DEAN’S GREETINGIf there was a recurring theme for the 2008-2009 academic year, it was, “How much

is the budget going to be cut?” The question pervaded everything we did. Moreimportantly, we were forced to ask ourselves, “What are we going to do about it?” In thefinal analysis, we have a mission to educate, investigate, and serve. We will not shrinkfrom that mission.

We address the “what to do” in some detail in this 2008-2009 Annual Report toInvestors. In a feature article that follows, I outline the thinking behind our decisionsand strategies for the future. Some decisions were easy, others were not. In other arti-cles throughout the report, we describe strategies we are pursuing to renew, sustain,and grow, and enrich our people, programs, and facilities.

The past year had some great successes, too, and we refused to let tough economictimes cast darkening shadows on those successes. We completed and dedicated theW. Baxter Perkinson, Jr. Building. Patients enter through a newly implemented valetparking service. Students, faculty, and staff occupy its clinics, classrooms, and labora-tories. We finished restructuring the clinical curriculum, now fully competency-basedand integrated in five groups of D2, D3, and D4 students.

I hope you will enjoy reading more about the school, its activities, and its accomplish-ments in this Annual Report to Investors. I especially want to thank the many, manygenerous individuals and organizations listed in the annual Honor Roll of Investors.Their gifts in support of our people and our programs have never been more important.We could not have achieved our success without them.

There is never “no more money.” The challenges are where

to find it and where to use it most wisely. This year, VCU School

of Dentistry faced some significant budget cuts and made

some tough choices. These are very difficult economic times.

We could have chosen to retreat, slash internal budgets, down-

size or eliminate programs, delay or abandon construction proj-

ects, or lay off personnel. Instead we chose a different path.

We chose to regroup, reorganize, and rethink our operation. We

chose to sustain strong programs, increase revenue streams,

and enrich and grow other programs responsibly.

When the 2008-2009 academic year started, few could

have predicted the swiftness or severity of the U.S. and global

economic crisis. As increasingly serious revenue shortfall

predictions and projected cuts came from Governor Tim

Kaine’s budget office, we realized the inadequacy of routine

belt-tightening measures, such as mandatory travel restrictions

and hiring freezes. More severe actions, however, threatened to

damage the school’s programs and to hurt the people we

serve. An infusion of federal stimulus funds into Virginia

postponed for two years the harshest, immediate cuts

of potentially 20 percent or more. We expect those

cuts will come, however, so we will use these two years to repo-

sition the school financially.

Importance of Programs & People

Because Virginia needs more practitioners, we couldn’t cut

enrollment. Because we must support our students’ educa-

tion and patients’ safety, we couldn’t cut faculty or staff. Our

mission is to prepare graduates who can provide oral health

care services, generate new knowledge, and provide oral

health care services to the public. Our education, patient

care, and research programs are vitally important to that mis-

sion. We also pledged to increase enrollment in our DDS and

dental hygiene programs, and expand our research programs,

with the opening of the Perkinson Building.

Our Revenue Streams

The school’s main revenue streams consist of state appro-

priations, student tuition and fees, and patient care clinic fees.

Grants and contracts, charitable gifts, and endowment payout

contribute smaller, but nevertheless important, amounts to the

overall operating budget. To offset cuts in state funding, we

attempted to tap all the other streams as potential sources of

increased revenue. We analyzed each stream’s potential for

increasing revenue and devised strategies to maximize their

contributions.

Tuition and fees do not flow directly or fully to the school.

Budget cuts, it is important to note, are applied to both state

appropriations and tuition and fees. The expected 20 percent

cut to our budget is more than $3 million.

Tuition

For many years, tuition levels at our school have remained

consistently lower than those of peer dental schools. Out-of-

state students pay significantly more than Virginia residents.

Two summers ago, to generate much-needed revenuesand maintain teaching faculty positions while paying for costoverruns on our new building, we combined the strategy ofenrolling more out-of-state students and differentially increas-ing nonresident tuition.

