SMOKE FREE HOSPITAL

1. Access to Hospital Services

You have the right in a medical emergency to beadmitted immediately to hospital. In cases other thanin an emergency, you will be placed on a waiting list ifyou cannot be admitted to hospital immediately. If youare on a waiting list and are concerned about yourcondition, you should consult your family doctor whocan then request that your condition be reviewed byyour hospital consultant.

Where a recommended medical procedure is not avail-able at the hospital, you will have the right to ask yourhospital consultant to transfer you elsewhere where theprocedure is available.

You have the right, should your admission be cancelled by the hospital, to receive adequate andtimely notice of such cancellation. However, in excep-tional cases arising from emergency pressures or staffillnesses, your operation may have to be cancelled atvery short notice. In these circumstances, the hospitalwill make every effort to contact you in advance.

You have the right, in the event of cancellation, to begiven a new appointment for an early date and to betreated on a priority basis.

2. Out-Patient Services

You have the right, when your family doctor refersyou to hospital for an out-patient appointment, to:

receive confirmation within a reasonable timeof the date of your first appointment;

be given an individual appointment time;

be seen by a consultant or senior doctor onyour first appointment.

If you feel your condition has disimproved, you shouldconsult your family doctor who can, if necessary, takeup the matter with the hospital.

You have the right, should your appointment at an out-patient department be cancelled by the hospital, to receive adequate and timely notice of such cancellation and to be given a new appointment on apriority basis.

3. Courtesy

You have the right to be treated in a courteous mannerat all times by every member of the hospital staff.

4. Visiting Arrangements

You have the right to receive visits from your relativesand friends, including children. The hospital mustensure that visiting arrangements are flexible, consis-tent with the nature of your illness and the needs ofother patients.

5. Religious Beliefs

You have the right to be treated with respect for yourreligious and philosophical beliefs.

6. Privacy

You have the right to have your privacy respected,especially when the nature of your clinical condition isbeing discussed with you or your relatives by hospitalstaff.

7. Information Concerning yourTreatment

Your have the right to be informed of the name of theconsultant under whose care you are being placed,and, if you are to be referred to another consultant,you have the right to be informed of the reasons forsuch referral.

You have the right to be informed of the nature ofyour illness or condition in language which you canfully understand, and to be informed concerning:

the results of your tests and x-rays;

the purpose, method, likely duration andexpected benefit of the proposed treatment;

alternative forms of treatment;

possible pain or discomfort, risks and side-effects of the proposed treatment.

8. Consent to Treatment

Generally, treatment should only be given to a patientwith his or her informed consent or, in the case of achild, the consent of a parent or guardian. You mayrequest the presence of a person or persons of yourchoosing during the procedure for granting consent.The consent form you are asked to sign should clearlystate the nature of the procedures to be undertaken.

Only in cases where a patient lacks the capacity togive or withhold consent, and where a qualified med-ical doctor determines that treatment is urgently neces-sary in order to prevent immediate or imminent harm,may treatment be given without informed consent.

A copy of this Charter can beobtained at your local hospital.An Irish language version of theCharter is also available.

9. Confidentiality

You have the right to total confidentiality in respect ofyour medical records.

You have the right to request the hospital to makedetails of your relevant medical records available toyou. Hospitals will normally meet your wishes in thisregard, except where it would be considered that thiswould cause serious harm to your physical or mentalhealth. In such circumstances, the information may becommunicated through a health professional, normallyyour family doctor.

10. Teaching and Research

You have the right to refuse to participate in the teach-ing of medical students by your consultant. Your per-mission must be sought before a consultant caninvolve you in the teaching of students. However, yourco-operation would be important in view of the needto ensure that future doctors obtain the best possibletraining.

You have the right to refuse to take part in any clinicaltrials or research concerning the use of new drugs ormedical devices. Clinical trials and experimental treat-ment should never be carried out without yourinformed consent being obtained by the hospital ormedical personnel.

11. Discharge

You have the right on your discharge from hospital tohave yourself and your family doctor informed of thenature of your condition, the treatment you receivedwhile in hospital, the medication required by you, andthe arrangements for any further attendance at the hos-pital.

12. Complaints

You have the right to complain about any aspect ofhospital service, to have the complaint investigatedand to be informed of the outcome as soon as possible.

This booklet provides usefulinformation to help patientsprepare for admission to St. James’s Hospital. Itcontains details on theservices and facilitiesavailable as well asinformation on how to getto the hospital andimportant information aboutthe hospital.

