On-Call Physician Our practice provides 24 hour coverage through an answering service when the office is closed. If you have an emergency, call 9-1-1. If you need to speak with a physician during non-office hours, the answering service will page the physician who is covering and that physician will call you back.

Scheduling AppointmentsWe do our best to schedule appointment times that are convenient for you. Our receptionists will schedule your appointment with the appropriate doctor.

We save a limited number of same-day appointments for patients who have urgent issues.

To schedule an appointment, call 508-336-9200 or 508-222-5511.

Phone InformationOur main phone number: 508-336-9200.

Our phones are forwarded to an answering service at lunchtime each day between 12:00pm and 1:00pm.

Our phones are always answered by a receptionist. If you require medical advice, they will forward a phone message to your provider. The provider will review the message and either they or their Medical Assistant will return your call as soon as they are available.

Let Us Know How We Can Do Better! We strive to continually improve the service to our patients. We welcome your feedback through patient satisfaction surveys that we mail to our patients. We encourage you to fill them out when you receive them. You can also contact our Practice Manager to discuss any issues or concerns you may have.

We appreciate that you have chosen us to provide your health care. Thank you!

What To Bring To Your AppointmentAll patients should arrive 10 minutes early to check in at the front desk. We will ask for your insurance card each time you come, so we are absolutely sure we have your current insurance information. We will also verify your home address and contact information, and collect any co-pay due. A photo ID is required.

ReferralsMany health insurance companies require a referral from your primary care physician before seeing a specialist. If your insurance requires a referral, you must obtain it prior to your scheduled appointment. Please provide 72 hours notice for all referral requests.

Billing ProceduresWe submit billing claims to your insurance company within a day or so of your visit. This is why we ask to see your insurance card each time you see the doctor. Most insurance companies send payment and an itemization of any patient co-pays, co-insurance, or deductibles within a month. We then send you an invoice for these items, for which we expect prompt payment.

We recognize that some health expenses are unexpected. If you cannot pay the full amount at once, please be sure to call our billing office to set up a payment plan.

We also recognize that health insurance can be confusing. Our billing staff is available to help you with any questions.

Cell Phone PolicyWe ask that you please put your phone on vibrate when you enter the building.

Our Providers

To schedule an appointment, call 508-336-9200 or 508-222-5511

Appointment Notification If you cannot keep an appointment, we ask that you please call as soon as possible, preferably at least 24 hours before your appointment so we may reschedule your visit. Failure to do so may result in a broken appointment fee of twenty-five dollars. Repeatedly missed appointments may result in discharge from the practice. Providing us adequate notice gives us the opportunity to best serve all of our patients by being able to offer last minute appointments to those who may be sick.