This strategy caused concern in some sectors of the dentalpractice community, most notably Northern Virginia and

Southwest Virginia. In the face of vigorous complaints that weare not admitting enough Virginia students, we shifted more posi-tions in the DDS class to resident status, leading to additionaltuition increases this past summer, especially for out-of-state stu-dents. We regret the tuition increases, but note that even with theincreases, tuitions at VCU still remain lower than those of ourpeers. Moreover, the return on the investment for our studentsremains excellent.

Patient Care Fees

The School of Dentistry is the only school at VCU with itsown clinical enterprise and the only school with the ability togenerate its own clinical revenue. The other health profes-sions schools send their students to hospitals and clinicsowned and operated by others for their clinical education.This unique situation provides a great opportunity for us. Therevenue generated through our student clinics, however, hasremained stagnant for several years, even as we increasedfees slightly. In addition, as part of the practice managementcurriculum, students must track their productivity, accountsreceivable, and unpaid balances. Recognizing the potential toboost clinical revenue to offset state funding cuts, we look forways to reinforce the need for efficiencies and productivity.Our financial management will improve significantly when weimplement a new clinic management system later this fall.

Research Grants To increase our potential for additional external research

funding, the expansion of the VCU Philips Institute for Oral and

Craniofacial Molecular Biology onto the fourth floor of the new

Perkinson Building continues on schedule. Four research fac-

Elsewhere in this report, we describe ways we strength-

ened our already robust continuing education program and

preceptorship program (Page 13).

To fulfill our mission, to use the new building’s resources,

and to meet the needs of students, patients, Virginians, and

society, many of our programs must grow. The challenge is

to grow these programs responsibly in this time of austerity.

Increasing or redirecting our revenue streams helps this

effort. Elsewhere in this report, we outline our plans for

growth in enrollment and faculty positions (Page 4), research

(Page 12), and information technology (Page 19).

The current economic climate creates

challenging, yet rewarding, times for leaders in

dental education. At VCU, we are fortunate to

have the unwavering support of university and

school leadership, the expertise of our faculty

and staff, and the intellectual curiosity and youthful vitality of

our students. The times may challenge us with financial

constraints, but as we confront

Charitable Gifts

Charitable gifts and endowments depend on the generosity

of alumni and friends who philanthropically support our efforts

to educate, investigate, and serve. (See Honor Roll of

Investors, Page 22.) The total dollar amount for gifts received

has remained fairly flat over the past three years, yet our needs

keep growing. Because of the great importance of charitable

giving to the school’s future and to the success of its programs

and people, I have restructured our advancement office, plac-

ing myself as the head of the advancement team. We will

improve the way we acknowledge, inform, and engage our

donors.

Sustaining and Growing ProgramsMany of our programs are doing well, and we seek to sustain

them at that level. For example, year after year all of our grad-

uate programs attract strong applicants, provide excellent edu-

cation, and produce well-prepared graduates. The dental fac-

ulty practice attracts a strong patient base. Faculty may choose

to use the practice to keep their clinical skills up-to-date and

add supplemental income to their state salaries. The ability to

supplement salaries is critical to recruitment and retention of

excellent faculty.

Adjunct FacultyAs the time that our students spend in the

clinic with patients increased, the school neededto recruit additional part-time faculty. A concert-ed recruitment effort in the fall semester, spear-headed by Dr. Jim Burns (D ‘72), associate deanfor clinical dental education, attracted 40 newpart-time adjunct faculty members from the prac-tice community.

For adjunct faculty, the rewards can be tangi-ble. The school recognizes their contributions oftime and expertise with free parking, certificatesto hang in the office, reduced fees for School ofDentistry-based continuing education, and anannual thank you celebration at The JeffersonHotel.

Full-time FacultyThis year, the school experienced unusually high

faculty turnover because of retirements and losses tograduate programs. This turnover, combined with theneed for more faculty to meet the demands of expand-ed enrollment, created more openings than usual.Nonetheless, all the vacant positions were successfullyfilled by the start of classes.

Faculty shortages in U.S. dental schools are well-documented nationwide, as dental education competeswith the private sector. Nonetheless, VCU continues tofind excellent candidates, successfully recruiting themto Richmond and our school.

