A Healthy New Start “I want to be happy again.” Evelyn was in a funk. She was in her 50s and, with a BMI of 32, was facing obesity. She had always been an upbeat person, but lately was depressed and in pain. She had a busy job that made it dicult to t in exercise, and had become sedentary. Month aŌer month, Evelyn watched the number on the scale creep up. None of her clothes t, her back hurt constantly, and her knees hurt aŌer walking just short distances. A visit to the doctor revealed damage to Evelyn’s lumbar vertebrae that required surgery, but he menƟoned this was just a temporary x. Even aŌer surgery her weight would conƟnue to put pressure on her bones and joints, causing chronic pain. Evelyn was miserable and felt defeated. Hope, and a plan Her surgery was a success, but Evelyn knew she sƟll needed to make a change. At an employee health fair at work, she learned about the Disease Management program from Meritain Health. She enrolled and began working with a nurse health coach named Sarah. AŌer listening to Evelyn’s story, Sarah designed a plan to help her reach a healthy body weight. Evelyn was hopeful that losing weight would help her gain freedom from the constant pain she experienced. A year of hard work Sarah’s ongoing support and encouragement kept Evelyn moƟvated to work towards her goals. She explained it would be hard at rst, but with pracƟce, regular exercise would become a habit. Sarah helped Evelyn nd acƟviƟes she liked that t into her busy day. They also talked about ways Evelyn could cut out extra calories and eat healthy meals. Slowly but surely, Evelyn’s weight began to go down. With each pound she lost, she felt more and more condent. AŌer a year of hard work, Evelyn has lost more than 65 pounds from when she rst started working with Sarah. She’s a changed woman: her back and knees no longer hurt, and she has a new-found love of tness. A Healthy New Start“I want to be happy again.”

Evelyn was in a funk. She was in her 50s and, with a BMI of 32, was facing obesity. She had always been an upbeat person, but lately was depressed and in pain.
