 4. Drilling Tech Manager 5. HSE Manager 6. Scribe (Event logging) 7. Logistics In-charge 8. Administration In-charge

3.2 Emergency Response Once the emergency message is received from the rig, the Principal Facilities Manager /Drilling Tech Manager shall inform Production Manager / Head – WCD, who after discussion with his team and if necessary, will activate the Emergency Management Teams (EMT) to the Emergency Control Room

3.3 Emergency Control Room at HOEC office and Site The emergency control room for all HOEC operations will be the Conference Room (4th Floor) at Chennai Office. The local emergency control room, at the drill site, will be HOEC Installation Manager/ Drilling Supervisor’s office. The alternate ECR at Rig will the OIM’s Office. The emergency control room shall have the following:

1. Updated Data Directory; 2. Platform / Rig Plan; 3. Block Area Map; and 4. Drilling plan / Details of Onshore & Offshore Facilities

The emergency control rooms shall be well-equipped with all basic provisions like phone, fax, internet, continuous electricity supply, papers, pen and pencil. These supplies shall be maintained regularly and shall used during an emergency. The ECR shall have a continuous power supply with Un-interrupted Power Supply (UPS) and shall be provided with emergency lights, boards and marker pens and dusters. 3.4 Call Out Procedure 3.4.1 During Daytime

The OIM after being notified, contacts HOEC Installation Manager /Drilling Supervisor at the installation. The Drilling Supervisor then informs Production Manager. The Production Manager convenes the meeting of the Emergency team members at Emergency Control Room. 3.4.2 During Night time

The OIM informs HOEC Drilling Supervisor on the rig and Chennai Radio room. The Drilling Supervisor informs the Production Manager / Drilling Tech. Manager at Chennai. The Production Manager will arrange to contact all emergency team members and ask them to assemble at the Emergency Control Room. The EMT Leader will inform the Joint Managing Director regarding the incident.

All Emergency team members will make all endeavors to assemble at the Emergency Control Room at the earliest. The names and contact numbers of the Emergency team members will be posted on the Emergency Control Room and notice board.

3.5.1 Medivac

• Medivac by helicopter would be undertaken in case of serious injury or serious medical condition requiring urgent medical attention.

• Person noticing injury or medical condition initiates the action by informing the Tool pusher /

Medic onboard (Drilling Rig) / Production Supervisor. The Medic onboard attends to the injured and determines if Medivac is required, in consultation, if necessary with shore base doctor & OIM.

• The Drilling Supervisor / Production Supervisor will advise the Drilling Head /Installation

Manager of the need for a Medivac. He will also ensure that the Logistics team makes suitable arrangements for ambulance/ hospital etc.

• If it is clear that medivac is required urgently, the Offshore Supervisor can initiate it and then

advise the Installation Manager. In case of Onshore plant requiring aero-medevac, the Installation Manager will initiate aero-medevac arrangements.

• In case serious injury / medical condition, someone must accompany the injured, depending on

the condition of the injured – it could be the Medic himself.

• Medic shall complete the Medivac Form prior to arrival of chopper. On the other hand it shall be faxed to Chennai office.

• If injured person is not from HOEC, then 3rd party company management/ concerned contractor

will be notified by Production Manager.

• Radio Officer (RO) at Chennai Office will inform approximate arrival time of the Helicopter to the rig and RO of Installation informs the approximate time of arrival (ETA) at base.

3.5.2 Action at Shorebase: The Logistics In-Charge will carry out the following:

• Informs the Helicopter Company to mobilise the Helicopter and obtain necessary clearance from Director General of Civil Aviation (DGCA).

• Arranges to get a doctor at the airport to receive the medivac patient, brought by Helicopter, and

also organises ambulance and hospital admission. 3.5.3 Night Operations: Night landing facility is not available at Pondicherry. Chennai Airport has night landing facility to carry out any emergency MEDIVAC during night time. The Chennai Air Traffic Controller (ATC) should be

informed of the medivac flight at the earliest possible considering the growing air traffic at Chennai airport. (It takes about 45 minutes to an hour to mobilise the Pilots and Airport officials). PY-3 field, which is in the near vicinity and with Helibase at Chennai airport has been tied-up for provision of helicopter for a night Medivac situation during failure/ constraint on the regular HOEC helicopter services. For serious medical cases, HOEC has made arrangements with medical doctors and Hospitals (see Data directory).

acetylene bottles, petrol, lube oil drums, gas bottles, tyres etc.)

