Linfogranuloma venereo è una rara
variante dell’infezione da Chlamydia (L1-
L2-3)
Sono descritte vari cluster epidemici in
Europa caratterizzati da :
– Quadro clinico atipico : proctite ulcerosa
– Nella massima parte in soggetti MsM HIV+
– Frequente coinfezione HCV

scambiato per IBD o tumore retto
Diagnosi ritardata in moltissimi casi
Diagnosi possibile solo mediante PCR
disponibili in laboratori specializzati
Generalmente buona risposta alla terapia
con doxiciclina per 21 gg

La sifilide è tornata ad essere una malattia
“dimenticata” da tutti, medici compresi
L’infezione luetica in soggetti HIV positivi
differisce dai soggetti immunocompetenti
– Sia nelle manifestazioni cliniche
– Sia negli aspetti laboratoristici

HIV e SIFILIDE
La sifilide in corso di HIV presenta
caratteri sierologici particolari, talora
paradossali , legati alla disregolazione
delle funzioni delle cellule B :
– Titoli di VDRL molto elevati nella L1
– Falsi positivi più frequenti
– Falsi negativi nelle fasi avanzate di inf. da HIV
– Più lento declino titoli VDRL dopo terapia

HIV e SIFILIDE
L’infezione da HIV può presentare
variazioni immuno-virologiche durante un
episodio luetico :
– Diminuzione cellule CD4+
– Aumento della carica virale HIV
Le modifiche tendono al recupero
completo
Tali variazioni sono più intense in soggetti
non in terapia HHART e se CD4 <200 c/µl

HIV e HSV L’Herpes genitale, essendo una malattia
ulcerativa, predispone alla acquisizione e
alla trasmissione dell’infezione da HIV
(RR >7)
La riattivazione dell’infezione in un
soggetto HIV + è correlata all’ aumento di
replicazione HIV ( anche di 3 volte) per
periodi prolungati
L’HSV condiziona il set point nelle fasi
iniziali dell’infezione da HIV
