obligation, in-your-home consultation with one of our experienced Nurse Care Managers. The nurse will outline all of the options available to you and provide detailed recommendations based on your personal needs and preferences.

• Help understanding long-term care options — There are many care options for people who need assistance with daily living, and it can be overwhelming selecting the best one. Our Nurse Care Managers can provide you the information you need to make an informed decision and assist with any long-term care insurance claims.

• Medical and non-medical assistance in your home — Our experienced Caregivers, CNAs, LPNs and RNs will assist you to remain in the comfort of your own home. Our services include companionship, housekeeping, and meal preparation, transportation, bathing and personal care, mobility assistance, medication management and assistance, diabetic care, pediatric care and other nursing services.

Making decisions about long-term carePlan Ahead!It is common to put off thinking ahead to a time when we will need care and assistance. Most of us are inadequately prepared to face this often uncomfortable issue. What is right for one family is not appropriate for another, and what works one day can change dramatically overnight. Waiting until a time of crisis before beginning to plan, in addition to limiting your options, often leads to family discord and conflict and makes a difficult situation even worse. By getting information, making decisions and planning ahead we can most effectively manage our future.

Advanced Directive for HealthcareIt is never too early to plan. In fact, people of all ages can be assured that their healthcare wishes will be followed by simply completing an Advanced Directive for healthcare. In Oregon, the Advance Directive form is used to direct how healthcare decisions will be made for you and who will make them if you become incapable of making them yourself. Elder LawMaking decisions about financial concerns is best done when we are reasonably healthy. There are several options available for consideration when making these decisions and the advice of an attorney who specializes in estate planning, sometimes called Elder Law, often makes this process simpler. Consider Cost and the Ability to PayThose who offer long-term care services are generally private businesses that set their own prices and each has a unique pricing structure based on the service provided. This often causes confusion for the consumer.• Thecostoflong-termcarevariesgreatlydependinguponthetypeof

care that is needed, qualifications of those providing the care and where the services will take place.

• Paymentoflong-termcareservicesprimarilycomesfromaperson’sown private funds or, if one was purchased, a long-term care insurance policy. Another funding source that is becoming more popular is a reverse mortgage.

• Medicare generally doesn’t pay for long-term care or help with daily living needs such as bathing, dressing, grooming, shopping, etc.

• Medicaid pays for long-term care for people who have few financial resources, limited monthly income, and significant personal care needs.

• Allserviceprovidersoflong-termcarearerequiredtodisclosetheirfees and terms of their services in writing. Ask to see this document and any other document that will need to be signed before making a final decision. In order to fairly and accurately compare costs for services you must have the complete information about their fees and terms of services.

Social and Other Important Things to ConsiderIt is important to keep involved, to the greatest extent possible. Professional Guardians/Conservators/Case ManagersGuardians and conservators are appointed through the court to protect a person and serve as their advocate, particularly when there is some type of abuse or potential abuse involved. Guardianship occurs when a person becomes incapacitated, as defined by Oregon law, and has not made previous plans, or the plans that they have made do not work out. Generally, it is a last alternative after other solutions have failed. It can involve a court proceeding if the proposed incapacitated person requests a hearing. Guardians are appointed by a judge and the guardianship order will specifically outline the duties and responsibilities of the guardian.A conservatorship is also a legal proceeding that gives a person power over another person’s property and finances. Only a court can grant this power and it is generally done as a last resort. Some Elder Law Attorneys can also be appointed as guardians or conservators.If you are unsure what type of help is needed, and what your options are, a professional case manager can help. Care managers are highly trained social workers and nurses who have experience working with older adults and persons with disabilities. Care managers can be invaluable if you are trying to assist a loved one from far away. Some in-home care agencies provide nurse case managers to oversee their in-home care clients.

Resource and Service InformationAdvocacy, Information, and Education

Senior & Disability ServicesThe following is a list of groups and organizations that focus on advocating for seniors or who provide information, education and support groups focusing on issues commonly facing older adults as a result of their age, illness, or a disability.

Senior and Disability Services (SDS)Senior and Disability Services is the Area Agency on Aging (AAA) for Linn, Benton and Lincoln Counties; their mission is to provide leadership and services for seniors and people with disabilities to enhance their independence, dignity, choice and individual well being. SDS provides a wide variety of financial, medical and long term care services for clients and their families. SDS determines eligibility for medical and service programs to assist people with disabilities 18–64 years of age as well as older adults 65 years or older. If eligible, people may qualify for full or limited medical benefits, Lifeline, transportation services, in-home care services, subsidized payment for services in adult foster homes, assisted living and residential care communities. Other benefits coordinated by SDS include FoodStamps, Senior Meals and Home Delivered Meals. www.ocwcog.org

