Osteoporosis: skeletal disorder characterized by compromised bone strength predisposing a person to an increased risk of fracture
NIH Consensus Development Panel on Osteoporosis: prevention, diagnosis and therapy 2001
o OsteoporosI: T-score < -2.5 DS
Diagnosi densitometrica
Farmaco Azione
(Calcio, Vitamina D)
Bisfosfonati anticatabolica
SERM’s anticatabolica
Denosumab anticatabolica (anti-RANKL)
Teriparatide anabolica
Ranelato di stronzio mista
Armamentario terapeutico
Questioni aperte: o è sufficiente fare diagnosi? (meglio prevenire)
o è sufficiente utilizzare la BMD in diagnosi? (molte diagnosi mancate)
o definire linee terapeutiche (anticatabolica, anabolica, entrambe)
o aumentare aderenza al trattamento

effective prevention and treatment
o at baseline
o at 3-6 month intervals
o 20-40% reduction for bone formation markers such as osteocalcin and bone alkaline phosphatase

Monitoring therapeutic response with bone turnover markers
Risultati:

Current Projects o Review literature and current status of available assays in order to
develop and undertake a project to establish a reference measurement system for serum β-CTx and serum P1NP or harmonisation of the assays for serum β-CTx and serum P1NP as appropriate
o Review literature and consider the critical decision limits and potential target levels of serum β-CTx and serum P1NP for treatment of postmenopausal osteoporosis and other causes of osteoporosis as appropriate

As the nature of bone turnover marker response is determined by the mechanism of action of the drug
Research priority: to identify the most promising treatment–test combinations
Systematic review of the use of bone turnover markers for monitoring the response to osteoporosis treatment
Vantaggi:
o Non invasivi
o Non costosi
o Buone prestazioni analitiche
o Possono essere ripetuti spesso
o Variano in tempi brevi
o Derivano sia da osso
trabecolare che corticale
o Riflettono l’attività metabolica di
tutto lo scheletro
Svantaggi:
o Non riflettono il processo di
mineralizzazione
o Alta variabilità biologica
o Risentono dei processi di
eliminazione
o Alterati dalle fratture
