The enclosed Permit No. 1978-A is your authority to construct, operate, and maintain the equipmentand/or control apparatus as set forth in your application dated or last amended January 17, 2001.

After considering the facts and requirements of AC.A §8-4-10 1et seq., and implementing regulations,I have determined that Permit No. 1978-A for the construction, operation and maintenance of an air

pollution control system for Rogers Animal Shelter be issued and effective on the date specified in thepermi t, unless a Commission review has been proper! y requested under §2.1.14 of Regulation No.8,Arkansas Department of Pollution Control & Ecology Commission's Administrative Procedures, withinthirty (30) days after service of this decision.

All persons submitting written comments during this thirty (30) day period, and all other persons entitledto do so, may request an adjudicatory hearing and Commission review on whether the decision of the

Director should be reversed or modified. Such a request shall be in the form and manner required by§2.1.14 of Regulation No.8.

SECTION II: INTRODUCTION

Summary

The City of Rogers operates the Rogers Animal Shelter at 2935 W. Oak in Rogers, Benton County. This

will be the initial permit for an incinerator to dispose of animal remains.

This permit will allow the installation and operation of a minor source incinerator for the disposal of Type

4 Pathological Waste (see Appendix A for waste classifications), This permit is not intended forMedical/Infectious Waste or Types 5 and 6 Non-solid and Solid Wastes. A waste incinerator is definedas a furnace or other apparatus for burning waste products.

Process Description

The Incinerator is a heavy duty high temperature controlled air design (Shenandoah Model P25-Ml) with

a primary chamber and a secondary chamber. Combustion is completed in the secondary reactionchamber. All of the air necessary for complete combustion of the waste is supplied through a controlledair system, with overfire and underfire air nozzles in the primary chamber and high''Velbcitynozzles in thesecondary chamber. Natural gas will be supplied to both the primary and secondary chambers at a ratehigh enough to bring the secondary combustion chamber up to a minimum of 1800 oF.

There are two gas burners for the primary and secondary chambers. The initial burner is rated at 335,000

Btulhr and the secondary burner is rated at 1,200,000 Btu/hr. Additional combustion energy comes fromthe Type 4 Waste, which is estimated at 1,000 Btu/lb. The burners have a blower, a motor with overloadprotection, and an automatic ignition system,

Waste is weighed to meet the lbs/hour capacity of the specific Waste Type, as recommended by themanufacturer. This incinerator is designed to handle up to 1,200 pounds of Type 4 Waste in a 10 hour day,based on batch loading twice daily.

Regulations

All minor source incinerators are subject to the Arkansas Air Pollution Control Code (Regulation 18).

4. Pursuantto §18.901 of Regulation 18, andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­4-311, the permittee shall not conduct operations in such a manner as to unnecessarily cause aircontaminants and other pollutants to become airborne.

5. Pursuant to §18.1004ofRegulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­4- 311, the permittee shall not process more than 219 tons of total waste material at the facility perconsecutive 12 month period.

12. Pursuant to §18.1004 of Regulation 18, and A.C.A. §8-4-203 as referenced by §8-4-304 and

§8-4-311, the permittee shall not feed waste into the incinerator at a higher rate (pounds/hour)than that recommended by the manufacturer for each type of waste. For this incinerator, themanufacturer recommends a maximum charge of 1,200 lbs per 10 hour day, based on batch

loading. The type of waste must be clearly identified in the plant records. (See Appendix A)

13. Pursuant to §18.1004 of Regulation 18, and A.C.A. §8-4-203 as referenced by §8-4-304 and§8-4-311, the facility shall record the time and date of each charge burned. Computer records are

acceptable. A record of the secondary chamber temperature each time the primary chambercharge is ignited, and also approximately half way through the cycle, shall also be kept. The recordsheets shall be kept on site, updated daily, and provided to Department personnel upon request.If the facility has more than one incinerator, individual incinerator records shall be kept on aseparate sheet.