We do all this to make surethat we can provide asmany appointments aspossible to the people whoneed them.

Under Department of HealthGuidelines, Public In-patients

are charged €66 per day upto a maximum of €660 inany 12 month period.Medical Card holders areexempt from this charge andmust produce a currentMedical Card.

The hospital operates a VHIand Quinn Direct PaymentScheme. Forms must becompleted prior todischarge in order to haveaccounts submitted directlyto VHI or Quinn Direct.

National TreatmentPurchase Fund (NTFP)The National TreatmentPurchase Fund is aGovernment agency fundedby the Department of Health& Children to reduce waitingtimes for public patients

awaiting surgery orprocedures on public waitinglists.

When you are placed on apublic waiting list at St.James’s Hospital as either apublic in-patient or day casepatient, certain informationabout you will be passed, ona confidential basis, to theNTPF patient treatmentregister. The detailstransferred will include: yourname, address, the date youwere put on the waiting listand the name of thetreatment you require.

ADMISSIONINFORMATIONAll admissions are arrangedthrough the AdmissionsOffice. You will receive anadmission card detailing yourprovisional date of admission,ward and consultant.

If you are unable to attend onthis date (e.g. because ofillness or personal reasons) itis really important that youcontact us as soon aspossible to reschedule. Thisappointment can then beallocated to someone else.However, please note that itmay be some time beforeanother admission datebecomes available. Thetelephone number to contactis on your admission card.

Waiting List DetailsFollowing assessment bythe Clinical Team, patientsmay be placed on the in-patient waiting list andprioritised by the teamaccording to medical needas: urgent, soon or routine.If you have any queriesregarding your waiting liststatus or if there have beenany changes in your details,(e.g. address, telephonenumber, your GP), pleasecontact the Waiting List

Office on 01 416 2256 or 01 410 3502, stating yourname, hospital number ordate of birth.

In order to ensure that wehave an accurate and up to date picture of our waitinglist, the list is closelymonitored and is constantlychecked. To do this we sendout a letter at regular intervalsasking you to confirm yourdetails. It is important thatyou complete and then returnthis letter to the AdmissionsOffice as soon as possible.

The Admissions Office islocated in the Concourse,Main Hospital. See the mapof the hospital on page 28.Follow the signs to Junction 2and park in the undergroundcar park (please note that anhourly rate is payable in thecar park).The lift or stairs willtake you straight to theConcourse where receptionstaff can provide directions.

There is a set down areaimmediately outside theConcourse where you canbe dropped off. No parkingis permitted there.

When you have booked inat the Admission Office youwill be given directions tothe ward. Once at theward, please report to thenurse’s station, which islocated in the centre ofeach ward.

• Bedwear (nightdress/pyjamas/ dressing gown)slippers (with non-slipsoles if possible)

• Toiletries/Towels

• All medication/tablets youare currently taking (you will be asked to send these home after they have been checked)

• Contact numbers of 2different people (next ofkin) who can be availablethroughout 24 hours

• The use of mobile phonesis not permitted in wardareas

• Please note that due to Health& Safety regulations electricalequipment, such as mobilephone chargers or plug-inradios, cannot be brought intothe hospital.

If you have not had yoursurgery within three months,the NTPF may write to you tosee if your operation orprocedure can be arrangedelsewhere. You can contactthe NTPF at Lo-Call 1890 720820 or visit their website onwww.ntpf.ie

Admission as a private

patient is by priorarrangement with yourconsultant. Your consultantwill contact the AdmissionsOffice to arrange youradmission as a privatepatient. Private in-patientcharges apply.

Attending forAdmissionAll admission dates areprovisional. Please telephonethe Admissions Office at11am on the morning of youradmission date to confirmthat a bed is available. Thetelephone number to contactis on your admission card.

Remember to follow any pre-admissioninstructions you mayhave been given by yourdoctor or nurse, such asfasting or medication.

When you have confirmedthat a bed is available,please make your way to StJames's Hospital and report atthe time requested to theAdmissions Office. If a bedis not available for you onthat day you will be givenanother date as soon aspossible.

If you are having surgery onthe same day as youradmission it is very importantthat you arrive on time. Latearrival may result in yoursurgery and your admissionbeing postponed.

