LP Warrant Claim Home Built Before 1995

Preparations

Please have the following available before you

get started on your claim:

• Black or blue ink pen

• Piece of paper to write notes on

Previous Claims

• If you have never filed a LP claim before then check no and move on to the next slide.

• If you have filed a LP claim before then write down the year that you filed and how much you

received and also check original settlement for the payment program.

A: Property Address

• For property address fill in your street address along with the city, state, and zip code

• For #1 on property owners fill in your name and information and for line 2 & 3 add any

additional homeowners.

B: Questions About Your Property

• Check new or previously owned when you bought your home, and provide the month and year

that you bought it

• Skip the next two boxes and fill in one for the number of buildings

• Check single family dwelling for type of structures

• Check siding and trim for type of LP product

C, D, & E

C: You need to provide proof of property ownership by attaching the following documents to your

claim: warranty deed, current tax bill, and a current utility bill.

D: For proof of LP siding prove a picture of the LP knot. (picture of knot is on next slide)

E: They are asking when the LP was installed on your home and that would be the year your home

was built. You also need proof of instillation and the easiest document for you to obtain is a

Certificate of Occupancy. If you do not want to get this document yourself you can call us and

we can receive it for you for $7.50.

F: I’m going to give you some information and it is your choice to use it or not. If you put that you

painted your home 5 or 6 years ago then you will be fine. If you put that you have painted your

home longer than 6 years ago then you will get hit with paint deductions, meaning they will take

money from your claim. They do not ask for any proof of when you painted either so it is up to

you what you want to put as your answer.

G: Leave blank if you have never filed before

H: Direction to property

You need to write out directions from the nearest highway or town to your home in the space

provided

Check yes that you want to be present when the state inspector comes out for your inspection.

List any obsticles that the inspector might have when he is inspecting your home

I: Assistance with the Form

I you paid your $50 upfront fee for us to start your warrenty claim process then leave this section

blank

If you did not pay the $50 up front fee and you chose that you wante a percentage taken off the

back end then fill in Section I with the following Information:

Your claim is now complete. All you have to do is

seal everything up in an envelope and send it

off!!
