Nutrition Indicators Nutrition is influenced by several indicators relating to health, water, sanitation,

hygiene practices, child marriage, education of girls. Indicators relating to health andnutrition are generally referred to as nutrition specific indicators, while other indicatorsare referred to as nutrition sensitive indicators.

Underweight, stunting and wasting among children

In India, the pace of reduction in malnutrition indicators is slow. Stunting of childrenunder 5 years declined from 45.5% in 1998-99 to 38.4% in 2015-16, while wastingincreased from 15.5% to 21% during the same period. However, steady decline inunderweight is recorded- from 47% to 35-8% during 1998-2016 (NFHS 2,3 &4).

Anaemia India has high prevalence of anaemia, across all age groups. About 58.6% of young

children, 54.1% of adolescent girls and 53.1% of women in India are anaemic. Anaemiaamong girls and women would lead to malnutrition of children.

BMI of women and adolescent girls There is a strong correlation between child malnutrition and the Body Mass Index

(BMI) of adolescent girls and women. In a vicious cycle, babies born to adolescent girlsare at an increased risk of being born too early or with a low birth weight. Thesechildren are at risk of growing up physically and cognitively stunted and are at risk ofcarrying this burden into adulthood, leading to the inter-generational cycle ofmalnutrition. Women in India have BMI less than the standard 18.5, while 20.6%women are obese with BMI more than 25. In the case of adolescent girls, obesity isnegligible (4.2%), but 41.9% have very low BMI.

Causes of malnutrition (based on UNICEF Framework, 1998)Sub-optimal breastfeeding and complementary feeding practices and child-carepractices Inadequate or inappropriate dietary intake including micronutrients Diseases and worm infestation among children Maternal under-nutrition, maternal/adolescent anaemia and low BMI, resultingfoetal growth restriction, low birth weight and pre-term delivery Unsafe water, poor sanitation, poor hygiene practices including handwashing withsoap at critical timesPoor access to health facilities Social determinants that decide the nutritional status are - level of maternaleducation, child marriage of girls before 18 years, women empowerment, socialprotection, food security, maternal mental health, and family planning

How to Prevent Malnutrition?Improve breastfeeding practices and complimentary feedingThree feeding practices - breastfeeding the baby within one hour of birth; exclusive

breastfeeding for the first 6 months of birth; and beginning the complementary feedingafter 6 months with continued breastfeeding until 2 years - can prevent childmalnutrition and child deaths. Currently, in India, these indicators are at 42%, 55% and43% respectively. Poshan Abhiyan promotes these practices. Breastfeeding within onehour of birth can reduce 22% of neo-natal mortality (Ghana study 2016) and optimalbreastfeeding practices can prevents 13% of under-5 child mortality (Lancet 2003). Italso reduces diseases and malnutrition among children.

Improve maternal nutrition and health check-up Maternal undernutrition, maternal stunting and low maternal BMI affects the fetal

growth of the baby, which in turn could lead to low birth weight and malnutrition of thechild. Maternal malnutrition can be addressed by improved diet during delivery. Amother also needs supplementation of zinc, Vitamin-A, folate and calcium.

Ensure rest, care & health checkup of pregnant women- A pregnant mother needsrest, care and at least 4 Ante-Natal Check-up (ANC). She should take 2 DT injectionand take 180 Iron tablets during pregnancy. Mothers who get full ANC is only 21% inIndia (NFHS-4). Pre-conception care is also critical, which also prevents pre-termbirths.

Reduce prevalence of pre-term delivery and low birth weight babiesInfants born pre-term (before 37 weeks of pregnancy) and are of Small for

Gestational Age (SGA) are at higher risk for neonatal mortality (Lancet, 2013). Pre-termbabies in addition to being at a higher risk of neonatal mortality, are at an increased riskof post-neonatal mortality, stunting, and long-term neuro-developmental impairmentduring childhood and in lifetime. Major causes of pre-term births are: a) adolescentpregnancy, b) advanced maternal age, c) poor pregnancy spacing, d) multiplepregnancy with increased rates of twin and higher order pregnancies, e) infection likeurinary tract infection, malaria among mothers, f) undernutrition, obesity and anaemiaamong mothers) diabetes, hypertension, asthma, thyroid among women, g) smoking, use of drugs and excess alcohol consumption by mothers, and h) depression, stressand poor maternal mental health.