14. Pursuantto §18.1004 of Regulation 18, and AC.A. §8-4-203 as referenced by §8-4-304 and§8-4- 311, the allowable particulate emission rate for the incinerators shall not exceed 0.10 grains

per dry standard.cubic foot, corrected to 12% CO2,

15. Pursuant to §18.1004 of Regulation 18, and A.C.A §8-4-203 as referenced by §8-4-304 and§8-4- 311, a copy of this permit and standard operating procedures for the incinerators shall beposted at or near each unit.

16. Pursuant to §18.1002 of Regulation 18 and A.C.A §8-4-203 as referenced by §8-4-304 and§8-4-311, within 180 days ofthe issuance ofthis permit, the permittee shall testthe incinerator forPM using USEP A Method 5. The permittee shall test in accordance with the provisions set forthin General Condition #7 of this permit. Certification of testing from an identical unit was submittedwith this application and meets this requirement.

6. Pursuant to §18.1004 of Regulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­

4-311, the permittee shall maintain monthly records which demonstrate compliance with SpecificCondition # 5. Records shall be updated by the fifteenth day of the month following the month to

which the records pertain. These records shall be kept on site, and shall be made available toDepartment personnel upon request.

7. Pursuant to §18.1004ofRegulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­4-311, the permittee shall keep a separate log listing the source and date of incineration of anywaste burned that is not from the Rogers Animal Shelter.

8.. Pursuant to §18.1004 of Regulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8-4-311, only animal remains, animal waste, and associated bedding, containers, etc., may be

incinerated at this facility.

9. Pursuant to §18.1004 of Regulation 18 and 40 CFR Part 52, Subpart E, the incinerator(s) shallnot burn any hospital waste, medical/infectious waste, or contaminated animal carcasses, body

parts, and bedding of animals that were exposed to infectious agents during research (including. research in veterinary hospitals), production of biologicals, or testing of pharmaceuticals.

9. Pursuant to §18.1004 of Regulation 18 and A.C.A. §8-4-203 as referenced by §8-4-304 and §8­4-311, the incinerator( s) shall be operated only in accordance with their operating instructions. Acopy of these instructions shall be posted at or near each incinerator. The permittee shall not ignite

the incinerator charge unless the secondary chamber has been correctly preheated to the requiredexit gas temperature. Por batch charges, the permittee shall not open the charge door during theincineration cycle.

10. Pursuant to §18.1004 of Regulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­4-311, the temperature at the exit of the afterburner shall be monitored throughout the incinerator

cycle. This temperature shall be maintained at or above 1,800oP when waste is being burned.

11. Pursuant to §18.1004 of Regulation 18 andA.C.A. §8-4-203 as referenced by §8-4-304 and §8­

4-311, the facility shalloperate any incinerator with a functioning continuous temperature monitoron the secondary chamber exit, or record exit gas temperatures every hour.

SECTION V: INSIGNIFICANT ACTIVITIES

There are no activities deemed insignificant on the basis of size, emission rate, production rate, or acti vity

in accordance with Group A of the Insignificant Acti vities list found in Regulation 18 and 19 Appendix A.

Insignificant activity emission determinations rely upon the information submitted by the permittee in the

application.

SECTION VI: GENERAL CONDITIONS

1. Any tenns or conditions included in this permit which specify and reference Arkansas Pollution

Control &Ecology Comrrussion Regulation 18 or the Arkansas Water and Air Pollution Control

Act (A. C.A. §8-4-10l et seq.) as the sole origin of and authority for the tenns or conditions are

not required underthe Clean Air Act or any of its applicable requirements, and are not federally

enforceable under the Clean Air Act. Arkansas Pollution Control & Ecology Commission

Regulation 18 was adopted pursuant to the Arkansas Water and Air Pollution Control Act (A.C.A.

§8-4-101 et seq.). Any terms or conditions included in this permit which specify and reference

Arkansas Pollution Control &Ecology Commission Regulation 18 or the Arkansas Water and Air

Pollution Control Act (A.C.A. §8-4-101 et seq.) as the origin of and authority for the tenns orconditions are enforceable under this Arkansas statute.

