CASE NARRATIVE
SAMPLE RECEIPT: Samples were received intact, at 22°C and with chain of custody documentation.
HOLDING TIMES: All samples were analyzed within prescribed holding times and/or in accordance with the TestAmerica
Sample Acceptance Policy unless otherwise noted in the report.
PRESERVATION: Samples requiring preservation were verified prior to sample analysis. Results were qualified where the
sample container did not meet the method preservation requirements.
QA/QC CRITERIA: All analyses met method criteria, except as noted in the report with data qualifiers.
COMMENTS: Results that fall between the MDL and RL are 'J' flagged. The samples were prepared in accordance with
the EPA Method 1311 leaching procedure described for TCLP with the exception that the extraction fluid
used was deionized water instead of the acetic acid solution described in the method.

Identification and Definition of Problem:
LCS and LCSD recoveries for Dieldrin, Endrin, Endosulfan II, Endrin Aldehyde, and Methoxychlor were below
laboratory control limits.
Determination of the Cause of the Problem:
The LCS and LCSD were incorrectly run through a clean-up process before analysis.
Corrective Action Taken:
Although only the LCS and LCSD were processed incorrectly, there is no positive quality control for these
compounds. The blank was processed correctly and indicates no positive bias. The surrogate recoveries for all
samples were within acceptance limits and indicate no excessive losses of sample during extraction and analysis.
There was not sufficient sample available for reanalysis within holding time. Results have been qualified to
reference this corrective action report. All extraction personnel involved have been retrained on correct procedures.
