Welcome!
It is our families and supporters who are the reason we are celebrating 20 years of service as the largest provider of community-based, dementia-specific services in Florida. Alzheimer’s Community Care serves 4,500 individuals in Palm Beach, Martin and St. Lucie Counties, with a team of professionals skilled and trained in dementia-specific care and education.
We provide our community with 11 Specialized Alzheimer’s Care and Service Centers, Family Nurse Consultants, a 24-Hour Alzheimer’s Crisis Line, an ID Locator Bracelet Service, Case Management, Community & Professional Education, Advocacy, Support Groups, and Training.
We invite you to learn more about us in the pages that follow, outlining the work we have done throughout our Fiscal Year, which ended June 30, 2016.

all families deserve to have access to quality, affordable, appropriate services and resources. The story below, as told by Mary M. Barnes, depicts why ACC is so vital to our families, as Don Baxter and our Founders had envisioned.
“A daughter-in-law called my office 10 months ago, distraught and concerned with the well-being of her father-in-law and mother-in-law, which was being jeopardized because of the effects of Alzheimer’s disease. During our meeting the next week, she continued to convey her deep concernfor her in-laws and her husband, as well. It became apparent that I needed to speak to her husband, one of six siblings who was obediently following dad’s orders of, ‘I do not need any help. Mom and I are doing just fine.’ The son came in with his wife and once I heard what was happening to dad, I started speaking with them about the ‘whys’ of the disease’s behaviors - theysoon realized that maybe things were not going as well as was reported and that with dad being in such denial regarding their diminished quality of life, a next step needed to be taken. Within two weeks dad and mom, son and daughter-in-law were in my office, and dad soon agreed that mom could ‘try’ our specialized Alzheimer’s Day Care and Service Center in Palm Beach Gardens.
“The A-Team at ACC went into crisis action mode. Mom immediately took to the music, dancing with laughter and soon had a new step in her walk that her husband and family had not seen in a long time. Unfortunately, fast forward to September 2016, she fell at home and broke her hip. In x-raying her hip they saw a mass on her lung and within three weeks she had succumbed to the effects of cancer. I attended the funeral service and it was deeply moving and inspiring to witness a loving image of how the family was all together, dad being the center of the children’s and other family member’s universe. They were holding, hugging each other and kissing tops of the little grandchildren’s heads and celebrating this wonderful mother and grandmother’s life they had been a part of for so many years.”

In October 1996, just days before one of our visionary founders, Don Baxter, passed away, Alzheimer’s Community Care (ACC) received our 501©(3) Internal Revenue Determination Letter. While Mr. Baxter never saw his dream building completed – we did move into the Baxter Building in West Palm Beach in July 1997 – he knew that ACC was a genuine non-profit and was going to care for and support Alzheimer’s families according to the belief that

Centers are licensed in accordance with the higher standards of Florida’s Specialized
Alzheimer’s Day Care Act of 2012.
“I cannot tell you, literally cannot put into words, how grateful I am for the angels who staff these day cares. I shake my head all the time at the amount of patience, care and love they exhibit for their charges. They allow them to retain their dignity, which I think is so huge for those suffering from this disease. It truly takes a special person to work with those afflicted by this disease. They go out of their way to welcome my husband, every single day. They truly do keep him stimulated and motivated the entire day! I can see when I pick up my husband he has had a good day and is a happy camper.”

Our Program Nurses and staff are dementia-care specialists, with ongoing training in Alzheimer’s disease and related neurocognitive disorders. We maintain a staff-to-patient ratio of 1-to-5, to ensure each patient receives the individualized care needed. Program Nurses monitor patient nutrition and hydration, administer medications, evaluate health status, respond to medical needs and ensure that patients are involved in care to their maximum benefit every day.

The Family Nurse Consultants’ primary role is to provide potential interventions, available resources, possible measures and outcomes at key stages in the disease’s progression. Interventions and needs assessments implemented throughout all stages of dementia are critical; this is the path to improving the quality of life and care for those affected by Alzheimer’s disease and related neurocognitive disorders.

The Family Nurse Consultant (FNC) program serves as a lifeline for families in Palm Beach, Martin and St. Lucie Counties who are living with Alzheimer’s disease and related neurocognitive disorders. Our FNCs provide caregivers with the support, tools and resources needed to continue providing home-based care for their loved ones.Our FNCs are a “one-stop shop” providing resources and support for:· Caregiver support groups· Crisis Intervention· Education· Advocacy· Home Health Agencies· Tools: - Family Risk Guide - Five Wishes

Our 24-Hour Alzheimer’s Crisis Line provides caregivers with understanding and guidance that can help them throughout the many stages of Alzheimer’s disease and related neurocognitive disorders.
The Crisis Line serves Palm Beach, Martin and St. Lucie Counties. Family Nurse Consultants (FNCs) respond to calls and can connect directly to law enforcement for assistance during search and rescues.
The Crisis Line is also an excellent crisis management resource for caregivers. In all cases, the FNC responds to the inquiry, follows up as appropriate, and links the family to essential services and resources.