2. Pursuant to A.c.A. §8-4-203 as referenced by A.C.A. §8-4-304 and §8-4-311, this permit shall

not relieve the owner or operator of the equipment and/or the facility from compliance with all

applicable provisions of the Arkansas Water and Air Pollution Control Act and the regulations

promulgated thereunder.

3. Pursuant to §19.704 of the Regulations of the Arkansas Plan of Implementation for Air Pollution

Control (Regulation 19) and/or A.C.A. §8-4-203 as referenced by A.C.A. §8-4-304 and §8-4­311, the Department shall be notified in writing within thirty (30) days after construction has

commenced, construction is complete, the equipment and/or facility is fIrst placed in operation, and .

the equipment and/or facility first reaches the target production rate.

4. Pursuant to §19.41O(B)of Regulation 19 and/or §18.309(B)of the Arkansas Air Pollution ControlCode (Regulation 18) and A.C.A. §8-A-203 as referenced by A.C.A. §8-4-304 and §8-4-311,

construction or modifIcation must commence within eighteen (18) months from the date of permitIssuance.

5. Pursuant to §19.705 of Regulation 19 and/or §18.1004 of Regulation 18andA.C.A. §8-4-203as referenced by A.C.A. §8-4-304 and §8-4-311, records must be kept for fIve years which will

enable the Department to determine compliance with the terms of this permit--such as hours of

operation, throughput, upset conditions, and continuous monitoring data. The records may be

used, at the discretion of the Department, to determine compliance with the conditions of the

permi t.

6. Pursuantto §19.705 of Regulation 19 and/or §18.1004 of Regulation 18 andA.C.A. §8-4-203

as referenced by A.C.A. §8-4-304 and §8-4-311, any reports required by any conditioncontained in this permit shall be certifiedby a responsible official and submitted to the Departmentat the address below.

7. Pursuantto §19.7020fRegulation 19 and/or§18.1002 of Regulation 18 andA.C.A. §8-4-203as referenced by A.C.A. §8-4-304and §8-4-311, any equipment that is to be tested, unless stated

in the Specific Conditions of this permit or by any federally regulated requirements, shall be testedwith the following time frames: (1)Equipment to be constructed or modified shall be tested withinsixty (60)days of achieving the maximum production rate, but in no event later than 180 days after

initial start-up of the permitted source or (2) equipment already operating shall be tested accordingto the time frames set forth by the Department. The permittee shall notify the Department of thescheduled date of compliance testing at least fifteen (15) days in advance of such test. Compliance

test results shall be submitted to the Department within thirty (30) days after the completed testing.

8. Pursuantto §19.702 of Regulation 19 and/or §18.1002 of Regulation 18 andA.C.A. §8-4-203as referenced by A.C.A. §8-4-304 and §8-4-311, the permittee shall provide:

a. Sampling ports adequate for applicable test methodsb. Safe sampling platformsc. Safe access to sampling platformsd. Utilities for sampling and testing equipment

as referenced by A.c.A. §8-4-304 and §8-4-311, the equipment, control apparatus and emissionmonitoring equipment shall be operated within their design limitations and maintained in goodcondition at all times.

10. Pursuantto §19.601 of Regulation 19 and/or§18.1101 of Regulation 18 andA.C.A. §8-4-203as referenced by A.C.A. §8-4-304 and §8-4-311, if the permittee exceeds an emission limit

established by this permit, they shall be deemed in violation of said permit and shall be subject toenforcement action. The Department may forego enforcement action for emissions exceeding anylimits established by this permit provided the following requirements are met:

a. The permittee demonstrates to the satisfaction of the Department that the emissions

resulted from an equipment malfunction or upset and are not the result of negligence orimproper maintenance, and that all reasonable measures have been taken to immediatelyminimize or eliminate the excess emissions.

b. The permittee reports the occurrence or upset or breakdown of equipment (by telephone,facsimile, or overnight delivery) to the Department by the end of the next business day afterthe occurrence or the discovery of the occurrence.

c. The permittee shall submit to the Department, within five business days after theoccurrence or the discovery of the occurrence, a full, written report of such occurrence,including a statement of all known causes and of the scheduling and nature of the actions

to be taken to minimize or eliminate future occurrences, including, but not limited to, actionto reduce the frequency of occurrence of such conditions, to minimize the amount by whichsaid limits are exceeded, and to reduce the length of time for which said limits areexceeded. If the information is included in the initial report, it need not be submitted again.

