Introduction TheseguidelinesareusedinconjunctionwithotherUniversitydocumentssuchastheETSUandTTUGraduateCatalogs,ETSU-TTUDNPStudentHandbook,andETSU-TTUDNPProjectGuidelines.TheETSUandTTUGraduateCatalogsalsocontainessentialinformationaboutUniversitypoliciesandprocedures.TheETSU-TTUStudentHandbookcontainsadditionalpolicies,regulations,resources,andservicesavailableforstudents.TheHandbookandGuidesarelocatedontheETSUCollegeofNursingwebsiteandtheTTUCollegeofNursingwebsite.ThisdocumentcomplieswithAACNEssentialsofDoctoralEducationforAdvancedNursingPracticetheConsensusModelforAPRNRegulation, Licensure, Accreditation, Certification, Education, NONPFCoreCompetenciesforNPEducation,Universitypolicyanddepartmentalpolicy. NursePractitionerScopeofPracticeUpongraduationfromanaccreditedschoolofnursing,anationalprofessionalassociationsuchastheAmericanAcademyofNursePractitionersortheAmericanNursesCredentialingCentermaycertifyNPgraduatesbasedonexaminationresults.StatelawisthemostpowerfulsourceofauthorityforNPpractice.Thestate’sNursePracticeActregulatestheNPspracticeandholdsauthorityforstatelicensure.Statutesoflegislatorsdefinethescopeofpracticeinsomestatesandthestateboardofnursingdefinesothers.ItistheresponsibilityofeachstudenttodeterminethescopeofpracticeforthestatewherethestudentwillpracticeandtoadheretotheBoardofNursing’srequirementsupongraduation.Reviewyourstate’sNursePracticeActhere.Nursepractitionerstudentsfunctioninalearningroleunderthedirectsupervisionofthelicensedpreceptor.Assuch,therearecertainpracticesandproceduresthatNPstudentsareprohibitedfromperformingwhileunlicensed.Further,patientsafetydictatesthatcompetencyisvalidatedpriortoperformance.Nursepractitionerstudentsareprohibitedfromperformingproceduresforwhichtheyhavenotreceivedpreviousclinicalvalidationfromfacultyoraclinicalpreceptorthroughreturndemonstrationand/orclinicalcompetencyexamination.TheclinicalpreceptormustprovidedirectsupervisionoftheNPstudentatalltimesduringanyinvasiveprocedures.Ifastudenthas