On the day of your dischargewe ask that you vacate yourbed by 9.00am or earlier. Youmay be transferred to theDischarge Lounge whilewaiting for your dischargearrangements to becompleted or while waitingto be collected. TheDischarge Lounge is asupervised comfortablewaiting area.

On discharge you will receivea Discharge Summary Letterfor your GP and, if required, a prescription, an outpatientappointment or medicalcertificate.

If you are unsure of anyaspect of your dischargeplan we would encourageyou to talk to the Nurse inCharge in your ward.

Special NeedsAt St. James’s Hospital weare committed to addressingthe special needs of ourpatients. If you have aspecial need, such asrequiring the use of awheelchair or an interpreterservice, please make thisknown, beforehand ifpossible, to the AdmissionsOffice. Every effort will bemade to accommodate you.

ValuablesYou are strongly advised toleave jewellery (other thanwedding band) & valuablesat home. The Hospital

Board does not accept

responsibility for the loss or

damage to any valuables,

money or other articles that

you bring with you. Thehospital does not providesecure storage areas.

Personal PropertyPlease send as muchpersonal property aspossible home in advance.If travelling by ambulance

please be aware that thereis a strict luggage limit.

Planning YourDischargeEvery effort will be made toprepare you and your familyfor going home. Yourdischarge plan will startfrom your time of admissionto St James’s and you willbe given an estimated dateof discharge as soon as it ismedically possible to do so.

It is advisable to think aboutand plan arrangements foryour discharge in advance,such as how will you gethome and do you have yourhouse keys?

All the health careprofessionals involved inyour care plan will worktogether with you to get youhome in a safe and timelymanner.

You may also be dischargedat short notice or in theevening time if you aredeemed medically fit fordischarge and in some

Acute MedicalAdmission Unit(AMAU)The AMAU is a short stayunit which receives acutelyill medical patients from theEmergency Department 24hours per day 7 days perweek.

When you arrive in theAMAU you will be assessedby a member of the Medicalor Nursing team. Our aim isto find out what special careyou will need to treat yourillness as soon as possible.An individual care plan willbe put in place providingyou with rapid access to investigations andC o n s u l t a n t / S p e c i a l i s tassessment. This is a shortstay unit, which means youmay be transferred to aspeciality ward for theremainder of your stay inthe hospital.

Day Care ProceduresPlease remember to readand follow any instructionsgiven to you in advance of

your arrival at the hospital. Ifyou are going to be sedatedfor the procedure you willNOT be able to drive for 24hours. You will not be ableto drive yourself homeafterwards. You will alsoneed to have a responsibleadult collect you and staywith you for 24 hours untilthe sedation wears off.

GP Blood TestingThis service is onlyavailable Monday to Fridayfrom 7.30am till 12.00noon. The PhlebotomyClinic (Blood Testing Clinic)is located in the Out PatientsDepartment on Route 2 viathe Main Concourse (seemap on page 28).

EMERGENCYDEPARTMENTThe Emergency Departmentis for emergencies only.Please do not come to thedepartment instead of goingto your GP.

On arrival into theEmergency Department, allpatients are registered atthe desk. They are thenseen by a trained TriageNurse who will assess theirmedical condition. Patientsare listed according to theseriousness of theircondition and the mostseriously ill people aretreated first. Every effortwill be made to keep allpatients informed aboutwaiting times.

Please understand that onoccasion some injuriesrequire more immediateattention than others.

When this happens thePatient Liaison Officer orStaff Nurse will inform youof an increase in yourwaiting time.

Emergency ChargesUnder Department of HealthGuidelines all patientsattending the EmergencyDepartment are charged anattendance fee of €66.00.

Exceptions include;

• Patients who have beenreferred by their GP andhave the referral letterwith them

• People who have medicalcards if they have theirmedical card with them

HOSPITALINFORMATION

HygieneHygiene is a fundamentalpart of the hospital’s qualitysystem to ensure thesafety and wellbeing ofpatients, staff and visitorsand it plays an importantrole in the prevention andcontrol of infection.Achieving and maintainingthe highest hygienestandards is everyone’sresponsibility.

Hand HygienePlease ask your visitors towash their hands or use the

alcohol gel, available at allward entrances, to disinfecttheir hands each time theyenter or leave the ward. Dothis yourself also if you gointo any other ward. Ask amember of staff to showyou if you are unsure howto use it. Feel free to askany medical staff you comeinto contact with if theyhave cleaned their hands.Pick up a hand hygieneleaflet available on allwards/departments. TheGolden Rules for Visitorsare displayed in all wards inthe hospital.