Many of our new hires are from other countries,have dental degrees from those countries, butadvanced education from accredited programs in theUnited States. We celebrate the cultural diversity theseindividuals bring to our school and to VCU.

New GraduateBecomes PublicHealth Dentistand Teacher

“Everything! I like the work, the people I workwith, and the community,” said Dr. Risa Odum(D ’09) when asked what she likes best aboutbeing the staff dentist at the Community DentalClinic in Martinsville, Va. “I am doing what I loveto do, in a great place with amazing people.Everybody has been so kind to me. They reallyhave taken me under their wing and are helpingto make my transition a smooth one.”

When Dr. Odum entered the VCU School ofDentistry as a first-year dental student four yearsago, she already knew she wanted to be a publichealth dentist. Growing up in the small town ofTazewell in Southwest Virginia, she recognizedthe shortage of rural dentists and the tremendousneed for care. She also knew she didn’t want toworry about anything other than taking care ofher patients.

“If you have the heart for it,” Dr. Odum said,“public health dentistry is a great way to go.”

Working as the staff dentist, however, is not herfirst experience at the Martinsville site. Dr. Odumfondly remembers her extramural rotation at theclinic as a fourth-year dental student last year.

“In addition to having the opportunity tointeract with the students, teaching at theschool also allows me to continue buildingrelationships with my colleagues and other fac-ulty,” said Dr. Clay Weisberg (D ‘05). “It’s greatto see the changes that have occurred. I lookforward to being a part of both the dentalschool and the dental profession, now and inthe future.”

Dr. Weisberg, a busy young practitionerdrives to Richmond from Virginia Beach to helpin the school’s clinic, also volunteers at the RedCross Clinic in Norfolk, one of the school’sextramural sites.

“There is never enough time for every-thing,” he commented, “but you make time forwhat is important.”

Dr. Burns emphasized that mid- and late-career practitioners who have stable and pro-ductive careers can move from “success” to“succession”. “I think many of our part-timerssee teaching as a way to have an effect andbe a positive influence on the lives of the nextgeneration,” he said.

care in a public health setting. While on rotation inthe small town of Martinsville, the students live ina dormitory-like setting constructed by the com-munity specifically for them. The comfortable andattractive living space is well-stocked with bever-ages and snacks for the students’ comfort andconvenience. In the clinic, they work with state-of-the-art equipment and keep 45-minute appoint-ments, just as if they were in private practice.

”It enriches their education tremendously,”Dean Ron Hunt explained. “They learn aboutunderserved populations, public health, and therationing of health care. Our hope is that if wesend our students to places outside the walls ofour clinics in Richmond, they will learn not onlyabout dentistry but also about life in other parts ofthe state and will consider the possibility of livingthere.”

“Last year I was a student here, and now I’mthe dentist,” Dr. Odum said. “I really enjoy work-ing with the students. The most surprising thingto me is that even though we all came from thesame school, took the same classes, and had thesame instructors, everyone is different. Each per-son approaches every situation a little differentlyand that’s exciting to see.”

Dr. Odum’s favorite patient is a 15-year-old recentimmigrant from Mexico. His parents don’t speakEnglish, so he translates for them.

“His mother said they didn’t have the money totake care of his teeth when he was younger, but shewants to make things better. Every tooth in his mouthhas caries,” Dr. Odum explained, “so I am rebuildinghis smile one tooth at a time. He never complains, isalways polite and very respectful. After I work with apatient like him I think to myself, ‘This is what I wasmeant to do.’”

“For students, it will be a great experience,” she com-mented. “They will have a great patient pool. People willbe so thrilled to have access to care. I think for students,an experience like that can inspire them to commit,strengthen their call to service.”

Graduation is a long way away for a first-semester,first-year student, but Emily considers the possibility ofreturning home. “I want to work somewhere where I amneeded,” she explained. “What I do is more importantthan where. Geography doesn’t matter.”

“I think the worst thing about Richmondis the traffic and the parking,” she said. “Athome there is one traffic light.”

As for parking, in a small town likeDuffield, population 62, it is never a prob-lem. In fact, Scott County, the county Emilycalls home, has a total population of about5,000 people.

