    Short term:After 6 hours ofnursing interventions thepatient will demonstrate

    ease in breathing.

    Long term:

    After 2-3 days of nursinginterventions the patients

    S.O will verbalizeunderstanding of thecausative factors that couldaggravate the condition andappropriate factors thatcould help the patient relivefrom gas exchange

    impairment.

    1.Monitor vital signsand assess patients

    conditions.2.Auscultate lungs for

    crackles ,consolidation and

    pleural friction rub.3.Assess LOC, distress

    and irritability.4.Observe skin color

    and capillary refill.5.Encourage rest.6.Encourage elevated

    HOB.7.Perform chest

    physiotherapy afternebulization.

    8.Administer oxygen asordered.

    1.To establish baselinedata.

    2.Determine adequacyof gas exchange

    and detect areas ofconsolidation and

    pleural friction rub.3.This signs may

    indicate hypoxia.4.Determine

    circulatoryadequacy, which isnecessary for gasexchange totissues.

    5.Rest prevents tissueoxygen demand

    and enhancestissue oxygenperfusion.

    6.To facilitate lungexpansion toenhance breathing.

    7.To dislodge thesecretions, for easyexpectoration

    8. Improves gas-exchange decreasework of breathing.

    Short term:The patientshall have demonstratedease in breathing.

    Long term:

    The patients S.O will

    verbalized understanding ofthe causative factors thatcould aggravate thecondition and appropriatefactors that could help thepatient relive from gasexchange impairment.
