Part 2: Continued - Selection of Targets
On the following 2 pages is a list of target items that contributed to risk for the five dynamic domains that scored “high” risk for Kris. (Note: In CaseWorks, this list is available for each domain by clicking on the red target buttons on the YASI wheel.)
First, identify whether the domain was selected as a priority in the last step by placing thenumber (1, 2, or 3) in the circle.
Then look at the items (for priority domains 1, 2, 3 only) and place a check mark in the boxesnext to the items that you want to help Kris address as part of the case plan.
In the space provided, identify when Kris has strengths in any of the domains.