Aging and Disability Resource Connection (ADRC)The SDS Aging and Disability Resource Connection (ADRC) helps assess your need for services, provides information about all the options available, and can help you select the solution best for you. Home Care SectionApproximately 7 out of 10 deaths each year are a result of a chronic health condition. About 70% of people 65 and older will need long term care in some fashion. By 2050, the number of individuals using paid long-term care services in any setting (e.g. at home, residential care such as assisted living, or skilled nursing facilites) will likely be 27 million people, almost doubling since 2000. This estimate is influenced by growth in the population of older people in need of care. Many of these people will need assistance on a regular basis and they prefer to receive this help in their home or current place of residence. The following information is an overview of some of the home care services available in Linn and Benton counties.

In-Home Care Agency • Provideshomecareservicesforafeetoanindividualandislicensed

by the State of Oregon.• Servicescanincludeassistancewithtasksofdailylivingsuchashouse-

keeping, meal preparation, errands, transportation, bathing, grooming, hygiene, dressing, toileting, mobility, and medication management.

• Anyagencythatestablishesitselfasprovidinghomecareservicesorserves as a link for scheduling caregivers, assigning work, assigning compensation rates, and/or defining working conditions must be licensed in Oregon.

• Independentindividuals,volunteers,familymembers,neighbors,oran agency that only offers housekeeping need not be licensed and is not regulated.

• Caregiversmustbeatleast18yearsofageandbetrainedandqualifiedto perform the services which they are providing. A criminal background check is required.

• SomeIn-HomeCareAgenciesemployqualifiedCareManagementstaff to oversee the services offered by the agency. “Care Management” is a term used when a healthcare professional such as a nurse or social worker provides an individual with assistance developing and implementing a plan for care.

• Assistance managing and taking medications and other specialnursing tasks can be done if supervised by a Licensed Nurse.

What is Required of the Home Care Agency?• AgencymustbelicensedbytheStateofOregon.• Disclosure in writing regarding clients rights, what services they

offer, the qualification and training of their staff, their charges and a written plan for services.

• Amanagerisrequiredtobeavailableduringallhoursthatservicesare provided. Some companies have a live answering service; others use answering machines.

• AgenciesareclassifiedbytheStateofOregonaccordingtothelevelof medication and nursing services that they are able to provide. Companies classified as "limited" or "basic," may not provide medi-cation administration. Only companies classified as "comprehensive" may provide nursing services.

What About Costs? • Thecost for services ispaid forprivatelyby theperson inneedof

services, or may be covered by long-term care insurance policies, DHS(Medicaid),VA,etc..

• Private hourly rates range from about $25 to $50 per hour for acaregiver/Certified Nursing Assistant (CNA) depending upon the amount of time contracted for and the services required. Rates for licensed nurses are considerably higher.

• Insomecases,Medicaidmaybeabletopayforsomeoftheseservicesfor eligible individuals through the Client-Employed Provider Program (CEP), Oregon Project Independence (OPI) or other Medicaid sponsored programs. To be eligible, a person must have few financial resources, limited monthly income, and significant personal care needs. Home Health Services These agencies’ services are generally not considered long-term care; however they often work collaboratively with long-term care service providers in the care of a person who has both long-term care and skilled needs. Both Home Health and Hospice Services require a referral and orders by a physician. Assisted Living CommunitiesAn Assisted Living Community provides housing and supportive services in an apartment type setting. Each apartment has a kitchenette and private bathroom with a handicap accessible shower. Size can range from a studio to one or two bedrooms. Assisted Living Communities are licensed and regulated by the Department of Human Services, Seniors and People with Disabilities.

Communities vary greatly in their design and size. Nursing Facilities Nursing facilities are licensed by the Department of Human Services, Seniors and People with Disabilities and are required to meet both federal and state regulations. Nursing facilities are most appropriate for people who need 24-hour medical oversight and/or rehabilitative therapies. All of Linn and Benton Counties Nursing Facilitiesofferbothskilledandlong-term care services. • Skilledservices—Rehabilitativecarewhichisgenerallyshort-term

while recovering from an illness. Generally these services are covered under Medicare or HMO insurance programs.

End of Life PlanningEnd of life planning is a difficult process involving emotional stress, financial decision making, and may seem overwhelming. You can begin the planning process long before you are even close to death. It is important to record your desires and preferences regarding the decisions your loved ones will be asked to make if you become unable to make them yourself.Importantinformationtorecordandsharewithlovedonesincludes:1. Burial, funeral and after death preparation and wishes.2. Medical documents and information – including an Advanced

Directive for Health Care (see our chapter on Making Decisions), and wishes for organ donation.

number.5. Biographical information – date and place of birth, parents names,