questionsaboutwhetherornotaprocedureisallowableintheclinicalsite,thestudentisrequiredtocontactthecoursefaculty.Studentsareencouragedtoobserveproceduresastheopportunityarises.Nursepractitionerstudentsarealsoatnotimepermittedtosignprescriptions,dispensemedications,ore-prescribemedications.Thisistheresponsibilityofthelicensedpreceptorandisbeyondthestudent'sscopeofpractice.Prescriptiveauthorityisgainedafterthestudentissuccessfullylicensedinthestateinwhichhe/shepractices.Thestudentisencouragedtoconferwiththepreceptorandmakerecommendationsregardingtheneedforprescriptions.AdvancedHealthAssessmentSkills&SimulationExperiencesNursePractitionerstudentswillhavetheopportunitytodevelopadvancedphysicalassessmentskillsthroughpractice.Studentsareaffordedtheabilitytouseequipment,suchasotoscopes,ophthalmoscopes,tuningforks,andreflexhammersavailableinthesimulationlab.However,studentsarestronglyencouragedtopurchasetheirownequipment.Thisallowsthestudenttohaveaccesstosuppliesoutsideofthelabandsupportsthestudent'sindependentpracticeofassessmentskillspriortoClinicalCompetencyExams(CCEs).
Note:Studentsarerequiredtohaveaccesstothefollowingforclinicalexperiences:ophthalmoscope,otoscope,stethoscope,reflexhammer,512-Hztuningfork,smartdevice(iPhone,droid,tablet,etc.),penlight,andretractablemeasuringtape.Suppliesareoftenlimitedinruralhealthclinicsandpurchaseoftheseitemscanensurethestudenthasessentialequipmentneededforpracticeandtheclinicalpracticumexperience.
TheSimulationLabsatETSUandTTUalsoprovidesNPstudentstheopportunitytopracticeadditionaladvancedskills.LabsmayofferthefollowingsimulationequipmentforNPstudents:(a)ventriloscopes(murmurs,heartsounds,lungsounds,etc.);(b)OpthaSim(reviewtechnique,normalinternalstructuresofeyeandpathology);(c)OtoSim(reviewtechnique,normalinternalearstructuresandpathology);(d)pelvicmodels(Papsmear,bimanualexam,andrectovaginalexam);(e)prostatemodels(reviewtechniqueandpathology);(f)testicularexammodels(reviewtechniqueandpathology).Somepractice/simulationtimemaybemandatoryforstudents,thiswillbecommunicatedinthepracticumcoursesyllabi.Studentswhowishtoarrangeadditionalpracticeand/orsimulationtimeshouldcontacttheLabCoordinatorateitheruniversityCollege/SchoolofNursing.WorkshopsDuringthesemester,theremaybeopportunitiestoattendworkshopsintheCollege/SchoolofNursingforskillsrequiringadditionalinstructionincluding,butnotlimitedto,suturing,incisionanddrainage,punchbiopsy,x-rayinterpretation,EKGinterpretation,andintraarticularinjection.Theseworkshopswillbeannouncedasavailable.FacultymakeeveryefforttoarrangetheseexperiencesforNPstudentsduringon-siteintensives.Workshops,simulation,and/oradditional

practicetimemayberequiredforvalidationofcertainskills(RefertoScopeofPracticeinformationabove).ClinicalCompetencyExaminationsNPstudentsmustsuccessfullypassa3-partClinicalCompetencyExamination(CCE)aspartofthecourserequirementsforNRSE5010:HealthAssessmentthroughouttheLifespanPracticumpriortobeginningclinicalexperiences.Thisincludes:(a)ahead-to-toeexam;(b)afocusedexam;and(c)aspecialproceduresexam.Thestudentmustscoreaminimumof80%oneachcomponenttosuccessfullycompletetheCCE.Studentswhodonotachievean80%oneachcomponentarenotpermittedtoentertheclinicalpracticumsite.SpecificinstructionsfortheCCEsarelocatedinthecoursesyllabus.NPClinicalSiteCriteriaStudentsshouldseekavarietyofclinicalexperiencesduringtheirstudies,includingbutnotlimitedtoruralclinics,urbanclinics,theETSUnurse-ledclinics,andothercommunitycenters.ClinicalsiteplacementisprimarilytheresponsibilityoftheNPstudent;however,thestudentmaygetclinicalplacementassistancefromcoursefaculty,ETSUOfficeofStudentServices(OSS),concentrationcoordinator,ProgramDirector/Coordinator,orETSUGraduateClinicalSiteCoordinator.Studentsmaynotusetheirworkplace(e.g.office,unit,departmentofemployment)asaclinicalsite.However,ifemployedatalargehealthsystem,thestudentmayobtainaclinicalplacementatanothersettingwithinthesystem.Obtaininganappropriateclinicalsiteisthestudent’sresponsibility.PleaserefertotheNPStudentClinicalSiteandPreceptorApprovalProcess.StudentsresidinginTennesseemaycontinuetheETSU-TTUDNPProgramifmovingtoanotherstate.However,therearestatesthatwillnotlicenseNPswhocompleteonlineprograms.Studentsareadvisedtocontacttheboardofnursinginthestatetowhichtheyaremovingtoassurethatlicensureinthestatewillbepermittedupongraduation.Additionally,notallstateswillallowNPstudentsfromotherstatestocompleteclinicalpracticumsinthatstate.Formoreinformation,pleaseaccessthefollowinglinkfromtheStateAuthorizationReciprocityAgreement(SARA):http://nc-sara.org/.Nursepractitionerstudentsshouldcontactthestateboardofnursingforclarification.Furthermore,priortorelocationthestudentisresponsiblefornotifyingtheETSUCONOfficeofStudentServices(OSS)orTTUSchoolofNursing,andhomeschoolOfficeoftheRegistrarregardingchangesinresidence.Thepatientpopulationseenduringclinicalexperiencesshouldmatchthestudent’sspecialization(i.e.FNP,PNP-PC,PMHNP,AGACNP,orWHCNP).Forexample,NPstudentsinprimarycaremusthaveclinicalexperiencesinprimarycaresites/settings,whereasNPconcentrationsinacutecaremusthaveclinicalexperiencesinacutecaresites/settings.Patientvolumeintheclinicalsitemustbesufficienttoallowthestudentanopportunitytoseeanarrayofpatientstomeetstudentlearningobjectivesforthespecifiedcourse.