Foetal Growth Restriction (FGR), due to maternal undernutrition, is attributed to 12%of child deaths. It also accounts for about 20% of childhood stunting, and low psycho-motor development of children (Lancet 2013). Low birth babies (LBW) with less than2500 grams at birth, prone to undernutrition and are 40 times more likely to die.

Ensure full immunization and Vitamin-A supplementation Before the first birthday (at birth, 6 weeks, 10 weeks, 14 weeks and 9 months),

children must be given immunization 5 times, to protect them from 14 diseases anddisorders. As child diseases are the major cause of malnutrition, full immunization ofchildren can prevent malnutrition. In India, only 62% of the children are fully immunized(NFHS-4). Vitamin-A supplementation is to be given to children from 9th month alongwith measles vaccination up to 5 years with 9 vaccinations. The recent Cochranereview of 2017 highlights that Vitamin-A reduces overall risk of death by 12%. TheCopenhagen Consensus (2008) says that provision of micronutrients is the second-best opportunity that generates maximum rate of return. In India, Vitamin-A coverageof children under 5 years, is only 60%, although Vitamin-A dose is given in campaignmode, twice in a year.

Prevent/ treat childhood diseases, especially pneumonia, diarrhoea and worminfestation

Pneumonia, diarrhoea and malaria together cause more than one third (36% of

ancestors of ours whoestablished Setubandha(Rameshwar) in the South,Jagannath in the East andHardwar in the North as placesof pilgrimage? You will admitthey were no fools. They knewthat worship of God could havebeen performed just as well athome. They taught us that thosewhose hearts were aglow withrighteousness had the Gangesin their own homes. But they sawthat India was one undividedland so made by nature. They, therefore, argued that it must beone nation. Arguing thus, theyestablished holy places invarious parts of India, and firedthe people with an idea ofnationality in a manner unknownin other parts of the world. Andwe Indians are one as no twoEnglishmen are. Only you and Iand others who considerourselves civilized and superiorpersons imagine that we aremany nations. It was after theadvent of railways that we beganto believe in distinctions, and youare at liberty now to say that it isthrough the railways that we arebeginning to abolish thosedistinctions. An opium-eater mayargue the advantage of opium-eating from the fact that hebegan to understand the evil ofthe opium habit after havingeaten it. I would ask you toconsider well what I had said onthe railways. Q41. According to the author,

under-five child deaths in India). Thesediseases can be prevented and treated, thereby prevent death and malnutritionamong children. Children with diarrhoeashould be provided ORS and Zinc.Handwashing with soap at critical times(before eating food and after going totoilet) can prevent about 50% of thediarrhoeal diseases and 25% ofrespiratory diseases including pneumoniaamong children (Curtis and Cairncross,2003). Babies who are exclusivelybreastfed for the first 6 months are 11times less likely to die from diarrhoea and15 times less likely to die from pneumonia(Ministry of Health and Family Welfare).

Manage Severe Acute Malnutrition(SAM)

SAM Children are 9 times more proneto death and gut infection. Lancet says68% child deaths are attributable tomalnutrition and SAM is a majorcontributor. About 85% of the SAMchildren can be treated at the community, while the remaining 15% SAM childrenwith medical complication, would needtreatment of about 14 days in nutritionrehabilitation centres (NRC).

Reduce anaemia across all agegroups with deworming

Almost half the number of children, adolescents and women of reproductiveage group are anaemic in India. RegularIFA supplementation coupled withdeworming and intake of high proteincontent diet and iron rich food would helpeliminating anaemia.

Successful reduction in the prevalenceof anaemia in women will improvepregnancy outcomes for mothers andinfants, resulting in intergenerationalbenefits for individual health and well-being. "Anaemia Mukt Bharat" is seen asa ray of hope in reducing anaemiaprevalence in India.

Ensure use of toilet, drink safe waterand wash hands with soap

Lack of access to water, sanitation andhygiene (WASH) can affect a child'snutritional status in many ways- viadiarrhoea diseases, intestinal parasiteinfections and environmental enteropathy. There is increasing evidence whichshows strong correlation between opendefecation and height of children (Esrey,1996; and Dean Spears, 2012a).