11. If gas fire, do not try to extinguish unless gas flow can be turned off IMMEDIATELY; if gas cannot be turned off, concentrate on stopping spread of fire and cooling activities (eg cooling exposed vessels etc)

12. Evacuate any personnel at risk to the Muster Point or to a safe distance upwind from the incident area and wait until advised otherwise

13. When rescuing personnel in danger, ensure own escape route first; do not endanger yourself by entering an area containing smoke and/or noxious fumes

14. Ensure all vehicles are moved away from the immediate area of the incident 15. Confirm all personnel are accounted for via Muster Point head count 16. If inside a building, contain incident by closing doors/windows/vents as you evacuate 17. Monitor events and determine need for full evacuation of personnel/equipment 18. Platform Evacuation ACTIONS

Tanker Oil Spill

Emergency Repair Procedures Following are the emergency repair procedures, which should invariably be followed while undertaking repairs on the pipelines: • No person other than a competent welder duly authorized, in writing by the Installation Manager,

shall carry out the welding or cutting work requiring use of flame or electric welding apparatus. • No welding or cutting work shall be undertaken by any welder in declared hazardous area unless a

written permit is issued to the welder by the Installation Manager. Copies of Hot Work Permit shall be entered in a Bound Register kept for the purpose. (Refer Hot Work Permit).

• Hot Work Permit shall be in the format as per HOEC Format shall specify the precaution to be

observed & procedures to be followed during welding or cutting work. No welding or cutting work shall be under taken in any hazardous area unless the area is duly examined and found gas-free by a competent person authorized for the purpose. A report of every such examination shall be recorded in a Bound Book kept for the purpose and shall be signed & dated by the person making the examination.

• It is advisable to lower a lighted torch bell hole before taking up any hot job even if the gas detector

has shown safe limits for working. • During welding / cutting operation, the welder shall see that all flammable material, such as oil,

grease, oil soaked soil etc. have been removed from the area. No matches, lighters or smoking material of any other kind capable of igniting flammable vapour is permitted in the area.

• Adequate precautions are taken to prevent sparks, slag or hot material. Adequate number of foam or

Dry Chemical Powder type extinguishers should be readily available for immediate use. Fire - Fighting Facility along with crew should also be deployed. Continuous fire watch with charged fire water hose may be kept available at the work location, as needed.

• When operations are carried out on pipeline, which contains flammable material, the line is

isolated, drained or purged with inert gas in case of gas or water in case of oil before hot work and adequate precautions are taken against pressure build-up in the line while hot work is in progress.

• The Installation Manager and Area Operator shall ensure that where Hot Work Permits are issued,

welding / cutting is done in accordance with the said permit. • A warning sign entitled ‘DANGER: WELDING & CUTTING IN PROGRESS’ is to be

prominently displayed. • If welding is to be done in an open line, then the Installation Manager will make sure that all

pumping units have been shut down in case of oil and clearance is given for closing the immediate near valve. These valves shall be closed & are locked to prevent operation by any unauthorised person. All valves so locked are to be tagged with a red danger board displaying ‘DO NOT OPERATE: WORK IN PROGRESS’: Also the valve wheel should be removed.

• Positive Isolation is a must for carrying out a hot job. A closed valve should not be considered as a positive block. It is desirable to isolate, drain & purge the pipeline with inert gas or fill it with water.

• First Aid Box with medicines, burn dressing etc., stretcher and a stand-by vehicle should be kept

near the work site. • Trenches/ Pits/ Bell Holes for repair of pipeline should provide sufficient room for the welder to

work safety and to make quick exit in emergency for which steps/ladder of appropriate height should be kept ready.

• A safety belt with lifeline attached should be used by the welder/ technician who works in deep bell

holes. The lifeline should be held by an experienced person during the operation. • All automobile equipment, welding machines, tractors and portable pumping equipment should be

fitted with the flame/ flash arrester on their exhaust and should be kept at least 30 metres from the work place. The operability of the equipment should be checked periodically.

