Important Information

This booklet is a summary only. The benefits aresubject to certain exclusions and policy conditionsnot stated in this booklet which in the event of aclaim will be applicable. The benefits are providedstrictly under the terms of the insurance policiestaken out by the trustees of the scheme.Subscription to the scheme entitles the member tothe benefits provided by the scheme but confers noownership of any of the underlying policies whichare vested in the trustees. The trustees retain thefull policy wording detailing all benefits andexclusions which may be viewed on request to theFederation Office.Applying to JoinServing officers can apply to join the scheme at anytime by completing a medical underwritingapplication form which is available from theFederation Office. The first three monthlysubscriptions are free of charge when joining thescheme for the first time. New probationary recruitsto the police service may join the scheme without theneed for the completion of the medical underwritingapplication form and their first 52 weeks of serviceare free of charge. The Federation and/or PhilipWilliams & Co reserve the right to decline anyapplications.Cohabiting Partner ExtensionsAny cohabiting partner extension will cease whenthe member or cohabiting partner reach 70 years ofage whichever is the sooner. Any cohabiting partnercover and/or extension will cease when the memberceases to be a member of the scheme.Career breaks, maternity leave,secondment or living overseasThose going on a career break, maternity leave,secondment or are living overseas must contact theFederation Office to identify if cover can be maintained.Retirement from the Police ServiceServing officers upon retirement from WestYorkshire Police may remain in the scheme as aretired member by submitting a completedmembership continuation form to the FederationOffice prior to retirement. Individuals are not eligibleto join the scheme after their retirement date.Transfer, resignation or dismissalServing officers who transfer, resign or are dismissedfrom the police service are not eligible to remain inthe scheme and all membership and benefits willcease including any cohabiting partner extension.

Subscription collectionSubscriptions are collected monthly by deductiondirect from salary/pension unless that is notpossible when alternative arrangements may beagreed. Monthly subscription payments must bemaintained in order to remain a member of thescheme and to qualify for any benefits.InsurersA list of the insurers is available on request. Fullpolicy wordings for certain policy sections areavailable to download via QR codes. These may alsobe available at the Federation Office, on theFederation Website or by visiting the Group Schemesection of www.philipwilliams.co.ukHow to cancel your coverIn the event that you need to cancel your coverplease contact the Federation Office.Complaints ProcedureThe Federation Insurance Scheme is arranged onbehalf of the trustees by Philip Williams andCompany who are authorised and regulated by theFinancial Conduct Authority (Registration Number308860). The trustees are responsible fororganising the policies and dealing with theinsurance broker. Any complaints about any aspectof the scheme should in the first instance bedirected to the Federation Office. 4 Police Federation Group Insurance Scheme

Life InsuranceOn death of a member or spouse/partner who arecovered under the scheme the cash benefit detailedin the current benefits table will be paid. The policyis written in Trust so that if a member dies theproceeds can be paid by the Trustees to themember's beneficiaries quickly, free of tax andwithout having to wait for probate. Members shouldensure that they have an up-to-date beneficiarynominated and have lodged the details with theFederation Office in order to assist the Trustees in theevent of a claim. If a member or covered partneraged 63 or under receives a terminal prognosis of 12months or less, they may apply to the Trustees for anadvance of the death benefit of 20% of the relevantsum insured.Child Death GrantThis benefit is paid upon the death of a dependentchild of a serving member, aged between 6 monthsand 17 years.Permanent Total DisablementPayable when a serving member or covered partneris unable to perform any gainful employment and isunable to exist independently requiring continualsupervision for activities of daily living as a result ofan accident.

Accidental Loss of Useand Permanent Disabling Injuries BenefitVariable Benefits paid in the event of:-- Loss of sight in one or both eyes- Loss of use of one or more limbs- Total loss of hearing in one or both ears- Total loss of speech- Total loss of use of:- a) back or spine (excluding cervical) without cord involvement b) neck or cervical spine without cord involvement c) shoulder, elbow or wrist d) hip, knee or ankle- Loss of or total loss of use of:- a) foot below the level of the ankle (talofibular joint) b) thumb c) one forefinger or big toe d) any other finger e) any other toeHospitalisation BenefitIf an accident directly results in a serving memberor covered partner making an immediate overnightstay in hospital (being admitted as an in-patientwith the stay extending from at least midnight to07:00 a.m. for each night claimed).