Frequent episodes of diarrhoea in thefirst 2 years of life increase the risk ofstunting and can impair cognitivedevelopment (Grantham-McGregor,2007; & Victora et al., 2008).Undernourished children have weakenedimmune systems, which make them moresusceptible to enteric infections and leadto more severe and prolonged episodesof diarrhoea (Caulfield et al., 2004).

Address child marriage andadolescent pregnancy

Under-aged mothers are likely to givebirth to low birth weight (LBW) babies, which causes undernutrition and delayedphysical and cognitive development(UNICEF, 2009). In India, about 15 lakhgirls are married before 18 years everyyear (UNICEF). Compulsory and freeeducation of girls at least up to 18 years, would prevent the child marriage rate, which is now 27% (NFHS-4).

Address malaria prevalence incommunity for reduction

Children with malaria are more likely tobecome dangerously malnourished.Severely malnourished children may havea high parasitaemia, while showing a fewor none of the classic signs of malaria.Malaria can be reduced and prevented inthe community through medication, activesurveillance, use of bed nets and keepingsurroundings clean.

Ensure education of girls at least upto 18 years

Currently, only 56% of girls of 16-18years are enrolled in class 11-12 in highersecondary schools. If there is a magicbullet for social and human development(including health and nutrition), whatcomes close to it is girl's education atleast up to 18 years.

Improve other social determinantsIn addition to the interventions

mentioned above, situation under othersocial determinants - agriculture and foodsecurity, social safety net, womenempowerment, and family planning - needto improve (lancet 2013).

Major schemes and programmes toaddress malnutrition:

Integrated Child DevelopmentServices (ICDS) Scheme: ICDS coverschildren below 6 years and women andprovide six services- supplementarynutrition, pre-school non-formaleducation, nutrition & health education, immunization, health check-up andreferral services.

National Health Mission (NHM): Apartfrom health services, NHM providesnutrition related services like - a)Treatment of SAM children throughNutrition rehabilitation centers; b) Bi-annual Vitamin-A supplementation forchildren of 9 months to 5 years; c) MAA(Mother's Absolute Affection), facility andcommunity-based interventions toimprove breastfeeding; d) Home-basedYoung Child Care (HBYC) with homevisits by ASHAs for children up to 2 years;e) Janani Shishu Suraksha Karyakram, which provides for medicine, diagnostics, referrals and nutritious diet to pregnantwomen and children of 1 year of age; andf) Integrated Diarrhea control campaign.

Anemia Mukt Bharat (AMB): AMBaims is to reduce anaemia across all agegroups with life cycle approach and multiministerial programme with 6x6x6strategy (6 age groups, 6 Interventions, 6institutional mechanisms). Interventionsinclude - a) IFA for children of 6-59months, all school going and out-of-school children, and all women of 15-49years; b) Deworming twice in a year forchildren 1-19 years and women of 15-49years and once for pregnant women; andc) Behaviour change communicationcampaign.

POSHAN Abhiyan: Prime Minister'sOverarching Scheme for Holistic Nutrition(POSHAN) Abhiyan or National NutritionMission, launched in 2018, to improvenutritional outcomes of children, pregnantwomen and lactating mothers.

Mid-Day Meal (MDM): Launched in1995, MDM provides hot-cooked mealevery day (except on school holidays) tochildren in class 1-8.

National Food Security Mission:

National Food Security Mission set up in2007 and National Food Security Act(came into effect in July 2013), aims toprovide subsidized food grains to abouttwo-third of India's 1.3 billion people, toaddress food and nutritional insecurity

Public Distribution System (PDS):This is to improve household food securityto some extent.

Swachh Bharat Mission (SBM): SBMand Swachh Bharat Abhiyan (SBA),initiated in 2014, aims to eliminate opendefecation and improve solid wastemanagement.

Pradhan Mantri Matru Vandna Yojana(PMMVY): The programme provides cashincentives for improved health andnutrition of pregnant and lactatingmothers. Pregnant women will get Rs.5000 in three instalments, subject toregistration of early ANC, receiving oneANC and completing first cycle of childimmunization.

Janani Suraksha Yojana (JSY): Itaims to improve institutional deliveries byproviding cash assistance to pregnantwomen - Rs. 1400 in rural areas and Rs1000 in urban settings.

