Survey Monkey re waste – showed that patients were not aware of how to dispose of medicines and some were flushing down the toilet and putting in the Council recycling bin. Campaign will be promoted by poster, social media, press – in gp’s, pharmacies, etc Could highlight the costs of the medicines – using social media – questions with multichoice answers to raise awareness of costs. Doncaster specific cost re waste?? Looking for celebrities that could front the campaign Look at reasons for waste

- 56 / 84 day prescribing. If change the medication in the middle of the 56/84 days then goes to waste – rather do 28 day prescribing

- Looking at EPS repeat dispensing - Inhaler technique checks (especially under 18’s) – need to extend the current service to include

children – pharmacies would just need some training on how to interact with children. NH / RH to send through the information on the service to Mark Randerson.

- Are the repeat prescriptions being checked properly at the gp practice – if been reviewed by gp and they issue for 6 months then the staff do the prescriptions until due for review again. These patients could be on repeat dispensing.

Is there a list of community groups that LPC / Pharmacy can go and speak to over coffee etc – CVS has a weekly bulletin that goes out to local voluntary groups.

Poster asking public to ‘check theirs and relative’s cupboards for medicines they don’t use and hand into pharmacy.’ MDS - if there is a change then all of the medicines in the MDS have to be thrown away so becomes very wasteful Need to work with gp practice staff and pharmacy staff around training – need to deal with the pharmacies who are not following SOP’s – this is a minority. Need to change patient’s perception of ordering medication. Need to get back to pharmaceuticals being special

Campaign plan European Antibiotic day in November 2015

- Multidisciplinary training on antibiotics Waste Campaign – January 2016 Point out inappropriate doses of antibiotics – pharmacy reporting back to gp practices Working in practices – highlighted that could be a way of extra pharmacy support – if there is funding then work with community pharmacists Sheffield have a pilot where every practice is taking on a community pharmacist to work within the surgery. Focus on recovery week this week. - Hatfield Waterpark – 200 people there - Recovery walk this weekend

Training – what areas wish to cover

1) Basic drug and alcohol awareness 2) Service structure – guidelines to follow, client pathway, service flow. Why the substance

misuse team ask for information and what is done as a result? Agreed that would like to go through service structure – suggested that some of the drug workers attend pharmacy to see how the system works from the dispensing side. Two dates – LPC will advertise and manage bookings. November and February ?? Aiming at any frontline staff who will be involved in substance misuse service. Have a non-medical prescriber joining the team – pharmacist Missed pick-up reports Had an incident recently where someone had not picked up in two weeks but the pharmacy had not sent through missed pick up reports and had led to an incident report being completed and social services being involved due to safeguarding concerns. Suggested that use these incidents as case studies at the training Advised that the service should inform key people (Superintendent / Area Manager) in organisations as well as individual practices. All organisations will have an internal reporting system. Sheffield and Barnsley use PharmOutcomes and Rotherham use Neo – they both have less issues as info is sent electronically. Louise reiterated the need to have fax headers sent through with reports stating the number of pages so RDASH can tell if they have received all pages through. Shortages Recent shortage of 0.4 Subutex – some clients are not willing to change. Pharmacies are not always made aware of shortages by wholesalers until they go to order them. Manufacturers will not alert because of issues with hoarding and pricing. Normally pharmacies will contact each other and see if they have any in stock.

1) Strategy plan Merged and taken things out to simplify – later on agenda

NH to look at presentations and put together a key message that members and contractors can use when talk to groups Generic presentation – NH has not done yet – NH has versions that have been used that can be adapted by any member in the meantime. Will do a generic one eventually to go on the website although there are already templates on the PSNC website. Write out to local groups and schools in the area and ask them to go and talk to their local pharmacy re attending their groups to explain about pharmacy services. Not done a mail-out to groups and schools – need to do in areas and work with others too. EHC and Inhaler Technique - Ventolin inhalers in schools – DMBC were writing out to schools. Up to individual schools not a legal requirement. 2) Substance misuse Information has been sent out via bulletin on providing fax cover sheets on missed pick up reports. Visiting pharmacies – contact Louise and see if she wishes to visit a pharmacy to see the process from the dispensing part of the service. 3) EPS Contact Joanne Lamb around repeat dispensing HSCIC have started the program of training – 700-800 workshops over the next 7 months 4) Health Education Yorkshire and Humber West Yorkshire LPC was the only LPC to receive funding through Health Education England. Started to get some information re training from Y&H Health Education since NH contacted them. Agreement from HEE that they would fund training but would not have the time to put on now. 5) APC NH raised the checking of booklets for lithium, insulin and methotrexate with Mark Randerson and asked if he would raise at the APC but the APC had asked that the LPC write to them – PC to raise at the next meeting of the APC, because APC had previously said to raise through Mark

6) Sexual health tender – TriHealth

LPC have not received the list of pharmacies that are providing the services.