Temporary Total Disablement(including Post Traumatic Stress Disorder)Cover is provided for serving members only. If anaccident, occurring on or off duty, is the sole causeand directly results within the subsequent two yearsin the member being unable to perform their policeduties and being signed-off sick then a temporarytotal disablement benefit may be payable. Thisbenefit will also be paid to members diagnosed withsuffering from Post Traumatic Stress Disorder (PTSD)as a result of attending an incident on police duties.Such PTSD must be the clear result of an incidentthat occurred within the two years prior to thediagnosis, was documented in the police records,and is of sufficient severity to prevent theperformance of duties. The benefit will be paid for amaximum of 104 weeks excluding the first 7 days.Benefit ceases on return to duty ordischarge/retirement from the police service.Sick Pay Benefit (Regulation 28)If a member suffers a reduction in pay under PoliceRegulations or terms of employment, the benefits asshown in the policy will become payable. Benefitsunder this section for Police Staff are shown in thefull policy wording. The benefit ceases on return towork or if the member retires, resigns, is dischargedfrom the police service, or fails to pay the monthlyscheme subscription. In addition the benefit shall notbe payable if the member has been offeredreasonable adjusted duties with a return to full payand has declined such duties without reasonablecause. Any overpayment due to a reversal in thedecision by West Yorkshire Police which results in aresumption of pay (including any back payment) ordue to a failure by the member to inform the insurersof a return to work must be repaid in a prompt andtimely manner.Unrecovered Criminal CourtCompensationCover is provided for serving officers only. If, as aresult of an on-duty assault, compensation you havebeen awarded by the Court has not been paid withinsix months from the payment date of the award abenefit payment will be made.

Unsociable Hours BenefitIf a serving officer sustains bodily injury following anaccident or contracts sickness resulting in totaldisablement entirely preventing that Insured Personfrom engaging in or giving attention to his/her usualoccupation We will pay a benefit equal to 7.5% ofthe Insured Person’s basic Scale Pay hourly ratewhile the Insured Person is unable to work theirUnsociable Hours that had been scheduled prior tothe commencement of disablement as recorded inPolice Force records.

We will not pay for scheduled Unsociable Hours forthe first 14 days of each period of disablement. Themaximum benefit We will pay in respect of thisextension is:Constables: £60 per weekSergeants: £75 per weekInspectors and Chief Inspectors: £95 per weekAssault BenefitThis benefit is payable where a member whilst onpolice duty suffers an assault with a firearm, knife orother sharp instrument which results in actualphysical injury which renders the member unfit forduty for a period of three consecutive days or more.Casting BenefitThis benefit is payable in the event of a Membertaking compulsory retirement on ill health pension asa result of an accident. No Casting Benefit is payableif a Member has accrued more than 10 years ofservice. Years of service means the number ofreckonable pensionable service attained by themember at the date of casting from the policeservice.Accidental Damage to TeethIf following an accident you damage otherwise soundand natural teeth, up to £250 towards costs willbecome payable.

The scheme benefits will be payable if a serving member, a dependent child aged from 6months to 17 years, or a subscribing partner, suffers from an insured illness and survives formore than 28 days from the date of diagnosis or surgery.

Policy Number MT 14/1396This policy covers the me mber, their cohabitingpartner and their unmarried dependent childrenunder 18 years (or 21 years if in full timeeducation) all normally resident in the familyhome, for any number of trips in any year up to60 days per trip. Main Conditions and ExclusionsCover under this policy ceases at age 70. Forsubscribing members who are aged under 70years, partners are covered up to age 74 years.

The policy contains an important warranty andexclusions relating to existing health conditions. An excess of £50 is applicable for most policysections. The excess is payable per person, persection, per insured incident subject to amaximum of £100.

This policy provides for you and your partner’smobile phone(s) to be covered against the repairor replacement cost in the event of theft,accidental loss, accidental damage, orbreakdown whilst in you or your partner’spossession during the period of insurance.

Pre-registration of mobile phones is notrequired, but ownership and age of the phonewill need to be evidenced by the relevant proofof purchase.

Claim limit(s)An excess of £50 applies for each and everyclaim.

The liability of the insurer in respect of any oneclaim and in aggregate for any 12 month periodwill be the replacement cost of the mobile phoneand in any event shall not exceed a total claimcost up to a maximum of £1000 including VAT.

The maximum liability limit includes cover of upto £750 including VAT for unauthorised datausage for a period of up to 24 hour directlyfollowing a valid theft or accidental loss.

Claims Procedures

Claims should be reported as soon as is practical and if possible within 30 days of theincident/diagnosis. If claims are not reported within 90 days of the date of the incident/diagnosisthey may not be met. The onus is on the member to notify a claim and it is not the responsibility ofWest Yorkshire Police Federation or Philip Williams & Company to make a member aware of theirentitlement to claim.
