Eight adult survivorship prototype sites (14 tumour specific teams) and ten children and young peoples test sites have worked with us to develop and test stratified pathways of care. The poster submissions included in this publication were presented at the NCSI conference event on 28 March 2012 and represent only some of the learning and the excellent work from the teams and sites participating in this programme. The National Cancer Survivorship Initiative (NCSI) waslaunched in September 2008 and is a partnership betweenthe Department of Health, Macmillan Cancer Support andNHS Improvement. The aim of the national initiative is toimprove models of aftercare provided for those patientssurviving cancer, many of whom will need a range ofclinical and non-clinical services to enable them to live ahappy, healthy normal life after their initial treatment forcancer.

Eight adult survivorship prototype sites (14 tumour specificteams) and ten children and young peoples test sites haveworked with us to develop and test stratified pathways ofcare that are tailored to individual needs with the focus onproviding the right support in the right place by the rightperson and encouraging self management whereappropriate to do so.

The poster submissions included in this publication werepresented at the NCSI conference event on 28 March 2012and represent only some of the learning and the excellentwork from the teams and sites participating in thisprogramme. They are presented in two sections Adult andChildren and Young People with the winner and highlycommended entries at the start of each chapter.

Delegates on the day were then given the opportunity tonominate the winning posters. The winners were thenpresented with an award and certificate by Professor SirMike Richards to congratulate them on their winningposter design.

National Cancer Survivorship Initiative: Livingwith and beyond cancer - Adult PathwayOver the past four years work has commenced with patients, NHS clinical teams and third sectororganisations to test care and support for those living with and beyond cancer. This work has culminated inthe development and pilot testing of pathways of care tailored to individual needs in four tumour types(breast, colorectal, lung, prostate) with the aim of improving quality, reducing unnecessary hospitalattendances and moving the focus of post treatment care from the disease and treatment to health andwellbeing.

This interactive generic pathway demonstrating examples of care and support from testing work and alsosignposting to other relevant sources of information.

CYP TEST SITEPOSTERSNational Cancer Survivorship Initiative: Teenageand Adult Aftercare PathwayThese Children and Young People Cancer Survivorship aftercare pathways have been produced andendorsed as clinical good practice following an extensive consultation process with input from clinicalexperts and cancers survivors.

These defined pathways demonstrate the evidence emerging from three years of testing and prototypingwith clinical teams and cancer patient survivors.
