Estudio retrospectivo - 148 pacientes.
Objetivo: actividad de Paclitaxel + Erbitux (PCS) en pacientes con CECC R/M, analizando factorespronósticos de interés clínico.
64 pacientes (43,2%) recibieron además Erbitux de mantenimiento.
Resultados:
Tasa de respuesta (RR): 47,3% RC o RP - 20,3% EE - 32,4% PE.
SG 10 meses - SLP 7 meses.
Los factores pronósticos adversos significativos en el análisis univariante (AU) para SLP fueron:
- Progresión de la enfermedad, Albúmina basal de 1, IC Charlson >3, ECOG>1, intervalo libre deenfermedad (ILE) ≤20 meses y Albúmina basal <3gr/dl y Anemización durante el tratamiento.
En el análisis multivariante la presencia de progresión de la enfermedad y la disminución de Magnesiodurante el tratamiento mayor de 10% resultaron significativos para SG.
Conclusiones:
Se confirma la actividad de PCS en pacientes RM-CECC no seleccionados, similar a los estudiosprevios, incluso con mejores resultados, y comparable al tratamiento basado en platino y Cetuximab.

Introducción y Objetivos: La combinación de agentes quimioterápicos basada en platinos con terapia molecular dirigida como el Cetuximab constituye el tratamiento estándar de primera línea en enfermedad metastásica y/o recurrente en cáncer de cabeza y cuello, en pacientes con buen performance status (PS). Analizamos la efectividad y seguridad del esquema Cetuximab-Docetaxel-Cisplatino en pacientes con cáncer de cabeza y cuello en recurrencia y/o enfermedad metastásica. Se analizó el intervalo libre de progresión (ILP) y el impacto en la supervivencia global (OS). Material y Métodos: Análisis retrospectivo, incluyó 18 pacientes con cáncer de cabeza y cuello tratados en el servicio de Oncología Médica del Complejo Asistencial Universitario de León, desde enero 2010 hasta enero 2016. Se analizó la efectividad y tolerancia del esquema Cetuximab-Docetaxel- Cisplatino, en práctica clínica habitual, el ILP y la OS. Resultados: Se incluyeron 18 pacientes. 88,8% varones. Edad mediana 56 años. El PS fue 1 para todos los pacientes. 88,9% eran carcinomas epidermoides. Grado histológico: G3 50%, G2 27,8%, G1 16,7%. 77,8% tuvieron recidiva locoregional y 72.2% a distancia, 84,7% a nivel pulmonar y 38,6% a nivel ganglionar. Se utilizó el esquema Cetuximab 500mg/m2- Docetaxel 50mg/m2 -Cisplatino 50mg/m2 cada 15 días. El 88,9% tuvieron algún grado de toxicidad, presentando toxicidad G3 22,4%. La toxicidad más frecuente fue la cutánea, presente en 87,5%. La tasa de respuestas objetivas fue de 88,9%, 50% respuesta parcial, 38,9% estabilización. El ILP fue de 9 meses (IC 95% 5-12)y la OS de 10 meses.
Conclusión: En nuestra experiencia con este esquema de Cetuximab-Docetaxel-Cisplatino quincenal hemos demostrado efectividad, con un perfil de toxicidad esperable y manejable. Nuestros datos están en consonancia con el estudio fase III (EXTREME) de Cetuximab-Cisplatino-5FU, en pacientes con buen PS.

Introducción y Objetivos: El cáncer de cabeza y cuello tiene generalmente un pronóstico pobre. La combinación de un agente quimioterápico como los taxanos y terapia molecular dirigida como el Cetuximab, ha demostrado efectividad en los pacientes con recurrencia o enfermedad metastásica. Analizamos la efectividad y seguridad del esquema Cetuximab- Docetaxel en pacientes con estado general intermedio y cáncer de cabeza y cuello tras recidiva y/o enfermedad metastásica. Se analizó el intervalo libre de progresión (ILP) y el impacto en la supervivencia global (OS).
Material y Métodos: Análisis retrospectivo, incluyó 27 pacientes con cáncer de cabeza y cuello tratados en el Servicio de Oncología Médica del Complejo Asistencial Universitario de León, desde enero 2010 hasta enero 2016. Se analizó la efectividad y seguridad del esquema Cetuximab-Docetaxel como práctica habitual en nuestro servicio.
Resultados: Se incluyeron 27 pacientes. 81,5% varones. Edad mediana de 63 años. El PS fue 2 para todos los pacientes. 92,6% eran carcinoma epidermoide. Grado histológico: G3 33,3%, G2 37%, G1 29,6%. 77,8% con recidiva locoregional y 40,7% a distancia, 90% a nivel pulmonar, 36,3% a nivel ganglionar. Se utilizó el esquema Cetuximab 500mg/m2-Docetaxel 50mg/ m2 cada 15 días. 74,1% tuvieron algún grado de toxicidad. Presentaron toxicidad G3 25%. 90% presentaron algún grado de toxicidad cutánea, siendo esta la más frecuente. La tasa de respuestas objetivas fue del 77,8%, 29,6% presentaron respuesta parcial y 48,1% estabilización de la enfermedad. El ILP fue de 7 meses (IC 95%: 5,6-8,2) y la OS de 11 meses.
Conclusión: En nuestra experiencia con este esquema de Cetuximab-Docetaxel quincenal hemos demostrado efectividad, con un perfil de toxicidad esperable y manejable. Nuestros datos están en consonancia con el estudio fase II de Cetuximab-Paclitaxel semanal, en paciente de estado general intermedio.