SLA is now out of date

Amy Booth, DMBC – has contacted Lynn Murrie to say that her Lloyds Pharmacy are now able to

provide the service as a new provider and she is sending out the SLA to her.

CCG have asked NH to put proposals together about how manage to keep to budget – worried about

overspend. ?? include a cap on activity. NH has sent through some information to Martha already

LPC conference

AE has booked places for MA and DS and also booked hotel places for DS on the Tuesday night after the Chairs and Secretaries meeting. 8) Internet access – Forward to next meeting

Action: RW will check if Weldricks can give access to the WIFI for LPC members.

4 Treasurer reports 4.1 Finances for July / August 2015 – Attachment C1 RH reported that the LPC have a healthy balance and may be able to have a levy holiday in the future. 4.2 PSNC levy Asked Garry Myers to attend a LPC meeting to justify why they had raised the levy. National MAS scheme and National Flu service – had some success. Asked members if wish to now pay the PSNC levy – all agreed to pay the 1st part of the Levy.

Action: RH to send through the payment for the 1st part of this year’s PSNC levy asap

4.3 Budgeting for 2016/17 - New services and C&D awards? Have a few innovative services / pilots that the LPC may wish to put in for an award Chest x-ray project is main pilot. CCG want to advertise the services in Doncaster. RH to budget for this next year.

5 Secretary reports 5.1 PSNC 5 Point Forward Plan for community pharmacy flyer – Attachment D For information – worth reading through PSNC asked LPC members to bear in mind the 5 point plan and if have opportunity to raise these points and be on message. 5.2 Social Prescribing Steering Group - Attachment E1 and E2 They are attending the Breast Cancer training on 10.09.15 to explain the service they provide and how pharmacy can refer in. 5.3 All Party Parliamentary Pharmacy Group - Special Workshop and Briefing Session NH has been invited to a workshop – postponed until October 2015. Costs being split over 3 LPCs. Stage 2 – Lifelab website Webpage and leaflets have been produced. Sexual health – if cannot deal with it then will signpost to relevant services Stage 3 – Seasonal flu As the listener demographic is the younger generation need to add in words along the lines of ‘make sure your relatives have had the flu vaccine’. Total of 1000 hits to end of July 2015 - Hallam FM researchers are going to split down the data into areas.

6 Chair reports 6.1 Verbal report on meeting with Dr Nick Tupper and Dr Jeremy Bradley Have regular meetings – seem to go through the same items. Opportunity to pick up what is happening in the CCG. Changes to Out of Hours and 8 till 8 provision Nick Tupper and Jeremy Bradley were unaware of the decision CHC have won the Out of Hours service tender Contractual changes for gp’s. Also running the unregistered care – Dr Khan’s practice then moving into Devonshire House. The Flying Scotsman will see registered patients 8 till 8 – run by CIC company – from October 2015 Pharmacy need to be made aware of changes and dates these are happening – NH and RW will keep a check. Need to invite Nick Tupper and Jeremy Bradley to a LPC meeting and discuss the amount of pharmacies who are open during out of hours and about working together. EPS and waste campaigns were discussed Community pharmacists working in general practice Received around 30 requests from gp practices to have a pharmacist in the practice but none have followed up or committed to this once it was explained to them re funding. CCG said that they were not supporting as they were waiting on results from the Sheffield Pilot

7 Tenders and bids / Consultations / Service Opportunities / LPC future - update 7.1 Primary Falls Prevention & Physical Activity Service tender update Not sure if this is replacing the Specialist Fall Service Submitted response to the expression of interest. More than community pharmacy can or would want to provide. ?? work with other parties – LMC and Doncaster Rovers etc Current specialist falls service run by RDASH – it is not a commissioned service and do not have a pharmacist in the team NH to talk to Steve Davies, RDASH re joint working on the falls service tender.

- PSNC have sent out some comments - Have the RPS done this to start off a debate?

