• Educación del paciente y del personal sanitario que interacciona con
él.
• El paciente debe ser evaluado antes de cada dosis de tratamiento.
• Laboratorio: TSH, LDH, bioquímica con perfil hepático
• Repaso de la sintomatología más frecuente.
• Manejo de efectos adversos en función de la gravedad de los mismos y
de los ALGORITMOS establecidos.
• Inicio precoz de del tratamiento de soporte en todos los eventos

¿Qué podemos aprender de los diferentes ensayos con los
COMBOs de inmunoterapia?
• La combinación IPI+NIVO presenta de manera estadísticamente significativa un
mayor beneficio en cuanto a OS y PFS respecto a IPI en pacientes afectos de
melanoma metastásico.
• En un análisis descriptivo:
• NIVO+IPI obtiene numéricamente un mayor beneficio en cuanto a OS, PFS
y ORR vs NIVO
• Dichos resultados son superiores para IPI+NIVO incluso en aquellos
subgrupos de pacientes más relevantes: BRAF mutados, LDH elevada
PD-L1 bajo expresores (<5% o <1%):
• Aunque la curvas de OS son parecidas para la población alta expresora
de PD-L1 (>5% o >1%), IPI+NIVO presentó una mayor tasa de ORR vs
NIVO independientemente del nivel de expresión de PD-L1
• La mediana de duración de respuesta resulta ser mayor para IPI+NIVO
• El tiempo medio a segunda línea de tratamiento no se ha alcanzado para
IPI+NIVO
• El perfil de toxicidad de IPI+NIVO es consistente en todos los estudios, con un
58% de eventos grado3-4, una tasa de discontinuación del 40% pero un 70% de
los pacientes que discontinúan el tratamiento se siguen beneficiando del
mismo.
• El 85-100% de los efectos adversos se resuelven en 3-4 semanas con
tratamiento médico.

Informe TC toraco-abd: Masa voluminosa TCS en región de hipogastrio, de 92 mm diámetro, adenopatías inguinales bilaterales de tamaño patológico (17mm y 15 mm), adenopatías en cadena Ilíaca externa derecha de aspecto patológico
• Confirmación con PET-TC y biopsia lesión subcutánea -> AP: M1 melanoma
• RMN SNC: negativa para M1
• AG: sin alteraciones, LDH 247.
Recidiva subcutánea y ganglionar irresecable
¿Cuál seria la opción mas adecuada para el tratamiento de 1ª línea
de este paciente teniendo en cuenta que … ?
• Determinación BRAF V600E: mutado
• Estado general: ECOG 1
• Edad: joven sin comorbilidades
• Carga tumoral: baja
• M1 SNC: ausencia
• Nivel LDH: normal
• Síntomas: dolor a nivel lesión subcutánea que se controla
con opiáceos
OPCIONES:
1. TERAPIA DIRIGIDA con iBRAF + iMEK puesto que es la única opción de tratamiento válida y efectiva para este paciente.
2. IPILIMUMAB como primera línea de tratamiento con inmunoterapia.
3. INMUNOTERAPIA basada en anti-PD1 + ipilimumab al ser una opción de tratamiento válida para pacientes BRAF mutados en 1ª línea al demostrar una PFS similar a la que ofrece la terapia dirigida
4. QUIMIOTERAPIA ENDOVENOSA para obtener una rápida respuesta.

• Evaluación a las 6 semanas: respuesta clínica cutánea.
• Evaluación TC tóraco-abdominal a las 12 semanas: respuesta parcial (RP)
• Evaluación TC tóraco-abdominal Diciembre 2014: respuesta completa (RC)
TC Tóraco-Abdominal (Abril 2014): 12 semanas de tratamiento
Informe TC toraco-abd: Práctica resolución de la masa en TCS de hipogastrio, de 67x 8 mm (92 mm), Adenopatías inguinales bilaterales patológicas, en región inguinal derecha de 11 y 10 mm (17 y 15 mm) (ΣDMs 29%)
Marzo 2014 (CICLO 2 SEMANA 2):
- Cefalea con síntomas de alarma.
- Amaurosis ojo derecho.
Se realizó como estudio inicial pensando en:
a.M1 SNC
b.AIT/AVC
• TC SNC con contraste: normal
• Eco doppler troncos supra-aórticos: normal
COMPLICACIONES:
• RESONANCIA MAGNÉTICA (RNM) 10/3/14 : En las secuencias centradas en hipófisis se aprecia un leve aumento global de su tamaño con margen superior abombado y con engrosamiento del tallo hipofisario, tras la administración endovenosa de contraste muestra realce, con área hipocaptante a nivel antero lateral izquierda, cranealmente ocupa parcialmente la cisterna suprasellar pero no contacta con el quiasma óptico, dichos hallazgos son compatibles con una hipofisitis.
• Perfil hormonal previo al inicio de corticoterapia 10/3/2014: T3 1ng/mL, TSH 0,3mcUI/mL, T4L 0,96 ng/dL, LH 3,5UI/L, FSH 6.6, Prolactina 4,8ng/mL, Testosterona 100ng/dL, Cortisol 26,8pg/mL, ACTH 41,7pg/mL, IGF1 2.97
AUMENTO DE GONADOTROPINAS Y CORTISOL
¿Cuál seria la opción mas adecuada para el manejo de la
toxicidad que presenta este paciente?
OPCIONES:
1. Actitud expectante pues las toxicidades inmunomediadas se resuelven siempre espontáneamente.
2. Parar el tratamiento que recibe el paciente durante unos meses a ver si mejora y remitir a endocrinología para estudio.
3. Iniciar tratamiento hormonal sustitutivo sin precisar la toma corticoides.
4. Iniciar tratamiento con corticoides a dosis altas, tratamiento hormonal sustitutivo, IC a endocrinología para seguimiento conjunto y parar la medicación del EC hasta mejoría de la sintomatología y de los valores analíticos.
- Se suspendió el tratamiento hasta RNM de control que mostraba resolución de hipofisitis y hasta la disminución del tratamiento con dexametasona hasta 1 mg al día.
- El paciente tras el inicio del tratamiento corticoideo presentó mejoría progresiva hasta la resolución de la sintomatología.
- Actualmente continúa asintomático y en tratamiento de mantenimiento con nivolumab 3mg/kg cada 2 semanas.
Diagnóstico: hipofisitis secundaria a tratamiento inmunológico. Tratamiento: dexamentasona 4 mg cada 8h, en pauta descendente.
TC Tóraco-Abdominal (Diciembre 2014): 11 meses de tratamiento (RC Radiológica) confirmada PET/TAC
TC Tóraco-Abdominal (Marzo 2017): 38 meses de tratamiento (mantiene RCr)
Informe TC toraco-abd: Masa TCS de 67x8 mm residual, adenopatías inguinal de 7mmy 8 mm (ΣDMs33%)
Informe: Masa TCS residual de 54x6 mm, adenopatías inguinales de 6 y 8 mm (ΣDMs 44%)
