I therefore hereby authorize you, without reference to me, to pay all amounts in

me in respect of any such invoices. For avoidance of doubt each invoice

or any subsequent or replacement Card issued in my favour, for all invoices

of my Card, at least 45 days prior to the Expiry Date of the Card or any

replacement of my Card, immediately on such replacement.

Jazz Number (“JAZZ Credit Limit”) and I also understand that JAZZ shall also be

replacement Card issued in my favour, amounts directly from you as and when the JAZZ Credit Limit is about to be reached. You are also hereby authorized to make payment of all such invoices.

I undertake that in the event of any dispute on invoices raised by JAZZ, I shall

To use the facility of making payment of your JAZZ bills directly through your Credit ChargeCard (“Card”) issued by an institution acceptable to Pakistan Mobile Communications Limited(“JAZZ”). Kindly fill out the information given below and execute this letter in the presence oftwo witnesses (two men or one man and two women). Upon completion of the instructionsand subsequent approval by JAZZ Credit Department, your Card will be automatically billedfor amounts due from you to JAZZ.

I refer to the direct debit payment facility introduced by Pakistan Mobile Communications Limited (“JAZZ”) for payments of my Phone bills through Direct Debit/charge to my Credit Card and hereby consent and agree to participate in the said facility. I confirm that, I am not in default of any payment due in respect of the Credit Card.

I authorize JAZZ to directly debit/charge through my Credit Card to recover all amounts payable by me, and to receive the payments through such direct debit/charge in satisfaction of the amounts billed to my subscription amount. I confirm that the authorized credit limit for my Credit Card exceeds the credit limit allowed by JAZZ for my subscription account.

I confirm that the arrangement stipulated in this Letter shall remain in force until such time as it is revoked by me by written notice of one month’s duration. The revocation of this agreement shall become effective on month after the date of notice and any debit to my Credit Card during the one month notice period shall be binding on me. I request JAZZ to continue to recover amounts due from me under the Agreement through my Credit Card even after the Expiry Date of my Card, I shall notify JAZZ in writing of the extension/renewal of my Card, at least forty five (45) days prior to such Expiry Date or any subsequent Expiry Date, failing to which JAZZ shall be entitled to suspend or terminate telecommunication services being provided to me. However failure to notify shall not prevent JAZZ from continuing to recover payments due from me through the procedure set out herein, if the Card is in fact renewed/extended.

JAZZ is hereby further irrevocably authorized and empowered by me to debit the Credit Card with any amount that may be payable by me to JAZZ during/after a month in case my credit limit has been exceeded to avoid blocking of service without any reference to or requirement of any further authority from me.

I further accept that in case any debit instructions by JAZZ are declined by my credit card issuing bank due to any reason, I shall forthwith make payment of my dues to JAZZ through alternate means. Any dispute between me and my credit card issuing bank shall not be a ground for me to delay or refuse payment or my dues to JAZZ. I further confirm, accept and acknowledge that any dispute between me and JAZZ in respect of bills generated by JAZZ shall be adjusted on a prospective basis in future bills and I shall not be entitled to any cash refunds for any amount erroneously billed and debited to my Credit Card.

I confirm that the information provided herein is true to the best of my knowledge as of the date hereof and further undertake that I shall forthwith inform JAZZ of any change in the information and particulars set out herein and shall indemnify JAZZ for any loss or damage suffered by JAZZ as a result of any information or particulars found to be incorrect or inaccurate or ceasing to remain correct and accurate.