Education Spotlight: Priscillia (Lia) Rinaldy, RNAlzheimer’s Community Care is extremely proud of the Nursing Preceptorship Program. Since 1994, we have provided classroom training for students of nursing assisting, licensed practical nursing, registered nursing, health administration and social work.
Priscillia (Lia) Rinaldy, RN came to Alzheimer’s Community Care as a student of Indiana State University (ISU). At that time, Lia was a Licensed Practical Nurse enrolled in ISU’s RN-BSN program. Lia quickly established herself as a diligent, self-directed student of registered nursing, and initiated several projects during her practicum. Lia drafted an Education Brochure for our Education Department, and created and presented an educational program for attendees of our West Palm Beach Support Group!
Lia is now our Family Nurse Consultant covering south Palm Beach County.

The mission of Alzheimer’s Community Care’s Education Department is to provide the latest education on Alzheimer’s disease and related neurocognitive disorders for patients, caregivers, staff, law enforcement, students of health professions and community health care partners. We also monitor, support and promote the organization’s accredited model for specialized, disease-specific care.
Alzheimer’s Community Care presents a two-day Annual Educational Conference in the spring of each year, providing family and professional caregivers with the latest information and strategies presented by experts in medicine and nursing, elder law, long-term care and current research.
All Conference programs are approved for Continuing Education Credit.

Case Managers are often called the gatekeepers at Alzheimer’s Community Care. Although this term is accurate, to a family in need the Case Manager is much more - they are their advocate and support system. Case Managers work with families with financial
need to determine their eligibility for funding.
Whether this funding is provided by the state, local government entities or through private funds secured by Alzheimer’s Community Care, these funds are critical to protect
the well-being of the patients and caregivers.
Case Managers bring together the people in need, with services and resources available, and connect them with the financial assistance to pay for these special needs.

The Alzheimer’s Community Care ID Locator Service provides an additional safety net tool for patients with Alzheimer’s disease and related neurocognitive disorders who may wander away from a safe and supervised setting. This program partners with SafetyNet Tracking Systems and Law Enforcement agencies of all three counties, to assist in a wandering event.
This technology, unlike cellular and GPS technology, assists rescue personnel to track a patient’s signal through concrete buildings, shallow water and densely wooded areas. Prompt location of the patient is essential because 80 percent of those missing for more than 72 hours will not survive the event.
In Fiscal Year 2016, The FAA of Palm Beach County funded the purchase of an additional 52 bracelets. The Leo & Anne Albert Charitable Trust & Boca Rio Foundation funded the purchase of 35 transmitter units, plus a year’s supply of 150 transmitters.

Sophia was found waist deep in a river in St. Lucie, after being located by Law Enforcement. The patient was supposed to take the family dog outside, but after noticing the patient had not come back, the community was alerted.
The patient walked to the dock, where the security gate had not been closed. The patient continued to walk and the gate closed behind them and locked. The dog remained at the gate as the patient went into the river. Fortunately, Law Enforcement was able to return the patient home safely. The caregiver is now on high alert and relieved because of Alz-heimer’s Community Care’s intervention and the ID Locator Service that benefitted their family in this situation.

Partnerships Making a Difference in Our Community
Alzheimer’s Community Care is grateful to the numerous individuals, foundations and organizations that provide ongoing support to ensure dignity, safety and well-being for families living with Alzheimer’s disease in our community. In FY 2016, thanks to this
support, we broadened our safety net of care through the following initiatives:
The beautiful new day center located on the campus of St. Michael Lutheran Church opened its doors on July 20, 2015, to serve up to 35 patients on
a daily basis. The total “make-over” of the center was made possible through major gifts
received from these two wonderful foundations. Along with providing therapeutic and
compassionate care for patients, this center is providing much needed respite from the challenges
of 24/7 caregiver for families.

To ensure continued quality service and coordination of care for our patients and caregivers, Alzheimer’s Community Care made the decision to
purchase and implement a new electronic medical records software system.
Along with providing seamless communication of confidential patient and caregiver information, this system allows us to track treatment outcomes for
quality assurance. The software has been customized to meet the unique care needs of families
receiving community-based care.

Impact 100 of the Palm Beaches
Palm Beach County is home to approximately 65,000 individuals with Alzheimer’s disease, with 70% being
cared for by family members in their homes. To maintain safety and well-being of both patients and caregivers, as well as the community-at-large, families rely on care and
expertise of nurses specially trained in providing dementia-specific care. To address the need for specialty training, Alzheimer’s Community Care formed strategic
partnerships with five Schools of Nursing in South Florida:
Thanks to a competitive award of $100,000, 250 future nurses will complete dementia-specific education coupled
with hands-on practicum experience caring for patients attending a Specialized Alzheimer’s Day Center.