Studentsmayspendnomorethan12hoursataclinicalsiteinoneclinicalday.Transittimetoandfromtheclinicalsitecannotbecountedtowardpracticumhours.Studentsareprohibitedfromdirectpatientinteractioniftheclinicalpreceptorisnotphysicallyattheclinicalsite.DefiningPrimaryCare:Primarycareisgenerallythepatient'sfirstpointofentryintothehealthcaresystemandismostoftenprovidedinanoutpatientsetting.Suchhealthcaresettingsprovidepreventivecareandteachhealthylifestyleschoices,whileidentifyingandtreatingcommonmedicalconditions.Primarycareprovidersaddressprimary,secondary,andtertiarypreventionmodalities,healthpromotion,diseaseprevention,patienteducation,counseling,referral,advocacy,comprehensive,andholistictreatmentplans.Thisformofcareisperformedandmanagedbyaprimaryprovideroftenincollaborationwithotherhealthcareprofessionals.Examplesofprimarycareinclude:(a)nursepractitioneroffice;(b)communityhealthcenter;(b)physicianpractitioneroffice;(c)long-termcare;(d)daycare,etc.(AAFP,2016).Studentsmayalsouseanurgentcarefacilityforclinicalhours.DefiningAcuteorNon-PrimaryCare:Acutecareprovidersorfacilitiesarethehealthsystemcomponentorcaredeliveryplatformaimedattreatingsudden,oftenunexpected,urgentoremergentepisodesofinjuryorillnessthatcanleadtodisabilityordeathwithoutrapidintervention.Suchproviderscareforimmediatelylife-orlimb-threateninghealthconditionsthroughacaredeliverysystemdrivenbyaresponsetoimmediatehealththreatsofindividualswithacutesurgicalneeds,myocardialinfarctions,acutecerebrovascularaccidents,evaluationofpatientswithabdominalpain,etc.Examplesofacutecareencompassarangeofclinicalhealthlocationsincluding:(a)emergencymedicine;(b)traumacare;(c)pre-hospitalemergencycare;(d)acutecaresurgery;(e)criticalcare;(f)urgentcare;(g)short-terminpatientstabilization,etc.Inconjunctionwithpreventivecare,acutecareservicescompleteahealthcaresystemparadigmthatfullyencompassesallessentialaspectsofthehealthcaredeliverysystem.Ifindoubtstudentsshouldconsultwiththeircoursefacultyregardingtheappropriatenessofaclinicalsite(Hirshonetal.,2013).

