CONTEXT How and where the survey is conducted means as much as the questions asked.
Know whats important
If you want to know what participation is like, dont ask what a person has ever done, ask what have they done lately - much more interesting.
Indicate which community activities you have done in the last twelve months.

quite a few followup qustions - dont know why they participated, followup survey is needed to try and figure out why people participate.
Test your assumptions
Surveys are a collaboration between the person who wrote the survey and the person taking it.Each question says as much about the person who wrote the survey, as the answer says about the person who is taking it.
Have you ever been discriminated against?- Yes- Noassumptions: youre in a party, these two parties are the most important ones, parties can address your political beliefs