NH advised members to respond to the consultation personally and he would do one from the LPC.

8 Topics for discussion 8.1 Barnsley Medication Policy for domiciliary care – Doncaster CCG service proposal Asked if they will commission a service if they like it / fits their purpose? Not had an answer. Asking for volunteer to work on service proposal over a couple of weeks – DP said he was happy to help. Members to let NH know if they also wish to work on this service proposal with PC.

Action: Members to contact NH if they also wish to work on the Medication Policy for Domiciliary Care service proposal with PC.

8.2 MDS issues (PC) Increase in demand – where is it coming from and what is the logic behind it plus capacity to do. Mark Randerson does not approve of amending MDS trays and believes these should be redone if there are any changes. Links to the above item and if receive a payment to do. Ideally need to gather all the carers groups and ask why they write their policies to state that they need to have a MDS to be able to help with medication even when some medicines are not compatible to go in MDS and also some already have calendar packs. However, suspect it is misunderstanding of carer policies by middle management as in ones that have been seen in the past they do not stipulate MDS as being required. Have medical support for too – can link to medicines optimisation and waste. RW and NH meeting with Rupert Suckling, Public Health next week. Need to be paid to support vulnerable people whatever way it is - MDS / MAR charts etc 8.3 Potential training sponsor?? - New Adrenaline auto injector education - potential savings for members and CCG More relevant to frontline pharmacists – fund educational evening- probably when launch the falls service. 8.3 Discuss how the LPC can further promote dementia friends and dementia champions – to include considering surveying contractors to identify how many pharmacies have DFs and DCs. Comments raised around how much pharmacy can deliver around Dementia. Wishing pharmacies to sign up to the Dementia Action Alliance. No-one knows how many dementia friends pharmacies have and if they class themselves as Dementia Friendly - Survey contractors ?? Good PR with CCG and DAA – RW felt that this is not for the LPC to be spending time on and CB suggested that should put on bulletin and ask them to contact the DMBC to let them know how many DF’s and DC’s companies have.

Action: Dementia friends and champions – bulletin article asking pharmacies to contact DMBC and let know how many Dementia friends and Champions etc they have within their pharmacies.

8.5 Check the meeting and venues are still ok and confirm meeting dates for the next year

Sovereign House (additional parking at Greasley House), Weldricks and Rutland House when both not available. Members happy to use these venues again next year. Stick with dates - 2nd Wednesday every other month Write to Garry Myers / Mike King – proposed dates for next year and how do they sit with PSNC meetings – if all clash then change a couple of the LPC meetings dates (back to 3rd Wednesday).

Action: AE to send through the proposed dates for next year (2nd Wednesday every other month) and see if all clash with PSNC meetings – if so then move a couple of LPC meeting to 3rd Wednesday to enable Garry to attend the LPC meetings

9 Members reports 9.1 APC – RH stepping down as LPC rep - suggest PC leads on and members identify a new deputy RH is taking on a role in Nottinghamshire and not able to cover the APC. PC is deputy member and has expressed an interest that he would like to take on this role. Members agreed that Paul Chatterton would attend the APC meetings Need to appoint a deputy to attend APC in place of Paul Chatterton. Members agreed that Mohammed Ahmed would be the deputy for the APC meetings. RH asked how can community pharmacy help with the problem of Antibiotic resistance

- Ensure take the compete course - Multidisciplinary training would be very helpful

Feel it is a LMC and CCG issue not pharmacy as they are the prescribers. RW feels that this is more relevant to the LPF. Can encourage pharmacists re their professionalism and provide joint learning.

10 Research 10.1 Codeine project update – event on 15th Oct (RH) Dissemination event – open to external people other than Weldricks employees. Those who have been in secondment will be feeding back and also academic from South Africa will be presenting. This is at Castle Park Rugby Club – pre-regs are welcome too. Will need numbers attending as putting on food for the event Invite people from substance misuse team – RW to do

10.2 MMS Evaluation funding request from Barnsley LPC – Attachment G1 and G2 Tom Bissett trying to get funding for evaluation so can be used as a tool to help with services being commissioned. Had scheme since 2008 and no-one has ever evaluated it. Been collecting data on PharmOutcomes over a year and then Claire spent 5 days analysing the data and providing a report. NPA have funded £6000 and they are asking Tom to find the rest of the funding. He had asked PSNC but they have refused. Need to fund the gap of £2000 – asking LPCs in SY to help. (Proportional to contractors per LPC). MDS scheme – social care or pharmacy can refer to the service Pharmacist goes out and assesses and then draw up a patient care plan – different levels of support Reimbursed for time. Should be followed up after 13 weeks. Why are the PSNC not funding evaluation ?? NH to ask Tom Bissett / PSNC Once have information back AE to send out a doodle poll asking yes or not for funding.