Impact of COVID on malnutrition: The socio-economic impact of COVID

will worsen malnutrition, anaemia, foodinsecurity and poverty, especially forthose in the vulnerable and marginalizedfamilies. Recent Lancet paper says thatthere would be 14.3% increase in wastingin the world. COVID and the lockdownhave adversely affected the servicesmentioned in section 4 for eliminatingmalnutrition. COVID pandemic will turninto a crisis for children, if the services forthe children are not resumed fully.

Seven Myths about Malnutrition1: Malnutrition is always due to lack

of food: Not true. Inadequacy offood is not the sole reason formalnutrition and malnutrition is notsame as hunger. Many children infamilies with plenty of food, are alsomalnourished. Food of course isimportant, but the major causes formalnutrition like - poor sanitation, child diseases, sub-optimalbreastfeeding practices andanaemia.

2: Poverty is the major cause formalnutrition: Not necessarily. Many children in non-poor families, are also malnourished. Yes,nutritional level may improve witheconomic development, but otherinterventions, mentioned undermyth-1, will yield better results ineliminating malnutrition.

3: Malnutrition begins when thechild is born: Not correct, malnutrition sets even before thechild is born. Malnutrition needs tobe addressed in the first 1000 days-from 270 days of pregnancy ofmother to the first two years of thechild.

4: Malnutrition is not a disease andnot a serious condition: No.Malnutrition is a seriouscondition/disease, which is the rootcause of 68% of child deaths inIndia. It affects the quality of life andproductivity in later life. Severeacute malnutrition (SAM) childrenwith medical complications needstreatment in nutrition rehabilitationcentre (NRC) for 2-3 weeks.

5: Giving formula food in the firstsix months will help the childgrow stronger: False. An infantup-to six months needs only breastmilk, not even a spoon of water orcow's milk. Food introduced before6 months could lead to infectionamong children. Exclusivebreastfeeding would reducediseases, malnutrition and deathsamong children.

6: Healthy foods are expensive andnot affordable: No. There areseveral misconceptions on healthyfood. Many think healthy foods areonly dry fruits, cake, fruits like apple, milk products, formula food etc.Locally available foods are alsonutritious.

7: There is no correlation betweenchild malnutrition and childmarriage: Not true. Child marriageis a major cause for childmalnutrition. A child bride is likely togive birth to baby with low birthweight (less than 2.5 kilogram),which causes malnutrition anddelayed physical and cognitivedevelopment of children.

DIFFERENCE BETWEEN ADIETICIAN AND ANUTRITIONIST

Nutrition and Dietetics is thescience of management ofnutrition and food for thepromotion of health amongpeople by instilling healthyeating habits. With an increasingconcern about lifestyle, healthand eating habits, the field ofNutrition and Dietetics is gettingpopular.

Dietitians can work in both thecommunity and the hospital. They evaluate, improve andeducate patients, nurses, doctors, community groups andhealth professionals about thedietary needs, and the healthystyle of life. They also work withpatients suffering from diseases,

such as kidney problems, foodallergies, cancer, diabetes etc.

While nutritionist and dietitianare very similar professionals, there is a slight differencebetween these two food & dietexperts. Anyone who offersnutritional advice on aprofessional basis can be calleda professional nutritionist. To becalled a certified nutritionist, theyneed to have a certification fromClinical Nutrition Certification(CNC) Board.

For becoming a dietitian, oneneeds to have the requiredacademic qualifications degreeand pass an exam to getregistered. That makes them aRegistered Dietitian (RD) fromIndian Dietetic Association (IDA).This is required by mosthospitals.

Many people use thenomenchature, Dietician, andNutritionist, inter-changeably buttheir job roles and educationalqualifications are different.Nutritionists are professionalswho advise people on the typesand quantity of food intakerequired specifically to increaseor decrease their body weight.

On the other hand, dietitiansare experts who guide patients(medical patients in hospitals, nursing homes, etc.) regardingthe right kinds of nutritionalintake. For example, a personrecovering from an accidentwould have differentrequirements from someonesuffering from diabetes, hypertension. They arespecialized in nutrition fordifferent categories of people, especially clinical patients.

Dieticians can work asNutritionists but Nutritionistscannot work as Dieticians.

WHAT DOES ANUTRITIONIST DO?
