WAIVER AND RELEASE OF ALL CLAIMS AND PERMISSION TO SECURE TREATMENT** Please read this form carefully and be aware in registering yourself or your minor child(ren)/wards for participation in any programs of the Bourbonnais Township Park District, you will be waiving and releasing all claims for injuries you or your child(ren)/wards might sustain arising therefrom and also giving permission to secure treatment for any said injuries. I recognize and acknowledge that there are certain risks of physical injury to participants in the programs, and I agree to assume the sole and full risk of any injuries, damages or losses regardless of severity which I or my minor child(ren)/wards may sustain as a result of participating in any and all activities connected with or associated with any such programs. I agree to waive and relinquish any and all claims I, or my minor child(ren)/wards, may have against the Bourbonnais Township Park District and its officers, agents, servants and employees as a result of participating in the programs.