“There is so much need in SouthwestVirginia,” she explained. “I realize many peo-ple here at VCU know this first-hand becauseof the Missions of Mercy projects. I have vol-unteered at them, and for me it really hit homebecause many of the patients were people Iknew personally. The people waiting at themissions were people I knew who didn’t haveinsurance and needed care. The experience was verytouching for me. It gave the access-to-care problem aface.”

Student Recruitmentand Satellite ClinicAre Linked to RuralDentist Shortage

“I miss the mountains and the trees,” said first-yeardental student Emily Bowen, who grew up in the smallSouthwest Virginia town of Duffield in Scott County, morethan a six-hour drive from Richmond. The VCU School ofDentistry is looking for more students like Emily.

For the past two years, the school has actively recruit-ed students from rural parts of the state. As a result, theclasses entering in 2008 and 2009 each had 15 studentsfrom Southwest and Southside Virginia.

“Our hope is that if we recruit students from theserural areas, some of them will return home and pursuetheir professional careers there,” said Dr. Ron Hunt, deanof the VCU School of Dentistry.

Emily’s hometown is about 30 minutes from theUniversity of Virginia’s College at Wise campus, nearwhere the new VCU Satellite Dental Clinic will open inspring 2011. Five members of her class will work in thesatellite clinic during their senior year, an idea she thinksis fantastic.

ResearchScientific discovery ultimately

Through a vari-ety of extramural and intramural fundingsources, the School of Dentistry’sresearch enterprise remains healthy andviable.

To increase our potential for addition-al external funding, we will nourish andexpand our research programs in theVCU Philips Institute. Last year, VCUallocated more than $250,000 to thedental school for the purchase of much-

needed additional research equipmentfor the institute’s new labs in thePerkinson Building.

Two cross-campus collaborations willfurther the research and will fosterimportant interprofessional activity. Aswe expand research in head and neckcancers with the VCU Massey CancerCenter, we also will build a new researchprogram in tissue bioengineering in col-laboration with the VCU School of

Refocusing theContinuingEducation Program

Our continuing education program remains robustand vibrant, but has shifted its offerings to capitalize onits strengths and new opportunities. As dental soci-eties stepped up their traditional lecture course offer-ings, we shifted to more hands-on participation cours-es, such as the MasterTrack Program and local anes-thesia and nitrous oxide courses for dental hygienists.In the coming year, wewill develop hands-oncourses for the DentalAssistant II positionbeing created by theVirginia Board ofDentistry.

Beginning this year,the continuing educa-tion program willreduce costs by host-ing more courses inthe new Delta Dentalof Virginia ConferenceFacility in thePerkinson Building. This facility includes a large,comfortable lecture hall, adjacent conferencerooms, a pre-function area, and fully functionalcatering kitchen. When the spaces are not beingused for continuing education programs, theymay be used for classes and school events.

Teaching,Learning,Reflecting atExtramuralSites

Our students have benefitted fromcollaboration with local dentists since2004 when they first began extramuralrotations in Lynchburg, Va. Public healthclinics throughout the Commonwealthnow serve as extramural sites for clinicalexperience for fourth-year dental anddental hygiene students. As they gainclinical experience, they also gain publichealth experience while meeting the oralhealth needs of uninsured, underinsured,and underserved people in variousregions of the state.

The pace at some of these clinicschallenges students to see many morepatients each day than they see in theschool’s clinics. This faster tempoincreases their productivity and pre-pares them for graduation and entry topractice by building confidence andencouraging independence at an appro-priate time in their development. Thepublic health experience, when com-bined with the support of staff, volunteerdentists, and the communities, fosterspositive learning.

“While they are at the sites, we askstudents to keep journals and reflect on

the clinical, behavioral, and socialaspects of their experiences,” explainedMs. Kim Isringhausen, director of theprogram. “We ask students to writeabout how serving in the clinics chal-lenges assumptions and consider howthis service might change the way theywill practice dentistry in the future.”