• When pipeline carries induced current because of Cathodic Protection (CP), the same should be

removed effectively before work is done on the line. Rectifier must be turned off or disconnected. • No hot job shall be carried out after sunset unless there is an emergency and requirement is certified

by not less than Installation Manager himself. In such cases only flameproof lighting system, shall be used within the vicinity of the area. All vehicles and other equipment shall be kept at least 30 metres away from the work place.

Extended Emergency

Some emergencies may extend over a long period of time requiring a change over of personnel. With extended emergencies, site response personnel, ERG and EMT members will need to be replaced by alternates to avoid fatigue.

Responsibility for arranging alternates is that of the individual members in consultation with the

site Incident Controller, ERG Leader or EMT Leader.

In the event that the site Incident Response, ERG and/or EMT is active for more than 12 continuous hours, the following procedure should be considered:

ACTION All Response Members: 1. Alert your alternate, giving time that he/she will be required at site Incident Coordination

Centre, ERC or EMC; (normally schedule for 8 to 10 hour shifts)

2. Stagger change-over time over a reasonable period to avoid concurrent change-over of several people and ensure continuity of response activities

3. Provide a complete briefing on emergency, current status, actions taken and actions required to be taken

4. Ensure any on-going external contacts are made aware of the change over 5. Stay with your alternate until satisfied that he is fully conversant with situation and duties 6. Ensure your relief is provided with all of your after hours contact numbers in case urgent

contact with you is required

7. Advise Incident Controller, ERG Information Coordinator or EMT Information Coordinator when your hand-over is complete and make arrangements for when you will next be required

8. Ensure you get adequate sustenance and rest in case you are required again

Remember.... A fatigued team member is a liability, not an asset!

If a person becomes ill or is injured, or is a respondent to an emergency for an extended period of time, it is necessary to advise their next-of-kin of the situation and to make appropriate arrangements on their behalf.

The EMT Leader and ERG Leader will coordinate Next-of-Kin notification activities with assistance from Administration staff.

10. Assure Next-of-Kin you will call them back as further details become known; maintain record of calls made, calls to be made, follow-up calls required

11. Arrange for personal effects of employee to be returned to home as soon an possible 12. Arrange a follow-up visit to Next-of-Kin, if required 13. Consider further visits to the Next-of-Kin to find out if further assistance is required 14. Keep EMT Leader and ERG Leader updated on Next-of-Kin notification; ensure Company

Oil Spill Contingency Plan Appendix II 1.1 Oil Spills Offshore operators are required by the Indian Government to hold an approved Disaster Management Plan, which includes an Oil Spill Contingency Plan. It should be noted that according to Indian regulations governing the operation of offshore installations

• Any oil spilled from a platform is the responsibility of the platform owners up to 500 meters radius.

• If the spill spreads beyond 500 meters from the platform the Indian Coast Guard assumes

authority for direction of the spill response, but the operator of the offshore facility must provide resources for the Coast Guard.

1.1.1 Spill Response Action: • KEI –RSOS/ any other agency – Oil spill response support, stationed at Thirukkadiyur, will be

mobilised immediately for spill response. • Oil Spill Dispersants (OSD) in drums of 200 Litres are located onboard the rig, supply vessels and at

Jetty.

1.2 Definitions

1.2.1 Reportable Oil Spills All escapes of petroleum to the sea must be reported to the:

(I) Ministry of Environment and Forests (immediately) (ii) The Indian Coast Guard in Delhi (immediately) (iii) State Pollution Control Board (later, for states where onshore impacts may occur)

Assistance may be sought from:

1.2.2 Spill Severity A spill, which occurs relatively close to the coast, or in an environmentally sensitive area, has the potential to create a much greater environmental impact than one occurring elsewhere. For ease of reference in reporting spills they shall be defined as follows:

MINOR SPILL = a spill of less than 10 tonne

MAJOR SPILLS = a spill of 10 tonne or more

• A spill, which remains within a 500-metre radius of the rig/ platform, will be dealt with by HOEC. HOEC will have spray booms and sufficient stocks of Oil Spill dispersants available on the drilling rig, shore base and the supply vessels.

• Spills, which go beyond the 500-meter radius, will fall under the jurisdiction of the Indian Coast

Guard. HOEC will seek assistance from the Coast Guard for responding to the spill. • Support from East Asia Response, Ltd., (EARL), Singapore, will be considered if resources, other

than those available within HOEC and Coast Guard, are required. HOEC will have a formal tie-up/ contract with EARL, for this service.