11. Pursuant to A.C.A. §8-4-203 as referenced by A.C.A. §8-4-304 and §8-4-311, the permitteeshall allow representatives of the Department upon the presentation of credentials:

a. To enter upon the permittee's premises, or other premises under the control of thepermittee, where an air pollutant source is located or in which any records are required tobe kept under the terms and conditions of this permit

b. To have access to and copy any records required to be kept under the terms andconditions of this permit, or the Act

c. To inspect any monitoring equipment or monitoring method required in this permitd. To sample any emission of pollutants

e. To perform an operation and maintenance inspection of the permitted source

12. Pursuant to AC.A. §8-4-203 as referenced by A.C.A. §8-4-304 and §8-4-311, this pennit isissued in reliance upon the statements and presentations made in the pennit application. The

Department has no responsibility for the adequacy or proper functioning of the equipment orcontrol apparatus.

13. Pursuant to §19.41O(A) of Regulation 19 and/or §18.309(A) of Regulation 18 and AC.A

§8-4-203 as referenced by A.C.A §8-4-304 and §8-4-311, this pennit shall be subject torevocation or modification when, in the judgment of the Department, such revocation ormodification shall become necessary to comply with the applicable provisions of the ArkansasWater and Air Pollution Control Act and the regulations promulgated thereunder.

14. Pursuant to §19.407(B) of Regulation 19 and/or §18.307(B) of Regulation 18 and A.C.A.§8-4-203 as referenced by AC.A §8-4-304 and §8-4-311, this pennit may be transferred. Anapplicant for a transfer shall submit a written request for transfer of the permit on a form providedby the Department and submit the disclosure statement required by Arkansas Code Annotated §8­1-106 at least thirty (30) days in advance ofthe proposed transfer date. The pennit will beautomatically transferred to the new permittee unless the Department denies the request to transferwithin thirty (30) days of the receipt of the disclosure statement. A transfer may be denied on thebasis of the information revealed in the disclosure statement or other investigation or, if there isdeliberate falsification or omission of relevant information.

15. Pursuant to AC.A. §8-4-203 as referenced by AC.A. §8-4-304 and §8-4-311, this pennit shallbe available for inspection on the premises where the control apparatus is located.

16. Pursuant to AC.A. §8-4-203 as referenced by §8-4-304 and §8-4-311, this pennit authorizesonly those. pollutant emitting activities addressed herein.

17. Pursuant to Regulation 18 and 19 and A.C.A. §8-4-203 as referenced by §8-4-304 and §8-4­311, this pennit supersedes and voids all previously issued air permits for this facility.

INSTALLA TION

Must be installed in accordance with local codes and ordinances, subject to regulatory agencies. Stack test data isavailable through the distributor [or permitting application. If on-site testing is required, it ~sthe responsibility of thepurchaser and can be arranged through the distributor.

Units may be installed outside or inside. A shelter is recommended for outside installations. Inside instal1ationsrequire positive pressures to insure adequate combustion air and may be subject to special insurance requirements. Aminimum of 4' clearance [rom any combustible materials must be maintained on all installations with any exceptionsbeing approved by the factory. Factory must be advised whether the installation is outside or inside.

Caution:

Unauthorized persons should not be in the area of the incinerator when charging is inprocess.

CONTROL SETIINGS:

The modulating linkage between the actuator, blower damper and gas valve is preset atthe factory and should need no adjustment.

The upper temperature control set point is to be set at 1800 degrees after curing is completed.

The lower temperature control indicates primary chamber temperature and cycles the lower burnerduring curing. When no cycling is required the control should be set at 2400 degrees.

It may be necessary to change the timer settings depending on waste type, experience andstate regulations.