Nurse practitionerchairapprovesclinicalhours. Hours shouldbe related to DNPprojectconcentration.
NursePractitionerPreceptorCriteriaStudentsmaynotpreceptwithrelativesorclosefriendsaspreceptors.Preceptorsmusthaveacurrent,unencumberedstatelicenseandnationalcertification,asappropriate,inthepopulationfocusedorspecialtypracticearea.Studentsmaypreceptwithanursepractitioner,physician(e.g.MD,DO),orPhysicianAssistant(PA).Thepreceptormusthaveatleastoneyearofexperienceintheareaofadvancedpracticerelevanttothestudent’sclinicalfocus.Studentsmayspendnomorethantwosemesterswithanindividualpreceptorwithoutfacultyapproval.Studentsshouldhavenomorethantwopreceptorsduringasingleclinicalcoursewithoutfacultyapproval.Therequiredstudenttopreceptorratioshouldbe1:1ifpreceptorisseeinghis/herownpatientsand2:1ifpreceptorisnotseeinghis/herownpatients.Studentsareencouragedtopreceptwithnursepractitioners.Anursepractitionerpreceptorisrequiredforatleastonesemesterduringtheprogram.NursePractitionerStudentClinicalSiteandPreceptorApprovalProcess:
1. Reviewthisdocumentinitsentirety. 2. ReviewtheappropriateStudentHandbookforyourdegreeprograminitsentirety. 3. LearntonavigateProjectConcert(seeinstructionsbelow).StudentsinFNPorPMHNPwill
usetheETSUProjectConcertandstudentsinAGACNP,PNP-PC,andWHCNPwilluseTTUMedatraxforallclinicals.TheexceptionisMSN-DNPstudentswhoarepursuingadifferentconcentrationfromtheirMSNconcentrationorcurrentcertification.ThesestudentsmayhavetolearntheProjectConcertandMedatraxsystemsinordertomeetSACCOC1/3-rules(statingthatatleast1/3oftheircurriculummustbetakenateachinstitutioninajointprogram).Contactyourconcentrationadvisorforquestions.
4. Beginpreceptor/clinicalsitesearchasearlyaspossible.Youmay(andshouldifpossible)securecommitmentsforfuturesemestersbeyondtheonetotwosemestersdiscussedbelow.Namesofpreviouspreceptors/sitesareavailableintheclinicalmanagementsystem(i.e.ProjectConcertorMedatrax).Thesenamesprovideinformationforpotentialpreceptorsforstudentsandarenotexclusivetothedegreeprogram(MSN,DNP,etc.)orconcentration(FNP,AGACNP,etc.).

5. Onceapotentialpreceptorisidentified,thestudentsubmitsthePreceptorIntentformfoundinProjectConcertorMedatrax.(Checkwiththeconcentrationappropriateschool’sProjectConcert/Medatraxcontactforacopyofpreapprovedpreceptorintentforms).
6. StudentswillsubmitaPreceptorIntentFormforeachpreceptorhe/shehasforapracticumcourse,evenifthestudenthaspreviouslyworkedwiththepreceptor.
7. ThePreceptorIntentFormissubmittedatleastonetotwosemestersearlierthantheexpectedclinicalrotation.Tofacilitatetheprocess,thefollowingdeadlinesforsubmittingthePreceptorIntentFormareestablished:
a. FallSemester–May1 b. SpringSemester–October1 c. SummerSemester–February1 Note:Latesubmissionswillbeprocessed,butmaynotbefinalizedbythestartofthesemester.Thiscandelayastudent'sentryintotheclinicalsite.StudentsarenotpermittedtoentertheclinicalsiteorworkwithapreceptoruntilapprovedanduntiltheofficialETSUsemesterstartdate.
8. OncethePreceptorIntentFormiscompletedinProjectConcert/Medatrax,theClinicalPlacementCoordinatororrelatedpersonnelreviewsandassuresthepreceptorhasthefollowing: A.
Currentunencumberedstatelicense.B. Currentnationalcertification.C. CurrentPreceptorAgreementandClinicalAffiliationAgreementisinplace
(managedbytheETSUOfficeofStudentServicesandGraduateProgramsClinicalCoordinatororTTUclinicalcoordinator).
Note:Ifnoagreementisinplace,oneisinitiated.ThesestepsaredocumentedinProjectConcert/Medatrax.Dependingontheamountoftimerequiredforcontractprocessingandapproval,thestudent'sentryintotheclinicalsitemaybedelayed.
9. AfterallagreementsandthePreceptorIntentFormarefinalizedthe(Graduate)ClinicalPlacementCoordinator,andthecoursefacultywillreviewtheinformationinProjectConcert/Medatrax.
10. ClinicalplacementisconfirmedinProjectConcert/Medatraxunderthe"Student"tabwiththestatement“PreceptorCredentialsVerifiedandAgreementApproved”.
11. AfterconfirmationinProjectConcert/Medatrax,thestudentwillcompleteandsubmitaClinicalRotationPlan(instructionsbelow)tothecoursefacultyinD2L.
12. AfterreviewingtheClinicalRotationPlan,thecoursefacultywillnotifythestudenttobegintheclinicalexperience.
13. Bythefirstdayofclinical,thestudentwillprovideacopyoftheStudentClinicalProfile,StudentLearningOutcomes(coursesyllabus),andcurrentprogramplan.
Note:Ifthepreceptor/clinicalsiteisnotapproved,thestudentbeginsanadditionalsearch.Ifthestudentisunabletofindanappropriatepreceptor,thestudentisexpectedtocontactthecoursefaculty,OfficeofStudentServices,ConcentrationCoordinator,and/orProgramDirector/Coordinatortodetermineanappropriatecourseofaction.