Our students say they learn to workfaster and gain confidence they willneed as they transition into practice. Atthe same time, they help the clinics meetthe oral health needs of the communityin a public health setting. It is a quintes-sential win, win, win. The clinic wins, thestudents win, and the patients win.

Some members of the graduatingClass of 2009 felt so positive about therotations that they donated much of theirclass gift to enrich the student experi-ence at the various sites.

“We couldn’t ask for a better testimonial for the

significance of this program,” Ms. Isringhausen

said. “The class gift canhelp sustain the programthrough lean times, and

then when things get better,it will enrich the rotations

for the students.”

ms Zimmer Dental Education Grant

Expands Implant ExperiencesTeaching students about dental implants today prepares them for the dental practice of

tomorrow. As state-of-the-art treatment for single tooth replacement and denture retention,implants now are an important element in contemporary dental practice.

“The Dental Implant Program at the VCU School of Dentistry continues to grow everyyear as more and more DDS students provide implant treatment for our patients,” said Dr.John Unger, chairman of the Department of Prosthodontics. “The percentage of our stu-dents who have the implant experience is growing. They completed approximately 80implant crowns for our patients in the past year, along with many implant-retained dentures.”

Last year, to help the school provide more implant experiences for students, ZimmerDental awarded the school an educational grant that allowed us to more fully develop ourdental implant program and implant clinic. The funding also helped create a preclinicallab component and expand both clinical and non-clinical aspects of teaching dentalimplantology.

At VCU students begin their education in dental implants in the second year by attending lectures on thevarious aspects of dental implants, including the biological, surgical, and restorative issues involved in suc-cessful implant treatment. Zimmer Dental also sponsored an all-day program on dental implants that com-plements the implant curriculum. In addition to the lectures, the students participate in preclinical experi-

ences that cover all phases of implant therapy from surgical place-ment to final restoration of single teeth and the fabrication of implant-retained overdentures. In the third and fourth year, students get first-hand experiences in all phases of dental implant treatment as theyplan and restore dental implants under the supervision of faculty fromOral and Maxillofacial Surgery, Periodontics, and Prosthodontics.Students participate in the diagnosis and treatment planning. Theyprepare the surgical guide for the surgeon and observe the surgicalplacement. They do the restoration phase themselves.

Students and faculty love the program. Students seek out addi-tional opportunities to become involved. They say the implant clinicoffers them practical opportunities to understand the treatment plan,design, and length determination processes in implant placement.

Kevin Rasmussen (D 2010) had two patients go through the clinic.“My complete overdenture patient experience was extremely satisfy-ing,” he said. “My patient had particularly poor experiences in her

dentures previously. With implants she left happy, having a lower denture that actually stayed. My otherpatient had a single tooth implant, and it has also been rewarding to see a satisfied patient.”

Foundation GrantsPromote Diversity

Racial and ethnic diversity enhances cultural sensitivity andcompetence among all dental students and better preparesthem for an increasingly diverse workforce and society. Thiscompelling interest in student diversity makes the recruitmentof underrepresented minorities a high priority for the school.

Prior research has reported on the value of minoritypractitioners noting that minority patients tend to seekout minority healthcare providers, however, the per-centage of dentists in Virginia or dental students in ourschool who are minority lags well behind the percent-age of Virginians who are minority. For example, theAfrican American population in Virginia exceeds 13percent, but less than 5 percent of our students comefrom this ethnic group.

To promote diversity and to support underrepre-sented minority and disadvantaged applicants, Dr.Carolyn Booker, assistant dean for student services,sought external support through foundation grants.Two foundations provided funds for academic enrich-ment programs:

• Pipeline, Profession and Practice: Community-Based Dental Education – Round 2 - VCU RAMpS : the Robert Wood Johnson Foundation, based in New Jersey

• Reaching Academic Milestones Through Participation and Service: the Obici Healthcare Foundation, based in Suffolk, Virginia

The past two summers, students participated in afive-week full-time program that took them throughintensive instruction in the basic sciences, simulationlaboratory practice, academic skill development, andseminars on the admissions process. A major pur-pose of these programs is to enable students to havea better understanding of the rigors of dental schooland to be better prepared for the dental schooladmissions process.