Your timer may be operated in any of the three timing ranges available on that particular timer.The timing range is changed by inserting a screw driver into the adjustment screw and rotating

it in either direction to desired timing range. There will be a slight click when the adjustmentscrew is in proper position. (See crouzet timer instruction sheet). '

THERE ARE THREE OPERATING MODES OF THE P25 INCINERATOR:(1) PREHEAT; (2) INCINERATE; AND (3) BURN-DOWN.

(1) Preheat Cycle

The P25-2GMl must be preheated 30 minutes before waste is loaded into the primary chamber.This is to prevent the waste from igniting before 1800 degrees F. can be achieved in the secondarychamber. In order to preheat to 2000 degrees F. allow 60 minutes.

Before preheating the incinerator be sure the "Power On" switch is in the ON Position.

To start the preheat cycle turn on the "preheat" momentary switch. This ignites the upper burnerand blower, which begins to preheat the secondary chamber. The fill door and ash door must be close(in order to achieve 1800 degrees during preheat.

Ash must be removed from the P25-2GM1 before starting the first burn of each day. Toremove theash, use the hoe provided to scrape the ash remaining above the grate into the ash chamber below.Remove the ash via the ash door opening with the hoe. Place all ash in a fireproof container to avoidany possibility of fire ..

PRE-HEAT SUMMARY

1. Remove ash.2. Close ash and fill doors.3. Be sure "power on" switch is on.4. Turn on "preheat" momentary switch5. Wait 30 minutes6. Load waste7. Proceed to Incinerate Cycle

(2) Incinerate Cycle

When the preheat cycle is completed, the waste is loaded and the fill door is closed turn onthe "incinerate" momentary SWitch.This activates the timers and interlock. The lower burnerwill automatically ignite 5 minutes after 1800 degrees is proven in the secondary chamber.Another timer will turn the gas off to the lower burner at a specified time depending on the typeof waste bein~ burned. Finally the incinerator will shut down at the preset time. (See page 10for timer settmgs).

The P25-2GMI may be recharged every hour. It may be necessary to adjust the interlock timeaccording t9 individual state regulations. Do not recharge if there appears to be excessiveflame when the fill door is opened. ;:

RECHARG E PROCEDURE:

1. Be certain safety precautions are taken (p. 10).2. Loosen fill door latches but do not unhook.3. Wait a few seconds to see that no flash occurs.4. Unhook door and open to full position.5. Charge waste.6. Hook and latch door. The timers are again reset.

NOTE: At times the P25 operates in an oxygen-starved condition. When this occurs,severe flames may flare out when opening the fill door. Because of this, it is very importantthat steps 1-3 are followed. '

(3) Burn-Down Cycle

The P25 requires a burn-down period to complete the incineration process. During this cycle,any waste, including liquids, that fell through the grates is burned, and it is ensured thatall ash and residue is sterilized.

Experience will dictate if the burn down time is adequate for the type waste being incinerated.If incineration is incomplete, adjust timer #2 to extend the burn down time.

MIXED WASTE:

The P25 was emissions tested using red bag hospital waste with a 12,000 BTU/LB heat value.

The P25 may be recharged every hour depending on the BTU content of the waste and stateregulations.

ANIMAL WASTE:

The P25 is designed to burn a single load containing 600 pounds of animals in approximately6 hours.

The P25 was tested using small animals with a 1000 BTU/LB heat value.

The maximum capacity of the primary chamber is 600 pounds of animals .

.MAINTENANCE1

Low mainten~nce can be anticipated if owner's manual procedures are followed.

Lubricate blower motor every year with oil provided or an equivalent. Removethe blower cover (4 screws) and lubricate according to the instructions on theblower label.

A coatin~ of graphite or light ~rease (not in excess) should be maintained atthe slip fItting on the modulatmg control linkage. (See Figure 1 - ModulatingControl Linkage.)

Apply WD40, or equivalent, to the actuator and gas valve shafts occasionallyto prevent sticking. (See Figure 1 - Modulating Control Linkage.)

Replacement parts are available from the Shenandoah dealer. Only originalequipment parts should be used for repairs. '