11 PSNC Communications Members are advised that many PSNC communications are now communicated electronically and repeated on the PSNC website. It is recommended that members regularly visit the PSNC website ‘ LPC members’ area to access the latest information available onhttp://psnc.org.uk/

12.2 NHS Health Checks in community pharmacies -survey results – Attachment I For Information Received SLA from Hallcross Medical Services at pharmacies Training – accreditation LM declared interest as Lloyds are in negotiations with Hallcross separately. Individual companies to look into the remuneration and workload and decide whether they wish to sign up to the service or not. 12.3 EPS - Deployment –Attachment J - Flavoured Products – issues in ensuring correct fees are paid (RH) Complex re payments for flavoured products – where multi / assorted flavour is being used then being paid for plain automatically therefore pharmacy are not being paid correctly. Difference between numbers for total count and the NHSBSA Problem is that there is different wording on practice systems - On the hints and tips from NHSBSA they have explained what to do Solution would be for them to create the DM+D for the assorted flavours

a. NHS Commissioning Board Area Team update i. Pharmacy Flu SLA 2015/16 – Attachment K

Concerned that there is conflicting information – need to ensure know which PGD and SLA they are providing the vaccine under. CPPE declaration of competence – challenged PSNC and they said that if sign the form then can provide the service.

b. Local Authority update

i. Doncaster Health & Wellbeing Board - Health & Wellbeing Strategy consultation – closing date 02.10.15

Had over 100 responses so far. Survey system can only accommodate 100 and so they have had to re-open the consultation Asked members to comment individually and then the LPC will also report in to the consultation.

ii. Tobacco Control meeting – Attachment L Looking at members of the tobacco control alliance – wish to score each member on their smoke free status

- No policy - Policy in place but don’t enforce - Policy in place and actively enforce

Would like pharmacies to report in what level they are at. All multiples have a policy and enforce and also MA reported that Eightlands have a policy and enforce it. Expecting government strategy Extended role for pharmacy in smoking Trading standards – problem with fake cigarettes in Doncaster. If get local knowledge then contact trading standards.

Action: NH will email Victor with the results from the smoke free status

SWYPT chasing on out of PGD Champix supply – Attachment O Medics are breaching CCG guidance if they write a prescription for Champix, because drug budget has been passed over to DMBC and then tendered out as part of the tobacco control service so controlled by SWYPT. Cannot be called a private prescription - it has to be a requisition by gp on behalf of SWYFT. NH to report back to Simon Lister. c. CCG update i. APC bulletin July 2015 – Attachment P

iii. Falls service update NH and CT are building the Level 1 platform and may be Level 2 on PharmOutcomes and also meeting with Elaine Thompson in the afternoon on Friday (11.09.15) Need to put training on for all contractors CT is leaving the LPC and so need to get completed while CT has the hours to work on. Hoping to set up reports to run automatically for the level 1 and then more evaluation will need to be done on the higher levels. Providing support with services – emails and phone calls for 8 pharmacies on Level 2 and then monthly emails and phone calls for the level 1 service

Still feel that there needs to be a development officer / support across SY . Doncaster are losing some of the active members who help to put some of the services and evaluation together and need to replace these in the near future.

d. LPN

a) Verbal report on any recent actions not covered elsewhere on the agenda - Will be responding to RPS / NAPC consultation

13 East Midlands Region 13.1 Y&H LPCs meetings – 6th Oct - ??? agenda items Robbie Turner from West Yorkshire LPC has sent out some items that wish to be discussed.

14 Strategy Plan Members to review the completed new strategy and agree final version

Attachment Q Handed round – taken out communication section – most of these items have gone into relationships Cannot combine DMBC and CCG in to one “commissioner” section – too much to cover. PSNC – not seeing the regional representative – trying to find a solution. There are a few gaps on the strategy – lead members – need volunteers

Action: Carry on reviewing the strategy / work plan at LPC meetings and encourage members to volunteer as leads for sections.