PreceptorResponsibilities

• Demonstrateprofessionalrole-modeling. • Providethestudentwithaccesstopatientclinicalrecords,documentation,andelectronic
healthrecordssystemsifavailable. • Explaintothestudentyourorganization,prioritization,patientevaluation,exam,diagnostic

Studentsarelimitedtoobservationtothefirst1-2daysafterwhichthestudentshouldbeginseeingafewpatientsinadependentclinicalrole.Overthenextseveralclinicaldays/weeks,thestudentshouldbecomeincreasinglyindependent,butalwayswithpreceptoroversightofstudentpractice.
• Assistthestudentwiththeselection/inclusionofappropriateandincreasinglychallenginglearningexperiences.
• Beavailabletoconsultwiththestudentortoassumeresponsibilityforcareasneeded. • Encourageandexpectthestudenttobecomeincreasinglymoreresponsible,pro-active,and
self-reliantduringthesemester.• Reviewclinicalexperiencesdailyandapproveorreject.• Documentanylearningdeficienciesorclinicalperformanceissuesastheyoccur. • Provideconstructivefeedbacktothestudentviaformativeassessment.• ReviewtheevaluationcriteriaontheMidterm/FinalPreceptorEvaluationofNPStudent
(Examplepreceptorevaluationformforviewingpurposesonly). • Addressconflictsorconcernswiththestudentasearlyaspossible,clarifyingyour

identified(e.g.inadequatenumbersortypesofpatientstomeettheclinicalcourseobjectives).
• CompleteatleastonesitevisitpersemesterasdescribedaboveinFacultySiteVisits.Ifschedulingconflictsoranemergencyarises,ittheresponsibilityoftheclinicalfacultytomakearrangementswithanotherfacultymemberforcoverageandtonotifytheCourseCoordinatorandtheappropriateProgramDirectorofthechange.
• Evaluatestudentcompetenciesintheclinicalsitetoensureoptimalcareandadherencetoagencyexpectations,guidelines,andstandards.
• Notifystudent,CourseCoordinator,Advisor,andappropriateProgramDirectorasearlyaspossibleinthecaseofat-riskorunsatisfactoryclinicalperformance.
• Document,incollaborationwiththepreceptor,specificdeficienciesandestablishadefinedplanforremediationusingthePerformanceImprovementPlanContract.Meetwiththestudenttoreviewtheplan.Provideacopyofthecontracttothestudent,thepreceptor,theCourseCoordinator,andtheappropriateprogramdirectorforplacementinthestudent’sacademicfile.
• Facilitatefaculty/student/preceptorproblem-solvingwhennecessary,communicatingwiththeCourseCoordinatorand/orappropriateProgramDirectorasneeded.