Alzheimer’s Community Care was honored to receive a grant award of nearly $1 million to expand dementia-specific services and resources for families living with Alzheimer’s disease in Southeast Florida. The award supports a three-year project, Pathways
to Care,which will provide dementia-specific education for approximately 500 nursing students,
healthcare professionals, and family caregivers.
Additionally, access to community-based resources, including expanded Specialized
Alzheimer’s Day Services and specialty nursing resources, will be available throughout Palm Beach,
Martin and St. Lucie Counties.
Pathways to Care is a collaborative initiative that includes the University of South Florida, Veterans
Administration Medical Center in West Palm Beach, and four Florida-based Schools of Nursing.

Volunteer Spotlight: Dale BruhnAt the age of 92, Dale Bruhn is a selfless man whose service to others provides them with hope and inspiration. Dale’s wife had Alzheimer’s disease, and he cared for her until she passed away in February 2002.
Dale was looking for some place to share his own experiences as a caregiver and found Alzheimer’s
Community Care. He has been a volunteer since October 2009. His activities are broad in scope, including: office work, Caregiver Connection, Men in the Kitchen programs and serving as the facilitator for two of our organization’s caregiver support groups, the Males Only and the Boca Raton Support Group.
Honored by the Town of Palm Beach United Way with the 2016 Nettie Finkle Award, Dale has significantly touched the lives of many staff, patients and caregivers at Alzheimer’s Community Care.

Alzheimer’s Community Care’s Advisory Councils & Standing Committees are an essential part of the organization. They provide a way in which individuals, who have
special skills or unique networks, can make an important contribution to the advancement of our organization.
Alzheimer’s Community Care’s volunteers are the organization’s most treasured asset. In Fiscal Year 2016, Alzheimer’s Community Care was fortunate to have 175
volunteers who generously offered 7,051 hours of their time!
Alzheimer’s Community Care is proud to benefit from our volunteers’ time, commitment, experience and various skill sets that allow us to operate as a true
community-based organization. Our volunteers are really our community ambassadors; they work diligently to help our organization make a difference.
Alzheimer’s Community Care is extremely grateful for our volunteers’ commitment and service. We show our deep appreciation throughout the year
with various events such as the Annual Volunteer Breakfast. How lucky are we? Alzheimer’s Community Care still has volunteers from as far back as 1996, the
year we were founded.

Advocacy
In November 2015, more than 50 caregivers, Alzheimer’s Community Care Board members, and staff traveled to Tallahassee to advocate for funding services and resources that are making a difference for families every day in Palm Beach, Martin and St. Lucie Counties. This marked the sixth time that Alzheimer’s Community Care had ventured to the Florida State Capitol to meet with senators and representatives. The purpose of the Advocacy Campaign is to educate legislators and administration on the needs of patients and caregivers that are afflicted with a neurocognitive disorder and to provide a forum for caregivers to share their life experiences within a well-planned and dignified environment. With our caregivers communicating their personal stories and the value of Alzheimer’s Community Care in their lives, we met with more than 100 lawmakers. Robert J. Gorman, Vice Chair of the Board of Directors of Alzheimer’s Community Care, serves as Chair of the Advocacy Campaign. Over the years, we have been the driving force in advocating and establishing much-needed legislation in the state of Florida, including taking a leading role in the establishment of the statewide Silver Alert program and the next phase of Silver Alert, known as Lost on Foot. Alzheimer’s Community Care is so committed to the power of advocacy on behalf of our families that the Board of Directors established Advocacy as a Core Service of our organization.

EventsEvery year, Alzheimer’s
Community Care conducts a series of special events
that are intended to further educate the public about
issues related to Alzheimer’s disease and
related disorders, as well as raise the funds necessary to
further our mission.
The 19th Annual Alzheimer’s Educational Conference, held in the spring of 2016, brought
together approximately 840 caregivers and professionals for two days of discussions
and presentations.
We also conducted two wonderful fundraising events in Palm Beach
County and a third on the Treasure Coast.

Alzheimer’s Community Care is so thankful for the many donors who help support the services and resources we provide to our families. Every time we receive a donation, we
promptly respond with a “thank you” letter to the donor. We also send a separate card to the caregiver, informing them of the individual who has donated in memory of their loved one.
The beautiful card that the caregiver receives is entitled “Sunrise.” It is based on a painting by Sophia P., who attended our Specialized Day Center in Boca Raton. As it reads on the back of the card, “One night her husband, Daniel, went with her to a lake and watched the
sunrise. Daniel also took her to the top of the Sears Tower to see the sunrise. Everything is Daniel, she is so in love.”