“Of course, we further hope that once prepared,these students will choose to come to VCU,” said Dr.Carolyn Booker.

Perkinson BuildingThis summer, the School of Dentistry dedi-

cated, opened, and occupied the W. BaxterPerkinson, Jr. Building. Contractors workedfeverishly to complete the four-story buildingon schedule, despite unanticipated site andinfrastructure problems and required designchanges.

The’s school’s third building, located at1101 E. Leigh Street, allows valet parking andoff-street patient drop-off and pick-up througha side entrance on 11th Street. It providesadditional clinical operatories, classrooms,seminar rooms, research labs, and facultyoffices.

The Perkinson Building enhances thecapacity of the school to achieve its missionthrough increased enrollment in the DDS anddental hygiene education programs, expand-ed research in head and neck cancers, andnew research in tissue bioengineering.

Plans for implementing digital oral radi-ography and a fully electronic dentalpatient record moved significantly forwardthroughout the past year. An aggressiveand ambitious timeline calls for going“live” with all the new digital technologyimmediately after the Thanksgiving break.

“The final product, a paperless patientrecord with digital radiographs, willimprove the service we provide to ourpatients and will teach our students howto use the most current technology avail-able,” said Ms. Rebecca Pousson, execu-tive associate dean.

During the past several months, teamsof faculty and staff in InformationTechnology, Oral Radiology, andAdministrative Services have tested, eval-uated, and priced a variety of systemsand equipment. They have met with salesrepresentatives, attended product demos,and consulted with users of the equip-ment.

Key questions included where tolocate digital radiography equipment,which image sensor systems to use,which clinic management system to use,and whether to develop a customizedelectronic dental record or purchase anexisting one.

“Our teams worked tirelessly to makeall the systems fuse seamlessly into onecoherent piece,” said Dean Pousson.“The process has not been without chal-lenges, but I am certain the end result willbe well worth the effort.”


The Digital Process...Steps Completed

• Identified and gutted an area on the Wood Building first floor for a radiology “temporary clinic.”

• Developed architectural plans for the temporary clinic.

• Developed architectural plan for a fully renovated oral radiology clinic on the Wood Building first floor

• Installed additional x-ray tubeheads in the Lyons Clinic (6), Wood Clinic ( 9) and Douglas Clinic (7)

• Purchased phosphor plate film systems and placed Optime scanner units for them in the Wood, Lyons, and Douglas clinics.

• Purchased MiPacs imaging software for image storage, security system, viewing template, and integration with the clinic management system.

• Purchased AxiUm’s clinic management system and electronic dental record system for all school clinics except Orthodontics, which has been fully digital for many years.

• Selected the Dexis direct digital sensor as the main system for the central Oral Radiology Clinic.

• Purchased two panorex systems: Gendex for Oral and Maxillofacial Surgery and FDP Instrumentarium for Central Radiology.

• Installed a Cone Beam Computed Tomography system on the Lyons Building second floor near the Orthodontics Clinic and Holcomb Oral Surgery Clinic.

A clear sunny spring day greeted teams participating inthe 4th Annual VCU School of Dentistry Golf Tournament.Special guests VCU President Eugene Trani, VCU RectorThomas Rosenthal, and VCU Vice President for HealthSciences Sheldon Retchin welcomed students and alumni towhat has become a reunion weekend tradition at theCrossings Golf Course.

As Dr. John Doswell, VCU Board of Visitors member andhonorary tournament chair, presented Dr. Trani with a com-memorative School of Dentistry ball marker, VCU's formerpresident mentioned that he has a collection of more than10,000 ball markers from golf courses all over the world.

Before players teed off, members of the VCU Rams golfteam showed off the shiny new black and gold team van.This year’s Rams won first place in their conference and gra-ciously agreed to be on hand to give our golfers some tradesecrets.

The student-organized benefit event, held in honor of Dr.Frank Filipowicz, raised $32,000 to support student involve-ment in the Virginia Dental Association’s Missions of Mercy .

“The goal of the event,” said Dean Ron Hunt, “is to have agood time, link students with alumni, and foster a valuablealumni network.”
