El Dilema de la Curva en J

En falta de ensayos especificamente diseñados, se han relacionados los eventos ocurridos con las presiones alcanzadas (análisis post-hoc).
Curva en J negada : HOT, PROGRESS, UKPDS Curva en J observada : IDNT, INVEST, ONTARGET, LIFE Curva en J observada también en pacientes con tratamiento placebo:

Limites de los Análisis Post-Hoc de la Curva en J

1. La aleatorización está perdida. 2. En el grupo en el que la PA con el tratamiento es más baja
los números de individuos y de eventos son muy escasos. 3. En el mismo grupo podía haber habido un riesgo
cardiovascular inicial mayor que causara tanto la reducción excesiva de la PA como el aumento de la incidencia de eventos cardiovasculares.
4. El nadir (el punto más bajo) de la curva en J varió mucho
en ensayos diferentes.
Necesidades Urgentes

Es urgente aprender hasta cual valor de PAS y PAD una terapia más intensa se acompaña a una mejor protección cardiovascular, y si este valor de PA es el mismo en grupos diferentes de pacientes. ESH ha diseñado un ensayo aleatorizado (ESH-CHL-SHOT) que va a iniciar pronto en colaboración con la CHL
Problemas que van a ser investigados en pacientes con un reciente accidente cerebrovascular o TIA : 1. ¿Cuánto menor la PA tanto mejor la prevención secundaria de la
repetición de un accidente cerebrovascular; o con una redución más intensa de la PA hay una curva en J ?
2. ¿Cuánto menor la PA tanto mejor también la prevención de otros
eventos cardiovasculares? 3. ¿Cuánto menor el LDL-C tanto mejor la prevención secundaria de
accidentes cerebrovasculares? 4. ¿La reducción intensa sea de la PA sea del LDL-C representa el
tratamiento optimal para la prevención de la repetición de accidente cerebrovascular, de otros eventos cardiovasculares, y del declino cognitivo siguiente a un accidente cerebrovascular?

Conclusiones
Unos recientes análisis de tres ensayos clínicos (FEVER, SCOPE y VALUE) han comparado pacientes que responden más o menos a la terapia antihipertensiva y han encontrado que los pacientes que responden menos a la terapia y necesitan más medicamentos están a un riesgo más alto de accidentes cardiovasculares, y traen menores beneficios de la redución de la presión arterial. Esta observación significa que un riesgo cardiovascular alto no entrega necesariamente un beneficio más ancho de la terapia antihipertensiva.

En la historia natural de la hipertensión la intervención terapéutica tiene que iniciar antes la aparición del daño de los órganos blancos, o antes que el daño se vuelve irreversible.
La hipertensión nunca nace resistente. Se vuelve resistente cuando no es tratada o es tratada insuficientemente.
Cuanto antes tanto mejor