1.2.3 Pro forma for Reporting Spills Appendices 2-1, 2-2 and 2-3 (Spill Report Form, Spill Worksheet and Dispersant Plan) have been compiled to assist offshore personnel in the completion of reporting forms on discovery of an oil spill. Its aim is to facilitate the recording of observations, spill data and standard information e.g. installation name and position prior to sending details to the appropriate organisations. For reporting to Coast Guard about any Oil Spill following format should be used, which have been taken from National Oil Spill Contingency Plan of Coast Guard.

1.2.5 Oil Spill Incident Form The incident reporting form (Appendix 2-4) aims to establish a mechanism for investigating the cause of spills, no matter how small. The HSE Manager should undertake analysis of these reports and any developing trends noted. If necessary the Emergency Response Plan shall be updated to incorporate recommendations from this exercise.

1.3 Procedure for Response to Oil Spills 1.3.1 General Responsibilities The Chief-WCD is ultimately responsible for ensuring the actions taken in the event of a spill conform to the requirements of this manual. The OIM will be responsible for immediate containment of the oil spill using the resources at the rig and supply boats.

1.3.2 Immediate Actions to be carried out by the OIM and / or HOEC Drilling Supervisor.

1) Attend to the safety of personnel. 2) Instruct Radio Operator to issue “Spill Notice” (very brief message) to Chennai office, shore

base, (which may be sent to the helibase for aerial surveillance if needed). 3) Report to the Head - WCD 4) Initiate all possible actions to stop further discharge of hydrocarbons into the sea. 5) Inform standby (multi-service) vessel of details of spill as available.

If the spill is of gas or condensate, NO CLEAN UP ACTION SHOULD BE TAKEN. If the spill is of oil, Dispersant spray should be applied if deemed appropriate or effective (see Appendix 2-5).

Supplies of dispersant will be maintained on the standby vessel, Rig and at shore base. A record of distribution to various locations should be maintained by the Logistics In-Charge, who will also be responsible for initiating action in good time for replacement of dispersant.

A record should be kept on the amount of Dispersants used. 6) Begin the procedure of recording details of the spill for reporting to the relevant personnel.

Complete a Spill Report Form, Spill Worksheet and Dispersant Plan (see Appendices 2-1,2-2 and 2-3). Transmit the data to personnel as appropriate.

7) Inform the appropriate authorities, which will include the Coast Guard, HOEC-HSE, MOEF, State Pollution Control Board and, if necessary, onshore emergency services and East Asia Response Ltd. Fatalities must be reported to the Directorate General of Hydrocarbons (DGH), Ministry of Petroleum, Directorate General of Mines Safety (DGMS) within 24 hours.

8) Initiate forecasting of the projected slick movement if possible (see Appendix 2-2). 9) Request all vessels in the area to report on the direction of movements of the spill, the rate of

dissipation, and distance from the installation etc. This information should be collected at regular intervals.

10) Initiate aerial surveillance of the slick using available helicopters in the area, as directed by

HOEC Emergency Management Team (EMT). Estimates of slick size and movement should be reported (see Appendices 2-2 and 2-6).

11) Whenever, the oil slick is thought to emanate from any source other than the installation in

question, samples of the oil should be taken either by the standby vessel or from the installation via its own launch. Samples should be collected using glass jars cleaned with dichloromethane, labeled according to position and name of vessel, date and time of sampling and then sent ashore for analysis.

12) Identify size of beach clean-up teams and select team leaders. 13) Continuously track the oil slick, together with 24-hour assessments of shoreline threats. 14) At the end of the incident, Production Manager should advise / agree with the HOEC Emergency

Management Team that the incident response is over and advise the standby vessel. The Emergency Management Team at the base shall notify the other organisations involved regarding the termination of operations.

15) At the earliest opportunity, the cause of the spill should be fully investigated and an Incident

Report Form (see Appendix 2-4) shall be completed. 16) Details of the oil spill containment resources available to combat a spill are presented in Data

Directory. Technical details are provided in the Data Directory.

1.3.3 Reporting Requirements

The following requirements are to be performed by or as requested by the Production Manager as soon as possible.

