ordered to pay a civil penalty of $5,000 because he engaged in public accounting during a period in which

he did not hold a current license. (10/02/18)

Aussenberg Bernson & Perilman PC, license no. AF001280L, of Pittsburgh, Allegheny County, was

ordered to pay a civil penalty of $5,000 and costs of investigation of $427.16 because it held itself out as

being composed of certified public accountants while its license was lapsed. (10/02/18)

Berks County Douglas G Werley, certificate and license no. CA033475L, of Blandon, Berks County, voluntarily

surrendered his certificate and license to engage in the practice as a certified public accountant, because

he pled guilty, pled nolo contendere, or was found guilty of a felony under any Federal or State law or the

laws of any foreign jurisdiction and convicted of a felony offense under the Criminal History Record

Bucks County Judith Anne Baldasari, certificate and license no. CA017827L, of Doylestown, Bucks County, was

pay a civil penalty of $2,500 and costs of investigation of $504.57, because he engaged in the practice of

public accounting during a period in which he did not hold a current license, failed to timely complete a

peer review for the renewal period January 1, 2016, to December 31, 2017, that was a required condition

Lehigh County McPhillips CPA & Company LLC, license no. AF056593, of Coopersburg, Lehigh County, had a public

reprimand placed on its disciplinary record with the Board, paid a civil penalty of $2,000 and costs of

investigation in the amount of $246.68 because it operated as an accounting firm in the Commonwealth

examination, said license immediately placed on probation, based on applicant’s crimes of moral

Erie County Monica Abbate, of Erie, Erie County, was issued a $4,300 civil penalty, assessed a $375.25 cost of

investigation fee and ordered to cease and desist, based upon employing four unlicensed individuals to

Philadelphia County Ann G. Rosenblum, of Philadelphia, Philadelphia County, was denied licensure based on her conviction

of a crime of moral turpitude, her inability to practice with reasonable skill and safety, failure to disclose

pending criminal charges and questionable moral character. (07/25/18)

Wayne County Candace L. Frisbie, license no. MSG005468, of Honesdale, Wayne County, was placed on probation for

three years subject to the terms of the Board order based on her felony conviction. (06/25/18)

reprimand placed on his permanent disciplinary record with the Board and paid a civil penalty of $1500,

because he practiced as an orthotist/prosthetist from January 5, 2015, until June 20, 2016, while his

Carbon County Teresa Elizabeth Hubbard, license no. YM012986, of Jim Thorpe, Carbon County, had her license to

practice as a respiratory therapist indefinitely suspended for no less than three years retroactive to

Dana Joyce Morgan-Buck, license no. BH003352, of Palmerton, Carbon County, was indefinitely

suspended for no less than three years retroactive to April 17, 2018, because she violated terms and

Crawford County Brennan Phillip Wetherbee, license no. YM014921, of Cochranton, Crawford County, was indefinitely

suspended based on being unable to practice with reasonable skill and safety by reason of illness and

made misleading, deceptive, untrue or fraudulent representations in obtaining a license. (06/15/18)

Delaware County Edward Sannoh Forson, license no. BH001051, of Collingdale, Delaware County, was indefinitely

suspended and assessed a $1000 civil penalty based on receiving ARD in the disposition of felony

Jose Luis Boggio, license no. MD427979, of Drexel Hill, Delaware County, will be ordered to pay an

assessment fee in the amount of $3,000 towards costs of investigation; and he shall attend and

successfully complete at least ten (10) hours of remedial education dedicated on the topic of medical

record keeping and two (2) hours of remedial education related to the topic of the proper prescribing of

Benzodiazepines. (10/23/18)

Erie County Raymond W. McAllister, license no. MD066253L, of Erie, Erie County, was indefinitely suspended for a

minimum period of sixty (60) days and upon reinstatement, his license shall be indefinitely suspended for

no less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3)

years of probation under the Disciplinary Monitoring Unit, subject to the terms and conditions of the

Board; and he shall attend and successfully complete the “Professional Boundaries and Ethics Course”

during the period of active suspension, because he violated the law by engaging in sexual behavior with a

Franklin County Tunde Tolani Tijani, license no. MD056431L, of Greencastle, Franklin County, was assessed a $7,930.73

cost of investigation based on engaging in immoral conduct. (05/23/18)

Lackawanna County Steven Joseph Kravitz, license no. MA003033L, of Archbald, Lackawanna County, was indefinitely

suspended for no less than three (3) years, such suspension to be immediately stayed in favor of no less

than three (3) years of probation, unless that period of probation is extended or modified for cause by

mutual agreement of the Respondent and the Professional Health Monitoring Unit, Disciplinary

Monitoring Unit subject to the terms and conditions of the Board, because he is unable to practice the

profession with reasonable skill and safety to patients by reason of illness or addiction to drugs or alcohol.

penalty and was required to submit evidence of having completed all missing continuing education within

6 months based on Dr. Mehta’s failure to complete required education during the applicable biennial

reporting period and for making misleading, deceptive, untrue or fraudulent representations in obtaining

Snyder County Ahmed Kneifati, license no. MD037158L, of Selinsgrove, Snyder County, was placed on indefinite

Westmoreland County Nabil Jabbour, license no. MD028428E, of Greensburg, Westmoreland County, was temporarily

suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may

suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may

Susan B. Reamer, license no. YM002114L, of Irwin, Westmoreland County, was indefinitely suspended

retroactive to December 6, 2017, and assessed a $227.50 cost of investigation based on her failure to

comply with a Board Order to submit for an evaluation. (05/22/18)

York County William Scott Wolfe, license no. MD417360, of York, York County, paid a civil penalty of $1,000 and

costs of investigation of $1,114.60; was suspended for a period of one year; and shall attend and

successfully complete remedial education, because he engaged in unprofessional conduct, is guilty of

immoral or unprofessional conduct, and acted outside the scope of his license, in that he engaged in
