The VGH Department of Emergency Medicine has a tradition of producing high quality original research, and collaborating on the research initiatives of other VGH Departments or external groups. This academic mission must be balanced with the demands research places on Emergency Department patient care and teaching of Residents and Medical Students. In addition, simultaneous research projects may conflict with one another and may compete for the same patients as well as for the

time and attention of our clerical, nursing, and medical staff.

For these reasons, all potential studies which require signoff by the VGH Department of Emergency Medicine undergo internal review, approval, and prioritization. Research Activities

Adverse Drug Events: In 2013, Dr. Hohl secured funds to validate the Adverse Drug Event Clinical Decision Rule, the first evidence-based tool that assists emergency de-partments to risk-stratify patients for adverse drug events (ADEs), and enable referral of high-risk patients for medication review services with a clinical pharmacist. The project is currently enrolling patients at 3 sites: VGH, Lions Gate Hospital, and the Ottawa Hospital. Interim analysis results indicate high sensitivity of the Rule for identifying patients at high risk of experiencing an ADE. The team anticipates that the validation of the Adverse Drug Event Rule will enable widespread implementation in compliance with Accreditation Canada's 2014 required organizational practices. In 2013, Dr. Hohl's team was also awarded a Partnership for Health System Improve-ment grant from CIHR to develop an ADE reporting platform in PharmaNet. The team has completed a systematic review of 105 international ADE reporting platforms to develop the reporting system, and has engaged in focus groups with various clini-cal stakeholders in order to refine the data fields for reporting. Piloting of the ADE reporting system is anticipated for the fall. Dr. Hohl continues to actively collaborate on national clinical decision rule development studies on neurosurgical and cardio-vascular emergencies, and is an active member of the Drug Safety and Effectiveness

Research Network.

Research Education: Riyad Abu-Laban, UBC Department of Emergency Medicine Research Director, led a highly successful 4th annual "NERD Block" (Novel Educa-tion in Research & Design) for 15 Royal College EM Residents and Peds EM Fellows with Dr. Quynh Doan in the fall of 2014. This 1 month program involves 34 interac-tive small group sessions and workshops facilitated by faculty members from the De-partment of Emergency Medicine and Division of Pediatric Emergency Medicine on all aspects of research and critical appraisal. NERD participants gain foundational knowledge, interact with and learn about the research being done by a wide range of Emergency Medicine faculty members, and develop and advance their own research projects. Since its onset, the NERD Block has gained wide attention, both across UBC and nationally. Dr. Abu-Laban presented the background and experience with the initiative to the Royal College of Physicians and Surgeons EM Specialty Commit-tee in Ottawa in June 2014, and it is anticipated that the NERD Block will receive specific mention in an upcoming CJEM consensus publication on "Promoting Excel-lence in Canadian Emergency Medicine Resident Research". The NERD Block now includes slots for EM Faculty members, and four people have taken advantage of this

publications) on any significant peer-reviewed or investigator-initiated collaborative project conducted in the VGH Emergency Department. This involvement should commence during the development of the study protocol and the ED should receive

appropriate acknowledgement for the role it plays for any such projects.

eHealth Strategy: Dr. Kendal Ho leads the UBC ehealth strategy office, driven by the vision to “connect people to better health”. Their research focuses on how health information technologies can assist patients, their families, and health professionals to optimize health outcomes. Highlights of their research endeavors this past year include: Inter-Cultural Online health Network (iCON): engaging the Chinese and South Asians in optimal self management of chronic diseases, Mobile Digital Access to Web-enhanced Network (mDAWN): using mobile sensors, social media and text messaging to support type 2 diabetes patients and their caregivers in optimizing self management, Social Media for public engagement, research, and health professional education, Electronic health record for medical education, Global eHealth for under-served communities, and Improving eHealth literacy of the general population in

B.C., a collaboration with the BC Ministry of Health.

Smoking Cessation: Dr. Ka Wai Cheung and team continue their smoking cessa-tion research. Her randomized control trial is examining the effectiveness of refer-ring ED smokers to the provincial Quit Now smoking cessation service. Patients are followed for 12 months to record patterns in smoking, desire to quit, and use of nico-tine replacement therapies. Her team has completed the follow up process for over 1,300 enrolled patients and data analysis is currently underway. Dr. Cheung’s team is also looking at whether specific patient factors affect a patient's choice of service modality. This will ultimately help refine smoking cessation services. Finally, the team is also looking at the accuracy of patient perception that their ED visit may be related to smoking. This will help to determine whether further patient education may be needed.. This past year three of Ka Wai’s students presented posters at the

CAEP conference using preliminary findings from their study.

Road Safety: Our road safety research team of Dr. Jeff Brubacher, Dr. Herbert Chan (PhD), and Dr. Roy Purssell have enrolled over 2200 injured drivers for their study “Cannabis and Motor Vehicle Crashes: A multicentre culpability study”. This CIHR funded study has been launched in 8 trauma centres across BC and will deter-mine the role of marijuana use in causing car crashes. Preliminary findings from that study indicate that alcohol was used by 17.8% of injured drivers and that 12.6% of drivers tested positive for cannabis metabolites, and 7.3% were positive for THC, indicating recent use. Cannabis was more common in males and in drivers <30 years old. Cocaine was positive in 2.8% of drivers and amphetamines in 1.2%. Medica-tions including diphenhydramine (11%), benzodiazepines (5.0%), zopiclone (0.6%), antidepressants (6.5%), and opiates (4.9%) were also detected. In total, 26.1% of

drivers were positive for at least one sedating medications.

The team published an evaluation of British Columbia’s new immediate roadside penalty (IRP) regime in the American Journal of Public Health. They found that the new laws resulted in a 21% reduction in all cause fatal crashes, an 8% reduction in hospital admissions for road trauma and a 7.2% reduction in ambulance calls for

road trauma.

Road Safety (continued): Dr Chan together with Cassie Fayowski and Lauren Stewart are studying crashes involving elderly drivers. That study is comparing the injury profile and use of healthcare resources in elderly drivers versus younger

drivers who are treated in the ED after a motor vehicle crash.

Dr Brubacher is chair and Dr Chan is a member of the BC Road Safety Research

and Data Committee which is one of the working committees for the BC Road Safety Strategy. The committee aims to improve access to high quality road safety

data and to increase provincial road safety research capacity.

Ultrasound Teaching & Research: The acute onset of flashes and floaters in the eyes is a common presentation to the emergency department. The most emergent etiology is retinal detachment, which requires prompt ophthalmologic assessment. This study, led by Dr. Dan Kim and Dr. Donna Lee is designed to assess the test characteristics of point of care ultrasound performed by a large heterogeneous group of emergency physicians for the diagnosis of retinal detachment. This will be a prospective observational study assessing a convenience sample of patients presenting to the Vancouver General Hospital emergency department with com-plaints of flashes or floaters in one or both eyes. Patients will undergo standard emergency department ocular assessment, after which a participating physician will perform an ocular ultrasound exam targeted at detecting retinal detachment. The criterion standard for retinal detachment will be the final diagnosis given by the retina specialist who will be blinded to the results of the ultrasound findings. If the test characteristics of point of care ultrasound are similar to previous studies (reported pooled sensitivity 99%), ocular ultrasound may be an excellent tool for triaging patients with new onset flashes and floaters to definitive ophthalmologic

assessment.
