GENERAL INSTRUCTIONS
This job evaluation review shall be completed for all company employees irrespective of work categories and levels in every Project / Department / Unit of the organization. The immediate supervisor who is tasked to do the evaluation has to exercise full impartially by appropriately addressing objectivity in giving the scores and remarks. The Performance Appraisal is composed of 10 Core Competencies which normally cover all aspects of the employee’s area of evaluation. Each core competency is given a score as indicated from 50 to 100. The evaluator is required to read and fully understand the Core Competencies as explained and give a score as appropriate from 0 to 100 for each category.

SCORE RATING
The Score is the average point taken dividing the total number of points from the given scores by the number of applicable core competencies and equate the score with the corresponding performance rating.
