Select Question Type: based on different visual representations of responses to be captured –Mind Node: option to select from multiple answers represented in interconnected circles.Box Form: option to select from multiple answers represented in squares.Toggle: option to select from multiple answers represented in rectangles.Likert Scale: option to select from multiple answers represented as SmileysMatrix: allows you to capture multiple items under one main question

Select Next to move to next screen

Select Add Question to add more question
