Location: Biosensor Laboratory, 2nd Floor Engineering Building

Detail of report:On Tuesday (26/05/2015) morning, Dr. Lee Su Yin form Faculty of Applied Sciences

discovered water accumulation on the floor of the mentioned laboratory. She informed

the cleaners and also the maintenance department on possible pipe leakage. No

equipment’s were damaged.

Cause:

Water was probably leaked out form the Ultra-Pure Water system.

Corrective action:

1. All the sinks were checked whether clogging occurred with the help of maintenance

department.

2. All the pipe route in the laboratory was thoroughly checked by the maintenance

department.

3. The Ultra-Pure Water system water outlet tubing has been tightened around the sink to

ensure water flows only to the sink area.

Preventive action:

1. Daily checking on the laboratory will be done and recorded in a log book.

2. The Ultra-Pure Water system will be shifted to a laboratory in the Medical Building to

ensure better monitoring of the equipment in the near future.
