What is a program• BLI has two types of programs. Habilitation Programs which are designed to increase

an individual’s personal growth by:• developing the individual’s vocational and/or daily living skills• enhancing social and communication abilities• teaching positive alternatives to undesirable behaviors• and sometimes providing goal-oriented activities• Programs for each client are chosen by the client’s team members at the client’s IPP

(Individual Program Plan) meetings. Most IPP teams rely on the client’s team lead for program ideas.

• The program developer uses the notes from the IPP and information from the client’s team lead to write the program

• Programs are normally designed to last one year• As of October 1st, 2015 all BLI Habilitation programs are on Therap.• BLI also has a Staff Objective/Service Needs (SOSNs) program and a Behavior Baseline

program. These are NOT designed to teach (or Habilitate) a client rather to gather information connected with the client.

Why do we have programs

• Habilitation Programs provide the state of Nebraska proof that all DHHS clients are learning to live as independently as possible.

• Providers, like Better Living, Inc., are paid, in part, to habilitate clients.

• Information gathering programs (like the SOSN and the Behavior Baseline programs) provide an efficient method of obtaining data that is required by the state. BLI is paid, in part, for the staff objective and service needs that DSPs perform (or complete) for the clients.

When do we run a programThis is especially important and will be on the test

• Each program in Therap has a subheading that reads: Schedule and Frequency. Under that heading it will state when the program is to be run.

Daily Programs: Most programs are run daily. Clients will have one or two daily programs. These programs must be run and documented at least once each day. Residential Day Programs: Continuous clients (ones that require overnight staff or who live

in a house that has overnight staff) will have a Residential Day program. These programs are run on the weekends and on days when the client is home instead of at work - like a snow day or a holiday. This is because BLI needs to show that not only was the client home during working (day) hours but that habilitation took place. RESIDENTIAL DAY PROGRAMS HAVE THIS SCHEDULE AND FREQUENCY:

Weekly programs:A few programs are run and documented once a week. These have Weekly in the title. It will normally say what day the program is run and documented. Examples of a weekly program are: Budgeting and Cooking

and sometimes an Activity will be a weekly program.

• A few programs are designed to have only one task element run each day. With these programs you will need to keep track of each day’s task element score and then document once a week. These are DAILY/RECORDED WEEKLY programs. An example of this type of program would be an activity program where on Monday the client chooses an activity, Tues. he schedules it/marks the time and day on the calendar, Wed. he discusses transportation and money needed, Thurs. he takes money from his allowance for the event, Fri. he chooses what he will wear, makes sure his outfit is clean and in good repair, Sat. he reviews his money, with whom he is going and his transportation and

Sunday he attends the event.

When do we run a program This is especially important and will be on the test

• Therapeutic programs:• Some programs are only run when the client is in the

residence during day service hours because s/he is ill, injured or when the client is being transported to a medical or therapeutic appointment. These are called Therapeutic programs – DO NOT run them daily – only when the client is home sick or being taken to a medical appointment. If in doubt call the support line.

When do we run a program This is especially important and will be on the test

• Review: Currently BLI Habilitation programs are:• Daily – all residential and/or DSC clients have one.

This program is run every single day that the client is at the facility for at least 4 hours between 6:00 AM and midnight.• Residential Day – continuous residential clients have one.

This program is run on Saturday and Sunday and on snow days and holidays when the client is in the facility for at least 4 hours between 6:00 AM and midnight. It is run along with the Daily programs.• Weekly – some residential clients have one.

This program has all the task elements run and documented one specified day of the week. If the client is out of the facility on that day the program should be run and documented the next day• Therapeutic – all residential clients have one

This program is run when the residential client is home sick (or injured) and/or when the client is being taken on a med run during the time s/he would normally be at work.

This program is NOT run daily – only when the client is ill or injured.• Daily/Recorded Weekly – a few residential clients have one.

This program has one task element run each day and all tasks recorded one day a week.

When do we run a program This is especially important and will be on the test

• Following are examples of which program(s) to run in varying situations. Not all situations can be covered so if you are not sure which program to run call the support line. The key to knowing which programs to run is in the amount of time the client is in the residence. Four hours is the magic number. In order for BLI to get paid for habilitation a program needs to run if the client is in the facility for a minimum of 4 hours. In order for BLI to get paid for “day” hours on Saturday, Sunday, snow and vacation days a client needs to be in the facility for 8 hours.

• Situation: The client is home on Saturday until 2:00 PM when his parents pick him up. He does not return to the residence until 8:00 PM Sunday.

• On Sat. he is home 8 hours. So Sat. morning run his daily programs and, if he has one, run his Residential Day program. Sunday evening he is home 4 hours so run his daily programs. If the program(s) has task elements that are exclusive a time of day when the client was not home mark them as an N/A.

Where do we run programs

• The first subheading on the Therap program is: Provider Program. This will give you the residence address where the client lives or it will say (DSC) Day Service Center.

• You cannot run a Day program at a residence nor can you run a residential program at the DSC.