Visiting We recognise the importanceof support from family andfriends when you are apatient in hospital. However,patient care is our primaryconcern at St. James’sHospital. In order toenhance the quality of careand ensure the safety andcomfort of our patients,specific visiting times andregulations have beenestablished.

• A maximum of 2 visitors,including children, at anyone time is permitted

• The recommended visitingduration is 20 minutes

• Any person feeling unwellis asked not to visit thehospital

• Children under the age of14 years are not permittedto visit patients in certainhigh risk wards

• Special arrangements canbe made for relatives tovisit critically ill patients.These are limited to nextof kin and must be agreedwith the Nurse in charge

• Closure of the MainDoors to the Hospital. Themain doors of all thehospital buildings arelocked from 10.00pm until6.30am. During thesehours no one will bepermitted to enter thehospital without specialpermission in advance

Visiting Times

Daily 2.00pm to 4.00pm

Evening 6.00pm to 8.00pm

1.Use enough soap to cover all of the hand area.

2.Wet hands and wash throughlywith soap.

4.Dry hands thoroughly. Your handsare now clean and safe.

3.Rub all areas and include fingersand thumbs.

Moving of Patients(Lifting) In St James’s Hospital clinicalstaff will assist, as much as ispractical, in the moving andhandling (lifting) of patients.However it is important that,where possible, patients doas much of the movingthemselves. The reasons forthis are to assist in patientrehabilitation and also toreduce the likelihood of injuryto hospital staff, inaccordance with Health &Safety regulations.

SecuritySt. James’s Hospital ismonitored by CCTVcameras 24 hours per dayand operates a zerotolerance policy for antisocial behaviour. Thehospital participates in theHospital Watch programmedeveloped by An GardaSíochána. Hospital Watch isa hospital-wide initiative,supported by Gardaí, whichenhances the safety andsecurity of patients, staff,visitors and property in andaround the hospital. St.

James’s Hospital has twodedicated Garda LiaisonOfficers and has regularpatrols in the grounds.Please report any securityincident to Security Staff ortelephone (01) 4162108.

Chaplaincy ServiceA Chaplaincy Service isavailable to all patients.Please inform a member ofstaff which denominationyou require. A member ofthe Chaplaincy and PastoralCare service visits thewards daily.

• St. James’s Hospitaloperates a zero tolerancepolicy relating to any kindof verbal or physicalabuse. Patients or visitorswhose behaviour causesdisturbance to otherpatients or staff will beasked to leave

Staff Identity

All St. James’s Hospitalstaff are required to wear aname badge, with theirphotograph and title on it, atall times. As well as thedoctors and nurses, therecould be several othermembers of staff in yourcare team, for example, aphysiotherapist, a dieticianor a porter. If you have anydoubts, please ask the staffmember to either show youtheir badge, or explain theirrole to you.

Smoke Free PolicySt. James’s Hospital is asmoke free hospital. Thehospital aims to provide asafe and healthy smoke freeenvironment for all patients,staff, contractors and thepublic. In keeping withhospital policy and legislation,smoking is strictly prohibitedin all hospital buildings(including the undergroundcar park) and anywhere nearall entrances. Designatedsmoking areas are providedand are clearly signposted.Cigarettes cannot bepurchased at the hospital.

Smoking cessation advice,information and training isavailable to all patients. Forfurther information onsmoking cessation pleaseask the Nurse in Charge tocontact the SmokingCessation Officer for you.

Data ProtectionSt. James’s Hospital takes veryseriously the protection of ourpatients’ rights to privacy andconfidentiality. We do this byfollowing best practices in howall information is handled andstored in the hospital, inaccordance with DataProtection law.

Your health record is made upof a combination of manualand electronic information.Once you become a patient ofSt. James’s Hospital, allconfidential information aboutyour care and treatment will beprocessed fairly. It will only beshared with others if it assistsyour medical care andtreatment. Information mayalso be shared as part of thenormal administration of thehospital.

Information, which cannot betraced back to you, is providedby St. James’s Hospital toother health care agenciessuch as the Department ofHealth & Children, the HealthService Executive (HSE), theEconomic and Social Research

Institute (ESRI). Thisinformation is provided forresearch and data analysispurposes.