1) Contact the Emergency Management Team and the Managing Director. For telephone and

mobile numbers of the Emergency Management Team Members (see Data Directory). If the spill is less than one tonne, confinable within a 500m radius and the installation can deal with the spill by itself, there will be no need to alert the Emergency Management Team, but the Drilling Supervisor must complete a Spill Incident Report Form.

2) If further information is required immediately, this will be requested by telephone and should be confirmed by email or fax with appropriate distribution.

3) During a major spill, information should be communicated to the appropriate personnel at

suitable intervals to enable the Emergency Management Team to make the best decisions. 4) When the incident has been declared over, an Oil Spill Incident Form (see Appendix 2-4) should

be prepared and distributed to the appropriate personnel within HOEC. 5) In case of a major spill where we will be utilising the services of EARL (East Asia Response

APPENDIX - III Action Plan for Well Blowout

Emergency Response Procedures In the event of an actual or potential blow-out:

1. The Operator /Driller raises the alarm, informs OIM. OIM takes steps to stop or control the emergency ensuring there is minimal risk to personnel. OIM will follow the Installation /Rig’s action plan to meet such events.

2. OIM / HOEC Drilling Supervisor notifies the Drilling Manager. The Production Manager notifies

the Management and activates the EMT which will advise the local authorities, emergency services and resource depots (Data Directory).

3. OIM / HOEC Drilling Supervisor evaluate the situation and consider three key issues to

determine whether the emergency is a minor or major incident:

ii) Are external emergency resources required? iii) Do rig personnel require external medical assistance? iv) Does full or partial rig abandonment of personnel need to be considered?

4. Drilling Supervisor updates the Facilities Engineer /Drilling Manager on status of the incident. 5. The Production Manager activates and manages the Emergency Response Plan in the event of a

major incident requiring the help of external agencies. 6. At the completion of the incident response, the Production Manager should advise/agree with the

Emergency Management Plan that the incident response is over.

7. At the earliest opportunity the cause of the incident should be fully investigated and an Incident Report shall be completed by the Installation Manager and the Production Manager, and distributed to appropriate personnel.

8. In the event of fatalities, a report must be made to DGH & DGMS, within 24 hours.

9. In case of fatalities of HOEC employees, the Company Spokesman will notify the next-of-kin of the deceased. The family will be notified first before communicating with the media.

APPENDIX - IV Action Plan for Helicopter Crash on Deck

Emergency Response Procedures

1. The first person who is aware of the incident raises the alarm and takes steps to minimise the

impact of the emergency ensuring there is minimal risk to personnel. 2. All personnel collect their life jackets and gather at the muster point in preparation for rig / platform

abandonment.

i) Drilling Supervisor / OIM notifies the Drilling Manager. The Production Manager notifies the EMT which shall coordinate with the local authorities, emergency services etc. Contacts helicopter operations for confirmation of helicopter personnel and flight plan.

ii) OIM/ Drilling Supervisor will deploy rescue, fire team and first aid personnel to the incident

area and provides on-scene co-ordination and assessment. He decides whether to shut down rig electrical power. Priority must be given to removing occupants from helicopter.

3. Facilities Engineer / Drilling Tech Manager evaluates the situation and considers three key issues to

determine whether the emergency is a minor or major incident:

i) Are external emergency resources required? ii) Do Platform / Rig personnel require external medical assistance? iii) Does full or partial abandonment of personnel need to be considered?

4. OIM / Drilling Supervisor updates Drilling Manager on the status of incident and takes photographs

to support subsequent incident reports. 5. If the emergency is considered to be a major incident (i.e., a disaster), the OIM/ Drilling Supervisor

notifies Drilling Manager. Production Manager calls together the Emergency Management and notifies other appropriate personnel (Data Directory).

6. The EMT Leader activates and manages the Emergency Response Plan in the event of a major

incident. 7. At the end of the incident the EMT should advise / agree with the Emergency Response Team that

the incident is ended.

8. At the earliest opportunity the cause of the incident should be fully investigated and an Incident Report shall be completed by the OIM/ Drilling Supervisor and shall be reviewed by the Production Manager along with the HSE Manager prior to distribution to the appropriate personnel within the Company.

9. In the event of fatalities, a report must be made to DGH within 24 hours.

Note: Management must consider the safety of the rig and helicopter personnel as priority before consideration of the equipment or operations and not delay evacuation of the personnel.
