Honoring Those Who Have Faithfully Served Us
BENEFITS OF A VETERAN ID CARD DD-214 INFORMATION
VETERAN ID CARD REQUIREMENTS
Your Veteran ID Card serves as proof of
honorable military discharge.
More convenient to carry a card.
Serves as a valid form of voter
identification.
Can be used to assist with obtaining
eligibility for hospital, burial, service
credit benefits, store discounts, and
more.
The Veteran ID Card is FREE of charge
for eligible veterans.
If you show your wallet-sized Veteran ID Card
issued by the Montgomery County Recorder’s
Office, you will receive year round discounts
from businesses not only in Montgomery Coun-
ty, but also across the state of Ohio and the
United States.
A list of businesses that offer veteran discounts
will available to view on our website:
www.mcrecorder.org by clicking the ‘Veteran
ID Card Program’ tab.
If you do not have your DD-214, you MUST
call (937) 225-4801 to schedule
an appointment with the Montgomery
County Veteran Services office to request a
copy. Their office is located on the 4th floor of
the East Medical Plaza Building at:
627 Edwin C. Moses Blvd., Dayton, OH
45417.
Once you have received your DD-214, you
are able to record it in the
Recorder’s Office FREE of charge.
Upon recording, the original DD-214
must stay in the Recorder’s Office over-
night and may be picked up or mailed the
following business day.
A certified copy of your DD-214 will be
provided at the time of recording.
At no time is the information from your
DD-214 displayed on public record.
Under the Ohio Revised Code, any veteran
who presents a DD-214 identifying a discharge
type other than dishonorable shall be eligible
to be issued a Veteran Identification Card.
In addition, veterans MUST bring two of the
following forms of identification:
Birth certificate
ID card from the U.S. Department of
Veteran Affairs
U.S. military ID
Social security card
CCW license
Ohio drivers license
State issued ID
Valid U.S. passport
*This Veteran ID Card does not independently
qualify a veteran for benefits offered by the U.S.
Department of Veteran Affairs (VA). The VA
retains sole discretion in determining eligibility for
their benefits.