St. James’s Hospital is aregistered Data Controllerunder the Data Protection Acts1988 and 2003. Under theData Protection Acts you havethe right to know whatpersonal information relating toyou is being held by St.James’s hospital and to accessthis information to ensure it isaccurate. DURING YOUR STAY

AccommodationYou will be accommodatedin a 4 or 6 bedded ward.Single rooms are notguaranteed as these aremainly used for medicalreasons (e.g. isolation).

You may be moved to adifferent bed or ward if yourclinical condition changes, orto accommodate otherpatients’ clinical conditions.This can sometimes happenduring the night. Your co-operation is appreciated.

Catering Menus are given out at8.30am to order meals forthe next day. Please informstaff of special dietaryrequirements, e.g. Coeliac,Kosher, diabetic, lowcholesterol, etc.

Mealtimes

TelephonesBoth coin and cardtelephones are available on each ward. For thosepatients unable to get out of bed, a coin telephone can be moved to each bedas required.

In order to respect theneeds and privacy of ourpatients the use of mobilephones is not permitted inward areas.

Participation inClinical Trials/Research

St. James’s Hospital is theprimary academic teachinghospital for Trinity CollegeDublin. You may, therefore,be asked to participate inthe teaching of medical/nursing students or to takepart in fully approved clinicaltrials or research projects.

You have the right to refuseto participate in any of theseas your permission must besought before you can beinvolved. However, your co-operation is important inview of the need to ensurethat future clinical staff obtainthe best possible training.This will not affect yourtreatment in anyway.

Cash Machines There are two ATMmachines on site; one islocated inside the mainentrance of the Concourseand one is located inside theconvenience store.

Postal ServicesThere is a post box locatedinside the main entrance tothe Concourse, next to theATM machine. Stamps canbe purchased in theconvenience store. 21

TelevisionA television (with earphones)may be hired from anexternal company for yourown private use on a daily orweekly basis for a nominalcharge. Please ask a memberof staff to contact the TV HireCompany for you.

FlowersFor medical reasons it is notadvisable to have flowers insome wards. Please checkwith the Nurse in Charge forthe local policy on your ward.

Laundry ServiceThere is no laundry serviceavailable at St. James’sHospital.

Pharmacy: There is a pharmacy shoplocated on the first floor in the Concourse, MainHospital. Medical Cardprescriptions are available at this pharmacy. This is anindependent communitypharmacy and is not managedby St. James's Hospital.

ShoppingThere is a morning paperround and a mobile shopvisits the wards daily.

There is a convenience storelocated on the ground floorin the main hospitalConcourse which stockssnacks, drinks, books,newspapers, magazines,cards, gifts and toiletries.

There is a gift shop alsolocated on the ground floor of the Concourse. Openingtimes vary.

public car park for patientsand visitors with 367 spaces.It is located off Junction 2(see map on page 28). Thereis direct access to the mainhospital Concourse andReception from the car parkvia the lift or stairs. Limitedadditional surface parking isalso available, located atJunction 2.

There are two parking

payment machines; one islocated inside the mainentrance to the Concourseand the second is in theunderground car park next tothe lifts. You may also pay atthe desk near the exit of theunderground car park.

There are a number ofdisabled parking spaces

available in the undergroundcar park and also at Junction3 and at other locationsaround the hospital. Youmust display a DisabledDriver sticker.

Bus RoutesThe Dublin Bus Route 123passes through the hospitalgrounds and stops near themain hospital entrance.From the city centre, the78A, 51 and 51B stop at theJames’s Street entrance.The 19 stops on the SouthCircular Road close to theRialto Gate entrance. The 17stops nearby at Rialto.

LUASThe Red Line betweenTallaght and ConnollyStation has three stopsadjacent to the hospital. The James’s stop is at theJames’s Street entrance.The Rialto and Fatima stopsare nearby. This line alsoserves Heuston andConnolly Stations.

An external managementcompany manages carparking on site. All vehicles parked unofficiallythroughout the hospitalgrounds will be clampedand a release fee of €24.00applies. This is in theinterest of safety and toensure the flow of trafficaround the hospital.

Other parking restrictionsexist in accordance with theRules of the Road and allusers are required to obeytraffic signs. Fire routes aremarked and vehicles causingobstruction or displayinginconsideration to otherusers will be clamped ortowed away (a clampremoval fee applies).

Normal security measuresapply in relation to securingvehicles. All patients andvisitors are advised not toleave any valuables ondisplay in their vehicle. TheHospital Board does notaccept responsibility for theloss or damage to anyvehicle parked within thegrounds of the hospital.