• A Therapeutic program can be run at the residence and/or a hospital, clinic, or during transportation to or from a medical or therapeutic appointment.

How to run an Habilitation program• Step 9: If this is your first time running the program you will have to

acknowledge it before you can run it. This page gives you all the information about the program:

• when to run it: Schedule and Frequency• When to document: Frequency of Documentation• It lists the STOs (short term objectives) and a space to mark when each one is passed: Criteria for

Completion• It lists the scoring method: Scoring Details• It lists the task elements: Tasks• It explains the reason for the program and how it relates to the individual: Teaching Method• Make sure you read the program carefully before you acknowledge it. It is your responsibility to know all

the information about the program

How to run an Habilitation program• Step 10: Run each task element. • Most task elements are written in the first person from the client’s

perspective. NOTE: The teaching level will be at the end of each task element. In the example below the teaching level is “with unlimited verbal instruction”. You will have to use your own judgment on how to score the task. Keep in mind the level of teaching and include the client’s effort or willingness as well as their skill level. For example - if the client is not very verbal and reluctant to speak and answers the question below that they ate “an apple and no candy”. You may decide that this is worthy of “Complete”. But, if the client is very skilled verbally and answers: “Apple” you may feel that this should be: “Declined”

How to run an Habilitation program

• Step 10b: Run each task element – continued.• Most programs do not specify a venue. However, an example of one that

does would be a cooking program – that would need to be run in the kitchen. Most programs are written so they can be run in a normal social situation. An example would be having a conversation with the client when s/he comes home from work or in the evening after dinner. In many communication type programs the client may not be aware that the conversation you’re having with them is actually a program. The more “natural” the setting and the more unobtrusive you are in running the program the better. Many programs, like task programs, require that the client complete a specific task or chore. This can be easier to score because they either did it with the current level of teaching or they didn’t do it or didn’t do it completely.

Staff Objective/Service Needs programs

• SOSNs need to be documented DAILY by EACH STAFF member for each client in the residence or at the DSC. Examples of SOSN “task” elements are:

FINANCE Assist with money management. BLI is payee. AS NEEDED

MEDICAL Schedule, transport, document and assist with medical appointments. AS NEEDED

MEDICAL SEIZURE PROTOCOL: If seizure occurs, call 911 immediately. AS NEEDEDSOCIAL Provide transportation to and from activities. AS NEEDEDDIET Remind to cut food into dice size pieces to prevent choking. EACH MEAL

How to score Staff Objective/Service Needs programs

• The reason each staff member must document the SOSNs for each client, each shift, is because it is often a matter of client safety that everyone working with that client knows the staff objectives and service needs. Documenting daily should insure every staff member working with a client is aware of that client’s needs.

• SCORING• This is very straight forward. If you, the staff, did cut food into bite

size pieces you would choose: “Service Provided”• If you were not there at meal time or the client was not there at

meal time you would choose: N/A (not applicable)• If •

How to record data on Therap

• Step 1: After you’ve hit Acknowledge (and hit the OK button in the pop up box) you will be taken to the Select Date for ISP Data Collection page. It will show today’s date but you can go back to any date from today to when the program started. Chose the date you want and Click on Submit.

How to record data on Therap• Step 2: This is the page that lists the task elements and allows you to score

each task. Click on the arrows on the right side of the Select Score boxes. Click on the score you

want recorded.

How to record data on Therap• Step 3: Therap will not let you leave a task un-scored. If there is a task

element that cannot be scored correctly using “declined” or “completed” choose N/A (not applicable/reportable) Not all programs have

an N/A but they soon will.The new scoring system is in the process of being implemented. You will start to see three choices for a score: N/A (not reportable), Declined, and Completed. If N/A is not available you can leave it as a comment in the box below the scores.

• This example says: “Christopher was not upset or mad today so Task Element # 4 should be an N/A.”

How to record data on Therap• Step 4: Once you’ve selected a score for each task element (and written a

comment if you feel one is necessary) go to the bottom right hand corner and click on Save. You have now successfully recorded data for this program.

Step 1: Go to the Individual page on Therap and to the subheading ISP Data then click on Search.

Step 3: Click the date that the correction(s) need to be made. Therap will only let you go back 6 weeks.

bottom right hand corner click Next.

the Programmatic report highlighted here and the Data Collection Monthly shown on the

following pages.• You will need to put in a Start Date and and End Date. And, choose if you want to look at a Day, Week, Month, Quarter, Year or Other. Then click on Next.

How to view different reports• Step 6: You’re now on a page that shows you what percentage of “Yes” or

“Complete” has been achieved. The number in the lower right hand corner is the percentage that the client has achieved for the selected program and for the selected time frame. This person has achieved 69.23 percent. We don’t use the numerals to the right of the decimal point – so they are at 69%.

How to view different reports• Step 7: The Data Collection Monthly report highlighted here

tells you which days data has been recorded. It will automatically be on the current month. Click on Next.

recorded data today. It also shows who recorded the data for each task element each day of the month.
